Indwelling self-retaining ureteral stents are important for the maintenance of urinary drainage in the ureter threatened by obstruction by neoplasm, edema, or calculus. Proximal migration of the stent into the ureter prevents simple cystoscopic retrieval and necessitates removal by other techniques. In this series both rigid and flexible ureteroscopes were used to remove stents from locations ranging from the renal pelvis to the distal ureter in 16 patients. Three-pronged wire graspers was the retrieval instrument most frequently used. Stents were removed in all patients without complication. Ureteroscopy provides the most convenient and least invasive technique for removing stents proximally located in the upper urinary system.

Prognostic factors in transitional cell carcinoma of the upper urinary tract were assessed with histopathological examination and flow cytometric analysis in a series of 127 patients operated upon between 1976 and 1988. In particular, we evaluated the usefulness of flow cytometry to identify patients who require adjuvant treatment among those with low grade and low stage disease (51% in this series). A multivariate analysis was done on 92 cases, considering patient age and sex, stage, grade and number of lesions (unifocal versus multifocal), site (renal pelvis versus ureter), presence of vesical tumors, recurrences along the urinary tract or in the bladder, type of operation and nuclear deoxyribonucleic acid (DNA) ploidy (diploid versus tetraploid/aneuploid tumors). Only the stage (p = 0.001), grade (p = 0.001) and, to a lesser extent, the DNA pattern (p = 0.031), as well as the number of lesions (p = 0.061) were determinant for prognosis. In regard to the subgroup of 41 patients with grade 2 or less, stage P1 or less tumors, no significant difference in survival was demonstrated between diploid and nondiploid tumor patients. However, 7 of 10 patients from the latter group are still under observation. Therefore, our conclusions may have to be modified in the future.

We measured epidermal growth factor (EGF) in the urine of 54 untreated patients with malignant tumors (7 prostatic cancer, 11 renal tumor, 31 bladder cancer, 3 renal pelvic tumor, 2 ureter tumor) in the Toyama Medical & Pharmaceutical University Hospital. We also measured EGF in the urine of 77 normal subjects (43 males, 34 females). Urinary concentration of EGF in normal subjects decreased with increasing age. It was significantly higher in females than males (p less than 0.05). Urinary concentration of EGF in patients with prostatic cancer or renal tumor was similar to that in normal subjects. The patients with prostatic cancer controlled by estrogen showed a slightly high level of EGF in the urine. In patients with renal tumor, urinary concentration of EGF decreased after nephrectomy. Patients with bladder cancer showed a significant decrease of EGF in the urine compared with normal subjects (p less than 0.05), and urinary concentration of EGF in the patients with bladder cancer of high stage was remarkably low. Low concentration of EGF in the urine was recognized in patients with renal pelvic tumor or ureter tumor. However, the relationship between the decrease of urinary concentration of EGF in these urothelial tumors and the growth of these tumors remains to be elucidated.

The symptoms of abdominal lymphangioma are unspecific and depend on site and size of the tumor. The most frequent symptoms consist of pain up to the clinical picture of a pseudo-appendicitis, digestive problems, palpable or even exterior visible tumors such as the pseudo-ascites. Often the diagnosis cannot be found by clinical examination or further investigation. As a differential diagnosis all mesenterial cysts as well as other cystic tumors which originate in an abdominal organ must be taken in account. In most cases possible complications make surgical intervention necessary through which an exact diagnosis of the actually asymptomatic tumor can be found. Lymphangioma can lead to obstruction of the intestine, the ureter or the bile-duct. Inflammations of the cyst wall, bleeding into the cyst, torsion of the cyst pedicle and rupture of the cyst occur. The only curative therapy is the total surgical removal. Prognosis is good and relapse seldom occurs. We report on a 4.5-year-old boy with a cystic lymphangioma of the mesosigma. In addition, we discuss cases from the literature of the past 15 years.

The multiplicity of transitional cell carcinomas in the renal pelvis, ureter and bladder was analyzed in terms of (1) tumor configuration, i.e., papillary, nodular cancers, (2) associated mucosal changes such as carcinoma in situ (CIS) and dysplasia and (3) the possible involvement of human papillomavirus (HPV) in the development of multiple papillary cancers in the bladder. The incidences of concurrent or subsequent bladder cancer in renal pelvic cancer and/or ureteral cancer cases were 7/31 (22%) for renal pelvic cancer, 17/28 (60%) for ureteral cancer and 10/15 (67%) for renal pelvic and ureteral cancer. In 33 cases of renal pelvic and/or ureteral cancer occurring since 1978, 67% of the papillary and 13% of the nodular cancers in the upper tract exhibited a simultaneous or later development of bladder cancer. In 211 cases of bladder cancer for which cystectomy was performed, 77% of the papillary cancers arose in multiple form, 57% being associated with CIS and/or dysplasia, whereas 72% of the nodular cancers developed singly, 55% being associated with CIS and/or dysplasia. No positive signals hybridizing to HPV types 1, 5, 6, 11, 16, 17, 18, 20, 33 and 38 were detected in any of 19 papillary bladder cancers and 13 specimens of normal bladder mucosa under conditions of low stringency, suggesting that HPV may not be a factor in multiple bladder tumor development. Definite findings from the present study are: (1) there is multiple development of papillary cancers but they remain superficial, whereas nodular cancers develop singly and are invasive, (2)) there was no steady tendency towards a relation between multiplicity and associated mucosal changes, (3) HPV was not, to our knowledge, involved in the multiple development of papillary cancers in the bladder.

A new technique of electro-incision of ureteral stenoses and strictured uretero-enteric anastomoses is presented. Incision is performed with a papillotome, routinely used to achieve endoscopic retrograde sphincterotomy of the duodenal papilla. Group I: 7 ureteral stenoses were performed, on a dog model after surgical ligation of the lumbar ureter. Ten days later, through a percutaneous approach, the papillotome was placed through the stenosis, deflected, and cutting current was applied to incise the stenosis. The IVP performed one month later showed disappearance of the stenosis in 4 cases, a residual stenosis without obstacle in 2 cases and a residual stenosis with obstacle in one case. Group II: 7 strictured uretero-enteric anastomoses on 6 patients. After placement of a percutaneous nephrostomy, a wire guided papillotome was placed into the stenosis. Cutting current was then applied to cut the stenosis. A 18 F ureteral stent was subsequently placed for 8 weeks. Two patients have a patent anastomosis, 7 and 10 months after removal of the stent. One patient died from metastases of bladder tumor. The three remaining patients are still stented. An urinoma occurred in one patient the day following removal of the stent, and was surgically drained. This new technique which combines electro-incision and stenting with a large caliber stent may be proposed as an alternative to surgery or balloon dilatation for the treatment of strictured uretero-digestive anastomoses.

We preoperatively diagnosed four cases suspected as malignant urothelial tumor of upper urinary tract using a rigid ureteroscope. In three cases of ureter tumor, biopsy was successful and showed low grade malignancy. Nephroureterectomy was performed in two cases with upper ureteral tumor. The tumor was fulgurated ureteroscopically in a case of lower ureteral tumor. In one case of renal pelvic tumor, biopsy was unsuccessful though the tumor was observed as a papillary configuration. Successively, biopsy using a flexible ureteroscope was performed and the tumor was treated by fulguration. Ureteroscopy is very useful for preoperative diagnosis of ureteral tumor and selection of treatment.

A patient with a tumor in the right pelvic ureter is described herein. Following diagnosis by ureteroscopy, the patient underwent TUR utilizing a ureteroresectoscope. No short- or long-term complications were observed. The scant number of cases in the literature and the advantages afforded by this technique (noninvasive, short duration of hospital stay, etc.) prompted us to report this case.

Of 9 patients found to have a nonopaque filling defect on intravenous pyelogram, 6 were shown to have radiolucent urinary calculi, 2 had intrapelvic blood clots and 1 had a ureteral tumor. The diagnosis was made by computerized tomography and confirmed by surgery in all but the case of ureteral tumor. We recommend CT scanning as the method of choice for unexplained filling defects in the renal pelvis and ureter or when a nonopaque calculus is suspected.

Early diagnosis of all the foci in multiple urothelial tumors keeps on being difficult. On comparison of the clinical and postmortem diagnosis the tumor of the renal pelvis remains unrecognized in about 31 per cent of the cases and the tumor in the ureter in more than 50 per cent. A case is reported of primary multiple urothelial tumor, for whose diagnosis all modern methods were used. In spite of all this, the focus in the renal pelvis was diagnosed four years after the onset of clinical manifestations of the disease. In this connection, the possibilities of the different methods of examination are discussed.
