Self-expanding metallic stents, frequently used in interventional radiology, were implanted into 15 obstructed ureters in 12 patients. The cause of obstruction was lymph node metastases of different malignant tumors or direct
tumor
compression of the
ureter
. Followup in 12 patients was 3 to 31 weeks. Immediately after implantation all stents drained well. During the initial 4 weeks slight obstruction occurred due to hyperplasia of regenerating urothelium. The stents were fully incorporated into the ureteral wall 8 weeks after implantation, they were covered by smooth epithelium and the hyperplasia had disappeared. Complications were hemorrhagia in 1 patient and incrustation in 2. No infection was observed. Complications due to the
tumor
were obstruction distal to the stent in the presence of a widely open catheter in 3 patients. Early results are presented. Late results are not yet available. The method seems to be minimally invasive and easy to perform.

A 78-year-old man visited our department for macroscopic hematuria in June, 1989. On the basis of the diagnosis of
tumor
of the bladder and right afunctional kidney, total right nephro-uretero-cystectomy and skin grafting of the left
ureter
were performed on August 2. The patient continued to have fever of unknown origin postoperatively. Repeat laparotomy, which was performed for rectal fistula on August 25, revealed that the abdominal wall, colon, small intestine and mesenterium adhered to one another, producing a mass and that two sites in the rectum were perforated. A part of the small intestine was excised, the perforated sites were sutured, and an artificial anus was created at the transverse colon. Since the patient had intermittent fever and continued to complain of abdominal pain after creation of the artificial anus, nosotropic therapy was continued. However, the patient died from cardiac insufficiency on October 10. Erosion and ulcer were histologically observed over a wide range in the excised small intestine. In addition there was a defect in one area of the small intestine, penetrating the tunca muscularis propria, in which many cytomegalovirus (CMV) inclusion bodies were observed. CMV inclusion bodies were also detected in the bladder with re-examination of specimens from the excised bladder. From these findings, it appears that endogenetic CMV may have been reactivated in the present case.

Immunosuppressive acidic protein (IAP) is a non-specific immunoreactive protein arising from inflammatory or malignant conditions in the human body. We determined the IAP levels in 65 cases with urological malignancies and in 31 cases with benign diseases as a control group during a 9-month period. There were significantly higher serum levels of IAP in cases of bladder transitional cell carcinoma (p = 0.025), prostate adenocarcinoma (p less than 0.00001) and upper urinary tract urothelial cancer (kidney and/or
ureter
, p = 0.013) as compared with those of the control group. Significant differences in IAP between different
tumor
stages were found in the bladder cancer group with high stage cases having higher IAP levels (p less than 0.0005). However, no significant differences were found between different
tumor
gradings. Most of the prostate cancer patients had extremely high IAP values (1,029 +/- 490 micrograms/ml) in this study. Renal cell carcinoma and testicular tumors showed no statistical differences from the control group (p = 0.89 and 0.37, respectively). No differences could be found in the different age groups (by decades) or sexes. The serum IAP level can be a good non-specific
tumor
marker for bladder cancer staging and probably a good follow-up tool for most urological malignancy patients.

With the extended use of transurethral endoscopic surgery, we modified the conventional nephroureterectomy that usually requires either a very long incision or two separate shorter incisions. From August 1983 to October 1988, 13 cases of renal pelvic

in this hospital were treated single-incision nephroureterectomy combined with transurethral incision of the bladder cuff. The advantages of this technique were less surgical time and a decrease in postoperative wound pain. There were no significant complications or local recurrence noted in this series. The proposed indications were urothelial tumors in the renal pelvis and upper
ureter
without demonstrable metastases.
Tumors
of an uncertain cell type preoperatively are especially indicated. The details of this technique are described.

To evaluate the distribution and density of epidermal growth factor (EGF) receptors (EGF-Rs) on urothelium, immunohistological studies using a monoclonal antibody to the binding portion of the human EGF-R were performed on frozen specimens of normal urothelium (N = 20), urothelium from patients with nonurothelial urological malignancies (N = 15) and inflammatory diseases (N = 8), low grade superficial transitional cell carcinomas (TCC) (N = 13), high grade superficial or invasive TCC (N = 28), and endoscopically normal appearing urothelium from patients with low grade superficial (N = 5) or high grade (N = 21) TCC elsewhere in the bladder (or ipsilateral renal pelvis/
ureter
). EGF-Rs are found only on the basal layer of epithelial cells (with scattered representation on intermediate cells) in 95% of normal urothelial specimens and 100% of pathological specimens without urothelial malignancy. Alternatively, 92.3% of specimens of low grade superficial TCC and 100% of high grade TCCs had EGF-Rs richly expressed on the superficial as well as the deeper layers of urothelium. This "malignant" distribution of EGF-Rs was also found on all specimens of endoscopically normal appearing urothelium in patients with TCC elsewhere. The density of EGF-Rs correlated closely with
tumor
grade on both "premalignant" and frankly neoplastic urothelium. We conclude that the expression of EGF-Rs on urothelium favors the interaction of premalignant and malignant tissue with urinary EGF. To determine if altering the physiochemical environment of urine could interfere with this interaction, the effects of pH on the binding of and growth responses to EGF were assessed on four human TCC cell lines. Scatchard plots demonstrated that varying pH from 5.0 to 7.5 did not significantly change the total number of receptors, but EGF-R affinity was reduced approximately 20-fold as pH decreased from 7.5 to 5 in each TCC target. Similarly, significant growth stimulation by EGF at pH 7.5 was abrogated at pH less than or equal to 7.0 while growth rates in the absence of EGF remained unchanged at lower pHs. It thus appears that urinary acidification may hold promise in the management and prevention of recurrent bladder cancer.

The ros1 gene was detected originally by virtue of its transforming potential; the cDNA of the human protooncogene was isolated from a
tumor
cell line expressing the gene ectopically. It encodes a receptor-type tyrosine specific protein kinase which is closely related to sevenless in Drosophila. Here we report the novel and remarkable in vivo expression pattern of c-ros1, which was determined in the mouse. By a combination of RNase protection and in situ hybridization, we find transient c-ros1 expression during development in the kidney, intestine and lung, coinciding with major morphogenetic and differentiation events in these organs. This temporally restricted nature of expression is unusual for tyrosine kinase receptors and suggests a role for ros1 during development. Furthermore, in kidney development c-ros1 transcripts are confined to subgroups of
ureter
cells known to be involved directly in inductive interactions between
ureter
epithelium and metanephric mesenchyme. Thus, this study implicates for the first time a tyrosine kinase receptor in mesenchymal epithelial interactions and suggests a molecular basis for these important inductive events in development.

Endoscopic ureterectomy is a minimally invasive surgical technique indicated in cases warranting ureterectomy to complement extended nephrectomy for pyelocaliceal urothelial carcinoma. The results achieved are identical to those of conventional ureterectomy (open surgery) without the inconvenience or sequelae that usually present with the latter procedure. In 1982, the authors developed the modified Dell'Adami and Breda endoscopic ureterectomy technique. To date, they have performed their modified technique in 10 cases: 9 with infiltrating urothelial or highly malignant pyelic or caliceal
tumor
and 1 with diffuse leukoplasia and cholesteatoma of renal pelvis. The technique described herein permits intussusception of the
ureter
into the bladder by transurethral traction of a Chevassu ureteral catheter to which that cephalad end of the
ureter
is attached during the extended nephrectomy procedure. Once intussusception of the
ureter
has been achieved, a resectoscope is used to cut the
ureter
at the level of the ureteral meatus. Thereafter the mucosa of the corresponding trigonal horn is completely resected. The results are highly satisfactory in patients whose
ureter
has not previously been submitted to open surgery and is not compromised by

spread.

The authors report a case of myxoid leiomyosarcoma of the kidney accompanying ipsilateral ureteral transitional cell carcinoma. A 74-year-old male patient complained of turbid urine and macroscopic hematuria. He also complained of left back pain, appetite loss and weight loss. Computed tomography revealed a large mass in the left retroperitoneum. Urine cytology disclosed two types of malignant cells, atypical spindle-shaped cells and transitional cell carcinoma. Left total nephro-ureterectomy was performed. The left kidney was occupied by a 6 x 4 x 4 cm, multinodular and mucinous
tumor
. A transitional cell carcinoma of the left
ureter
was also observed. The renal
tumor
was composed of atypical spindle-shaped cells in the mucinous stroma, which showed positive immunoreactivity for anti-muscle-specific actin and anti-desmin antibodies. The ultrastructural examination revealed intracytoplasmic microfilaments with dense bodies, pinocytotic vesicles and junctional structure. These findings were suggestive of the myogenic feature of the case. Urine cytology revealed a number of sarcoma cells in this case since the sarcoma cells markedly invaded the renal pelvis and were apt to separate individually in myxoid stroma. Simultaneous and ipsilateral double malignancies of the renal sarcoma and ureteral transitional cell carcinoma have never been reported in the literature.
