Endoscopic ureterectomy is a minimally invasive surgical technique indicated in cases warranting ureterectomy to complement extended nephrectomy for pyelocaliceal urothelial carcinoma. The results achieved are identical to those of conventional ureterectomy (open surgery) without the inconvenience or sequelae that usually present with the latter procedure. In 1982, the authors developed the modified Dell'Adami and Breda endoscopic ureterectomy technique. To date, they have performed their modified technique in 10 cases: 9 with infiltrating urothelial or highly malignant pyelic or caliceal tumor and 1 with diffuse leukoplasia and cholesteatoma of renal pelvis. into the bladder by transurethral traction of a Chevassu ureteral catheter to which that cephalad end of the
ureter
is attached during the extended nephrectomy procedure. Once

has been achieved, a resectoscope is used to cut the

Continent urinary reservoir has recently been paid much attention by urologists for its contribution to improvement in quality of life in patients who are in need of urinary diversion. We, herein, present some preliminary result in the operation of continent ileocecocolonic (ICC) or ileocecal reservoir which was performed on 13 patients. Since several improvements have been made on the operative procedures for ICC reservoir, the most recent techniques are described below: A 20-25 cm of cecocolonic and 12 cm of terminal ileal segment were isolated after a full length mesenteric incision. The cecocolonic segment was longitudinally split open in 15-20 cm on its antimesenteric border to be re-configuration later. The mesentery was detached for 8 cm in the terminal ileum which was randomly incised into its seromuscular layer. Following the
intussusception
of the terminal ileum into the cecal lumen, the nipple valve of the ileum was stabilized by three row-staples and two row-paired nonabsorbable sutures. The valve was re-enforced by nonabsorbable sutures and dacron mesh which were placed at the base of the
intussusception
. The
ureter
was implanted in the tenia of the colon with antireflux technique of the submucosa tunnel. The distal end of the split cecocolonic segment was, the, folded to and sutured with its proximal end, making a Heineke-Mikulicz type of re-configuration reservoir. A new method for urinary diversion to the rectum was elaborated in animal experiments and currently has been used in 19 patients. Reflux of the rectal content to the colon and to the upper urinary tract is prevented by the fashioning of an
intussusception
valve at the rectosigmoid junction. The rectum is augmented by anastomosing an ileal patch to the anterior rectal wall. A transverse colostomy protects the construction for 6 to 8 weeks. Of the 19 patients 3 had local recurrence or metastasis within 6 months. Thus, 16 patients with a followup of 3 to 14 months are evaluable. All patients are continent during the daytime with an emptying frequency of 3 to 5 times and dry at night with a frequency of 0 to 2. In 3 patients partial sliding of the sigmoid

valve occurred causing reflux of rectal contents to the sigmoid and to the right
ureter
in 1. Reflux to the upper urinary tract has not been revealed in any of the other patients. With 1 exception excretory urography has demonstrated either improvement or stabilization of the upper urinary tract post-operatively. The rectal capacity increased from 200 to 700 ml. after 6 months. With the reservoir full the mean basal pressure was 17 cm. water and the mean maximum pressure was 24 cm. water.

When an ileal loop is used to replace the
ureter
or to create a urinary pouch, an adequate anti-reflux mechanism is essential.
Intussusception
of the ileal wall can be used but a proper technique must be chosen to prevent necrosis of the nipple. Good knowledge of the vascularization of the ileal wall is necessary. Leaving a 1-cm strip of mesenterium when stripping it from the ileal wall provides adequate circulation. Nipples of 3 cm or longer are enough to create an anti-reflux mechanism.
