The seminal vesicles are difficult to evaluate clinically. Transrectal ultrasound (TRUS) has only recently allowed adequate assessment of these structures and their disorders. As the use of TRUS increases, radiologists must familiarize themselves with diseases of the seminal vesicles and the normal and abnormal appearance at ultrasound. This paper presents four cases illustrating the uncommon disorders that TRUS can demonstrate in the seminal vesicles: ectopic
ureter
entering the seminal vesicle, symmetrically enlarged seminal vesicles with
male infertility
, a pelvic arteriovenous malformation mimicking a seminal vesicle cyst, and tuberculosis. In two patients, TRUS was used to guide needle aspiration of seminal vesicle contents safely and to inject contrast for radiography.

A 30-year-old man was referred to our hospital with a complaint of
male infertility
, presenting abnormal semen analysis (semen volume 0.1 ml, sperm concentration 1.2 x 10(6)/ml, sperm motility 0%). Radiological examinations demonstrated right renal agenesis and a cystic mass extending from the prostate to the posterior of the bladder. Our final diagnosis was obstruction of the ejaculatory duct secondary to right ectopic
ureter
associated with ipsilateral renal agenesis. The patient was treated by transurethral unroofing of the cyst. Three months after the surgery, the cystic mass disappeared and the seminal findings showed marked improvement.

Theoretically, ESWL can cause several side effects on the male reproductive system. We determined here the long-term effects of ESWL on sperm with transmission electron microscopy (TEM) in patients with distal ureteral stone. Fifteen men with stones in the distal
ureter
applied to our clinic formed the group of study. The other 15 men with renal or upper
ureter
stones formed the group of control. The ESWL sessions, including maximum 19 kW energy level and 3000 shock waves, were performed with Siemens Lithostar (electromagnetic; Siemens Medical Systems, Erlangen, Germany) lithotriptor. We examined the semen samples from all patients on the day before and 90 days after ESWL. The semen samples were examined with transmission electron microscopy (TEM) to detect ultrastructural changes on the day before and 90 days after ESWL. All the statistical analyses were realized with SPSS 10.0 (SPSS Inc., Chicago, USA) statistical package program. When the control and study groups were compared for initial and day 90 sperm concentration and motility, a significant decrease was found in the study group. Although there was no important anomaly in the control group, we determined some damage on sperm structure in 5 patients (33.3%) who are in the study group 3 months after ESWL. It can reduce sperm concentration and motility permanently. It can also cause severe ultrastructural defects on sperm after a long term period in patients with lower ureteral stone. Therefore, we suggest other treatment modalities for young men with distal ureteral stones to prevent the development of
male infertility
.

Official medical statistics have been analysed corcerning overall urological morbidity in the Russian Federation (RF) and in some particular regions of the RF. The analysis covered morbidity and mortality from urogenital diseases including glomerular, tubulointersticial and other diseases of the kidneys and
ureter
, prostatic diseases,
male infertility
, cancer of the urinary bladder and prostatic gland. It was found that information on urological and oncourological morbidity in the literature is available but its amount is not satisfactory. It is necessary to introduce innovations in organization of medical statistical service, in registration of urological diseases, in particular. Active screening for urological cancer in the RF does not meet requirements and potentialities of modern health care. Special screening programs must be designed to reduce mortality and improve follow-up of urological patients.
