This review was done because most texts and some recent reviews do not recognize the obstructive cause. It is important only because of the high incidence of acute pyelonephritis during pregnancy. This is so common that it has been given a special name, "pyelitis of pregnancy". We have shown in the monkey that bacteria when introduced into the ureter under low pressure does not often cause pyelonephritis unless associated with ureteral obstruction. The presence of bacteria even with partial ureteral obstruction increases the chances of acute pyelonephritis. Care must thus be taken to eradicate bacteriuria during pregnancy to prevent acute pyelonephritis as has been shown by Kass, especially since vesicoureteral reflux occurs more frequently in the pregnant than nonpregnant primate. When we understand the obstructive cause of hydronephrosis of pregnancy and the effect of positional change, therapy will be more effective. The left lateral position then will not only relieve the pain of acute hydronephrosis of pregnancy but will also greatly assist in the therapy of acute pyelonephritis of pregnancy since it will relieve the associated ureteral obstruction.

The phenomenon of ureteral regeneration has been of interest to clinicians, anatomists, and physiologists for many years. The object of this investigation was to study the regeneration of a long pedicle graft of ureter. Regeneration of the pedicle graft over a stent took place in 4 to 5 weeks. At sacrifice 8 to 10 months postoperatively, 7 of 10 operations were completely successful with no evidence of hydronephrosis, obstruction, or pyelonephritis. Two animals exhibited slight dilation of the upper tracts, and one operation was a failure. It is evident from this study that a strip of ureter removed from the ureteral artery will regenerate over a stent in 4 to 5 weeks if the blood supply from the adjacent adventitia is left intact.

Preureteric vena cava is a rare congenital anomaly usually presenting clinically with hydronephrosis and an "S or fishhook" deformity of the ureter at the third or fourth lumbar vertebrae. Unusual presentations do occur and can lead to misdiagnoses. Herein such a case originally diagnosed as a ureteropelvic junction obstruction is presented and the embryology reviewed.

Congenital obstructions of the uretero-pelvic junction of the urinary tract have severe sequelae: urine retention, hydronephrosis, and infections, leading to renal insufficiency without adequate therapy. As the operative results are not always optimal, we studied the morphology of 56 operative specimens and analyzed the structural changes. We found the following conditions causing stenosis of the proximal ureter: 47 localized hypoplasia of the proximal segments of the ureter, systematic hypoplasia in 4 cases, and hypertrophy of the total upper urinary tract in 5 cases. The intrapelvic increase of pressure and the necessary additional work cause characteristic changes of the renal pelvis in the form of hypertrophy of the muscularis and the tunica propria, fibrous destruction of the wall, and secondary formation of valves, which eventually enhance the stenotic effect of the proximal ureter. In the kidneys caliectasis, reduction and hypoplasia of the renal medulla with minor reduction of the renal cortex may be observed. Finally, infections complicate the clinical course. The choice of the surgical procedure should be determined by type and progression of the structural changes and not by personal opinions.

A comparison was made between the excretion urogram and the radio-isotope renogram in 35 patients with uretero-sigmoid anastomosis. A good correlation was obtained between the 2 tests in the presence of radiographic evidence of (a) hydronephrosis, (b) poor renal function and (c) absent function. Renographic evidence of mild impairment of secretory function and of delayed excretion was associated with a significant number of normal excretion urograms and in a few instances there was later progression to severe renal impairment or hydronephrosis. Hydro-ureter associated with a normal renogram carried a good functional prognosis. Renography is recommended in the long-term supervision of patients with uretero-sigmoid anastomosis.

A 16-week-old male cat with urinary incontinence was initially diagnosed as having a right ectopic ureter. Other abnormalities of the urinary system included phimosis, hydronephrosis, and localized right megaloureter. Surgical removal of the right kidney and right ureter was partially successful in stopping the urinary incontinence. Further radiographic studies and retrospective examination of initial radiographs revealed that the left ureter was also ectopic.

The authors report 22 cases of uretero-ileo-plasty for uretero-hydronephrosis due to schistosomiasis. The indications for operation depend on the following criteria: the degree of dilatation which varies from simple atonia to very large hydronephrosis which one must not wait for, ureteral stenosis, vesico-ureteral reflux, the degree of renal failure assessed by studies of creatinine and urea clearance and the resistance to treatment. The operative technique which is not specific for bilharziasis includes uretero-ileo-plasty which is often bilateral, for even in asymmetrical cases, the least affected ureter is often of poor quality. There were failures in two cases due to irreversible renal failure, and in two cases, due to peritonitis. The late results of the other cases appear very favourable: increased vesical capacity, diminution of cystalgia, comfort and improved, general health, were the main factors. Stenosis of the anastomosis, vesico-ureteral reflux and urinary infection, acidosis, lithiasis are rare or not very severe.
