A case of emphysematous pyelonephritis is presented. A 49-year-old male with
diabetes mellitus
complaining of high grade fever attack and right flank pain was referred from internal medicine. KUB demonstrated that the right
ureter
, pelvis and calyces were filled with gas. Anti-bioticus was given intensively and the abnormal gas shadow on plain film disappeared before RP was done, but high grade fever attack persisted and right nephrectomy was undergone. After this operation, the fever was relieved and the patient was discharged at the 30th day post-operatively. A search of available literature in Japan has disclosed only 17 reported cases. Diagnostic methods, treatment, complication and etiology are discussed.

Urinary N-acetyl-beta-D-glucosaminidase (NAG), a lysosomal hydrolase located the proximal tubule of the kidney, has been used as a marker for subtle renal injury. In humans and other animals with
diabetes mellitus
, urinary NAG activity has been shown to increase within 12 hours of the onset of hyperglycemia and glycosuria. Whether the rise in urinary NAG activity is in response to the hyperglycemia or to the osmotic diuresis associated with glycosuria is not known, nor has the time course of the rise in enzyme activity been determined. A study was designed using four groups of dogs to examine these possibilities: group 1 (n = 5), control dogs; group 2 (n = 5), mannitol-infused dogs; group 3 (n = 5), low-glucose dogs; and group 4 (n = 5), high-glucose dogs. In groups 2 and 4, mannitol and glucose, respectively, were infused at a rate to double urine flow from the left
ureter
without altering the contralateral urine volume. In group 3, sufficient glucose was infused to elevate left renal vein glucose level without producing glycosuria. In the control dogs infused with normal saline solution at a constant rate throughout the control and study periods, no differences were found in urinary NAG excretion when data from individual clearance periods were compared for the right and left kidneys. In the low-glucose dogs, urinary NAG/creatinine ratios were significantly increased (P less than 0.01) when the left and right kidneys were compared for the duration of the infusion.(ABSTRACT TRUNCATED AT 250 WORDS)

and antibiotic treatment with cefotaxime and amoxicillin. The concrement was removed by dissolution.
Diabetes
and urinary stasis appear to be the main predisposing factors for this exceptional case of septic urinary infection caused by L. gasseri.

A 30-year-old male patient without
diabetes mellitus
and tuberculosis was referred to our outpatient department for the purpose of further examinations of azoospermia. Transrectal ultrasonography revealed the bilateral calcifications of vas deferens. Kidney-
ureter
-bladder X-ray and computerized tomography revealed also the same findings. Seminal vesiculography revealed no obstructive sign of vas deferens. Histopathology of testes showed hypoplasia. Judging from these findings, we diagnosed the case as calcification of vas deferens associated with spermatogenic disturbance. Review of the 31 cases reported in the Japanese literature including our case was performed.

A microsurgical technique for en bloc kidney and whole pancreaticoduodenal transplantation with bladder drainage employing triple vascular anastomoses without the need for a vascular cuff is described. Nineteen combined isografts were performed using this technique in inbred male Lewis (RT1:I) rats with streptozotocin-induced
diabetes
. Six recipients died within 1 month from early complications (two from uremia, two from pancreatitis, one from bleeding, one from peritonitis); the other 13 survived more than 1 month after transplantation with both the pancreas and kidney grafts functioning. Four of the 13 rats died after 1 month (one from uremia secondary to an obstructed
ureter
, one from unexplained uremia, one from peritonitis after a biopsy, and one of unknown causes). The pancreas isografts of two animals were excised at 1 and 3 months to confirm dependence on the graft; both animals became hyperglycemic after graft pancreatectomy and had immediate declines of urine amylase activity to normal. One animal was sacrificed at 3 months to determine the insulin content of its native and transplanted pancreas; insulin was very low in the former and normal in the latter. The remaining rats survived with both grafts functioning for at least 6 months (normoglycemic, high urinary amylase levels, normal or near-normal plasma creatinine concentrations), before being sacrificed within the context of other experiments and for histological observations. The effects of systemic hypoxia upon cardiovascular and renal function in normotensive Wistar-Kyoto (WKY) and spontaneously hypertensive rats (SHR) are controversial. We examined the effects of systemic normobaric hypoxia (12.5% O2 in N2 for 4 hours) on arterial blood gases (pO2, pCO2, pH), mean arterial pressure (MAP), heart rate (HR), effective renal blood flow (ERBF), glomerular filtration rate (GFR), urine flow (UMV) and renal sodium excretion (U Na V) in conscious unilaterally nephrectomized WKY (n = 12) and SHR (n = 14) chronically instrumented with an arterial, venous and
ureter
cannula. In WKY hypoxia caused a reduction in arterial pO2 and pCO2 but no change in MAP and HR. In SHR hypoxia induced similar reductions in arterial blood gases, a small decrease in MAP and no change in HR. In both strains hypoxia caused significant increases in ERBF, GFR and U Na V, but insignificant changes in UMV. The hypoxia-induced natriuresis developed 90-120 min after starting the hypoxia. These data indicate that a 4-hour lasting hypoxia has profound effects on sodium excretion in conscious WKY as well as SHR. Systemic hypoxia did not cause significant changes in arterial blood pressure in both rat strains.

The influence of increased functional load on the macroscopical and histological appearance of the
ureter
was investigated. Sixty rats were divided into five groups: (1) sucrose-fed rats with non-osmotic polyuria; (2) diabetic rats with osmotic polyuria; (3) uninephrectomized rats; (4) sham-operated control rats; and (5) control rats. The 24-hour urinary volume was measured on days 7, 14 and 21. Growth of the kidney,
ureter
and bladder was investigated and the histological appearance of the
ureter
was further evaluated. Diabetic and sucrose-fed rats had comparable polyuria with a seven-fold increase in urinary output. The urinary volume for the remaining kidney was doubled in uninephrectomized rats. After 3 weeks, diabetic rats had increased weight of the kidney,
ureter
and bladder, sucrose-fed rats had increased weight of the bladder, whereas uninephrectomized rats had increased weight of the kidney and
ureter
. The cross-sectional area (CSA) of the
ureter
wall from control rats increased from the proximal to the distal portion. The size of the whole

from diabetic rats was dramatically increased, the CSA of the wall of the distal
ureter
portion being four times that of the controls. The CSA of the

We report on a case of an isolated common iliac artery aneurysm infected by Candida albicans. To our knowledge, only one other case of this condition has been reported. The patient, a 49-year-old man with
diabetes mellitus
and a history of fungal urinary tract infections, had recurrent right knee pain and swelling. The knee effusion grew C albicans. Mild right hydronephrosis and a 4.6-cm aneurysm of the right common iliac artery without involvement of the aorta or iliac bifurcation was revealed by means of a computed tomography scan. The aneurysm wall was inflammatory, and there was associated purulence at the time of operation. The right
ureter
was densely adherent to the anterior aspect of the aneurysm, but could be palpated and dissected free because of a ureteral stent that was placed before the surgical incision. The aneurysm was resected, and the proximal and distal margins were oversewn without graft placement. C albicans was found in the resected aneurysm. The patient recovered without limb-threatening ischemia or claudication, but the distance he could walk remained limited because of right knee symptoms. The aneurysm may have formed by direct extension of infection from the right
ureter
or by hematogenous or lymphatic spread. This case raises interesting issues about operative strategies and etiology.
