Single system ectopic
ureter
is a rare congenital malformation of the urinary tract, frequently associated with genital tract malformations. We report the first case of an adenocarcinoma arising in an ectopic
ureter
in a woman and mimicking uterine cervical adenocarcinoma. A 34-year-old woman, previously diagnosed as having bicornuate uterus, presented with post-coital bleeding. On gynecological exam, there were two cervixes with a small nodule on the left cervix. After nodule biopsy, the initial diagnosis was adenocarcinoma of the
uterine cervix
. Pelvic ultrasound and magnetic resonance imaging demonstrated an ectopic tumoral
ureter
draining a dysplastic pelvic kidney and inserted in the cervix of a bicornuate uterus. The patient was treated by radical hysterectomy and left nephroureterectomy. Pathological exam demonstrated an adenocarcinoma arising in the ectopic
ureter
. We suggest that this case could be an argument for recommending regular follow-up for women with ectopic
ureter
for detecting malignant transformation.

Ureteral endoprosthesis results are presented for 22 patients (9 males and 13 females) with long strictures of the upper urinary tract (UUT). Stenosis was caused by rectal tumor in 2 patients, by cancer of the
uterine cervix
in 5 patients. In a female with Burneville-Pringle disease metallic prosthesis was set in the pyeloureteral segment (PUS) of the solitary right kidney. Six patients had long strictures of the abdominal part of the
ureter
, 4 had PUS strictures, 3--of the
ureter
of the transplanted kidney and 1 had stricture of ureterocalicoanastomosis after Neivert operation. Endoprosthesis was established by a retrograde approach in 13 cases, via percutaneous antegrade approach--in 9 cases. In postoperative period 2 patients had exacerbation of chronic pyelonephritis, they received anti-inflammatory treatment. Patancy of the
ureter
was reestablished in all the cases and persisted for all the follow-up period (4-62 months). Late after operation a patients with Burneville-Pringle disease developed UUT obstruction because of ureteral compression by enlarged angiolipomatous nodes under endoprosthesis. Therefore, one more nitinol stent was set in the upper third of the right
ureter
retrogradely. Thus, ureteral endoprosthesis in long and recurrent strictures of the
ureter
, ineffective plastic surgery on the UUT is an operation of choice.

Nontuberculous mycobacterial (NTM) infection is increasing across the world. Although the most common clinical manifestation of NTM disease is lung disease, a rare form of disseminated NTM disease has also been documented. Disseminated NTM usually develops in severely immunocompromised individuals, especially those with advanced AIDS. This manifestation is rare in non-HIV-infected hosts and is associated with immunosuppressed conditions. However, recent reports have suggested that disseminated NTM disease in immunocompetent patients without HIV infection has been increasing. Dissemination may involve any organ system, but a case in the female genital tract has never been reported. We report a case in a 67-yr-old previously healthy woman who presented with a disseminated NTM infection in the
uterine cervix
. The primary presentation was general fatigue and body weight loss. The patient also presented with a mass formation that mimicked cervical cancer on magnetic resonance imaging. In addition to the cervical mass, the patient presented with a mass formation in the omentum; wall thickening of the vagina, bladder, and
ureter
; and retention of pleural/peritoneal fluid. Vaginal cytology was negative. A diagnosis was made only after detecting acid-fast bacilli in a biopsy specimen of cervical mass, which was conducted under suspicion of cervical malignancy. Then, Mycobacterium avium was confirmed in a polymerase chain reaction test of cervical tissue. After administration of antimycobacterial therapy, the mass and other findings on magnetic resonance imaging disappeared. Infection in multiple organs leads to the diagnosis of disseminated NTM. This case indicates that, for prompt and accurate diagnosis, efforts to detect specific lesions by an imaging study and to confirm diagnosis pathologically are equally important, especially when local cytology is not convincing. The clinical course of this case may serve as a useful reference in the diagnosis and treatment of NTM.

Wide tumour excision is currently the standard approach to surgical treatment of solid cancers including carcinomas of the lower genital tract. This strategy is based on the premise that tumours exhibit isotropic growth potential. We reviewed and analysed local tumour spreading patterns in 518 patients with cancer of the
uterine cervix
who underwent surgical tumour resection. Based on data obtained from pathological examination of the surgical specimen, we applied computational modelling techniques to simulate local tumour spread in order to identify parameters influencing preferred infiltration patterns and used area-proportional Euler diagrams to detect and confirm ordered patterns of tumour spread. Some anatomical structures, e.g. tissues of the urinary bladder, were significantly more likely to be infiltrated than other structures, e.g. the
ureter
and the rectum. Computational models assuming isotropic growth could not explain these infiltration patterns. Introducing ontogenetic distance of a tissue relative to the
uterine cervix
as a parameter led to accurate predictions of the clinically observed infiltration likelihoods. The clinical data indicates that successive infiltration likelihoods of ontogenetically distant tissues are nearly perfect subsets of ontogenetically closer tissues. The prevailing assumption of isotropic tumour extension has significant shortcomings in the case of cervical cancer. Rather, cervical cancer spread seems to follow ontogenetically defined trajectories.

Primary lymphoma of the
uterine cervix
is a very infrequent disease, usually affecting perimenopausal women. Symptoms are very similar to other gynecological malignancies, but treatment and prognosis completely differ, as most of these patients have a better survival. This condition has to be suspected in women with recent normal Pap smear test, rapidly growing tumor and initially non-contributory biopsies. We report a case of primary diffuse large-B-cell lymphoma of the
uterine cervix
mimicking a locally advanced cervical cancer with right
ureter
hydronephrosis at diagnosis. She was medically managed with a combination of rituximab and chemotherapy with cyclophosphamide, hydroxydaunorubicin, oncovin, and prednisone, associated to methotrexate for central nervous system prophylaxis. We will discuss about the role of combined treatments with surgery and radiotherapy, and the fertility sparing management in young women.
