One hundred and twelve patients with evaluable genitourinary tumors were treated with cis-diamminedichloroplatinum (II) as a single agent under multi-institutional clinical trials. Most patients had extensive prior therapy. Of 30 patients with testicular tumors, 5 complete responses (16.7%) and 15 partial responses (50.0%), of 29 patients with bladder carcinoma, 2 CR (6.9%) and 7 PR (24.1%), of 34 patients with prostatic carcinoma, 6 PR (17.6%), and of 10 patients with pelvis and
ureter
carcinoma, 1 CR and 3 PR were obtained respectively. No responder was seen in eight patients with
renal cell carcinoma
and in one with urethral carcinoma. Adverse reactions were similar to those already reported including gastrointestinal reactions, nephrotoxicity and myelosuppression. Ototoxicity, peripheral neuropathies and transient elevation of GOT-GPT levels were occasionally encountered. Cis-diamminedichloroplatinum (II) appears to be highly active as a single agent in the treatment of advanced genitourinary tumors.

An unusual case of localized amyloidosis of the
ureter

Autopsy records of
renal adenocarcinoma
cases were analyzed to investigate the mode of metastatic spread in instances of a tumor thrombus in the renal vein and/or vena cava. In this series of 1,828 cases 137 (7.5 per cent) had a tumor thrombus. Analysis revealed no significant differences in the incidence of the tumor thrombus between sexes or sides of primary renal involvement, or whether nephrectomy had been performed. The cases with a tumor thrombus showed higher frequencies of metastases in the liver, hepatic hilar lymph nodes and

, while metastases in the bones, contralateral kidney and brain were less frequent. The results suggest that bypass routes or lymphogenous spread of the tumor might be facilitated in some cases with a tumor thrombus.

Muscle coat specimens from human calices, renal pelvis, pelvi-ureteric junction, upper, middle and lower
ureter
segments were examined under an electron microscope. These specimens were taken from 8 patients who had undergone nephroureterectomy: 6 for localized
renal carcinoma
and 2 for papillary tumor of the pelvis. Two types of smooth muscle cells were observed, "typical" muscle cells and "special" muscle cells. The latter are rich in agranular endoplasmic reticulum, have few myofilaments and are interconnected in numerous, extended, peculiar contact areas. The ratio between these two types of muscle cells differs as also does their innervation between the various segments examined. On the basis of our findings we propose that the "special" muscle cells perform a "pacemaking" function.

and renal pelvis. This simple and inexpensive technique is valuable in the preoperative diagnosis of tumors of the
ureter
and renal pelvis. In our series it was possible to recognize clearly 29 of 36 such tumors (80.5 per cent) through preoperative lavage cytology. No falsely positive diagnoses were made. Lavage cytology failed in 6 patients with
hypernephroma
. The diagnostic reliability of lavage cytology with urothelial tumors is far greater than that of exfoliative urine cytology and reaches almost equally good results as brush biopsy.

The incidence and mortality rate of urogenital cancers in Japan are both low compared to those in western countries. However, the incidence and mortality patterns of cancer in Japan are currently becoming closer to those of western countries, and the importance of urogenital cancers is increasing. We conducted an analysis of urogenital cancers in Gunma Prefecture. The subjects were newly detected urogenital cancer patients living in Gunma Prefecture diagnosed between 1985 and 1992. A total of 37 patients with various renal and adrenal diseases underwent laparoscopic nephrectomy, radical nephrectomy and adrenalectomy since July 1991. The underlying diseases included renal calculi, vesicoureteral reflux, ureteropelvic junction obstruction, ureteral stenosis, ectopic
ureter
, vascular disease,
renal cell carcinoma
and adrenal tumors. Twenty-nine kidneys (including 5 with
renal cell carcinoma
) and 5 adrenal glands were removed successfully. Three cases failed because of dense adhesion to the surrounding structures, hemorrhage from the renal vein and hemorrhage from the vena cava, respectively. Of the remaining 34 patients 3 required additional open laparotomy to control bleeding or remove lost calculi. Mean operating time was 265 minutes for nephrectomy, 383 minutes for radical nephrectomy and 199 minutes for adrenalectomy, and mean estimated blood loss was 455 ml., 430 ml. and 80 ml., respectively. Average hospital stay was 10 days for the 31 patients without open laparotomy and convalescence was completed by postoperative day 21. These results indicate that laparoscopic surgery for renal and adrenal disease is a feasible, minimally invasive procedure. In the case of laparoscopic radical nephrectomy, however, longterm followup is necessary to confirm the efficacy.
