Renal autotransplantation with/without extra-corporeal surgery was performed in 53 patients between September, 1975 and december, 1987. Original disease was obstructive disease of the upper urinary tract in 25 patients, renovascular hypertension and renal vascular disease in 13, renal calculous disease in 12 and
renal cell carcinoma
in 3. Ten of the 53 patients had solitary kidneys. Three patients died on 14, 21 and 49 postoperative days of massive bleeding with disseminated intravascular coagulopathy caused by the rupture of transplant arterial anastomosis (1 patient with urinary obstructive disease) and sepsis caused by wound infection (2 patients with renal calculous disease). Two kidneys were removed on operative day and 8 postoperative days due to arterial thrombosis in 2 patients with aneurysm of intrarenal artery. The deterioration of renal function was observed in previously damaged kidneys of two patients with extensively damaged
ureter
. No other severe complications were observed. In 23 of 24 patients with the obstructive disease of the upper urinary tract, disappearance or improvement of the obstructive change was observed after surgery. All 5 patients with renovascular hypertension showed normo-tension without administration of antihypertensive drugs after surgery. In 3 of 5 patients with an aneurysm of the intrarenal artery, the aneurysm was removed and reconstruction of the artery was performed successfully. Two patients with arterio-venous fistula and one patient with nut cracker syndrome had no severe hematuria with bladder tamponade after surgery. Ten of 12 patients with renal calculous disease were treated successfully without residual calculi by this procedure. Three patients who had solitary kidney with
renal cell carcinoma
were treated successfully by this procedure.(ABSTRACT TRUNCATED AT 250 WORDS)

We reviewed 56 cases in which the preoperative diagnosis was Wilms' tumor at the Children's Hospital of Philadelphia (CHOP) during the three calendar years 1983 through 1985. Fifty-one had Wilms' tumor (38 operated on at CHOP; 13 were referrals for further treatment or consultation), three had
renal cell carcinoma
(
RCC
), and one each with metastatic mesoblastic nephroma and clear cell sarcoma. We reviewed these cases to determine the significance of the urographic finding of nonopacification of kidney with suspected Wilms' tumor, and what additional diagnostic studies are needed. Five Wilms' tumor patients (10%) and one
RCC
patient had nonopacification of the involved kidney on intravenous pyelography (IVP). Four Wilms' tumor patients had tumor extending into the collecting system: into the proximal
ureter
in one, obstructing the superior pole in another, and filling the renal pelvis in two, one of which also had tumor extending through the inferior vena cava and into the right atrium. One patient with Wilms' tumor and one with
RCC
had huge left-sided tumors that crossed the midline and appeared to replace completely the functioning renal parenchyma on that side. The most common cause of nonpacification of kidney by Wilms' tumor is collecting system blockage by tumor. Although extension into the renal pelvis is relatively uncommon in Wilms' tumor (five of 51 cases during the study period, 10%), when it occurs, nonopacification of IVP often results (four of five cases, 80%). Although renal vein involvement is more frequent (eight of 51 cases; 16%), nonopacification is unusual (one of eight cases, 13%; the tumor also had filled the collecting system).(ABSTRACT TRUNCATED AT 250 WORDS)

We tested for fragile sites in lymphocytes from nine patients with genitourinary tumors to determine if a correlation existed between their cancer chromosome breakpoints and fragile sites. Induction was done for rare fragile sites in all known classes by exposure of cells to fluorodeoxyuridine and bromodeoxyuridine (BrdU). No rare fragile sites were found. Induction was also done for common fragile sites in all known classes using aphidicolin (Apc), 5-azacytidine, and BrdU. Although 56 common fragile sites were detected, only a single site corresponded in location to a genitourinary tumor chromosome breakpoint. That was the common fragile site in band 3p14. No overall correlation was found between fragile sites and chromosome rearrangements in carcinoma of the kidney,
ureter
, bladder, and testis. The sole known candidate for a possible biologic role is the 3p14 common fragile site in
renal cell carcinoma
.

A sixty eight-year-old man was admitted to our hospital complaining of macroscopic hematuria. Ultrasonography, X-ray and laboratory examination revealed a right
ureter
tumor and left non-functioning adrenal tumor. Pathological diagnosis was transitional cell carcinoma of the
ureter
and left adrenocortical carcinoma. A review of persistent Japanese literature revealed this case to be the 97th case of non-functioning adrenocortical carcinoma and the first case synchronously occurring with transitional cell carcinoma of the
ureter
. Forty eight cases of synchronously occurring transitional cell carcinoma of the
ureter
with cancers in other organs, especially
renal cell carcinoma
and gastric cancer, have been reported. Recently, an increasing number of cases of non-functioning adrenal carcinoma are detected by CT scan, ultrasonography, adrenal radioisotopic scintigraphy and urinary 17-OHCS, 17-KS.

The early diagnosis of primary urogenital tumors shows varying efficacy regarding the various tumor types. While the urothelial tumors of the renal pelvis, the
ureter
and the bladder reveal early symptoms such as hematuria
hypernephroma
, urethral carcinoma and carcinomas of the prostate and the testes remain clinically silent for a long time. Thus early diagnosis of carcinomas of the prostate or the testes might only be made by palpation at best. Aside from cytologic examination of the urinary tract, there are no other laboratory techniques at hand to give clear evidence of early tumor stages. The possibilities of early recognition of
hypernephroma
, carcinomas of the renal pelvis,
ureter
, bladder, or urethra, and carcinomas of the penis, testes, and prostate are demonstrated and compared with each other in respect of their specificity.

Using a polyamine-test enzyme kit, the urine polyamine concentration was determined in 74 patients with malignant urological disease (12 with
renal cell cancer
, 13 with pelvic-
ureter
cancer, 24 with bladder cancer and 25 with prostate cancer), 7 patients with BPH, 20 patients with benign urological disease and 20 normal subjects. The urine polyamine level was significantly elevated in all the patients with any malignant urological disease compared to normal subjects. It was also significantly high in the patients with BPH. Defining the mean +/- 3SD (= 50 mumole/g Cr.) of 20 normal subjects as an upper limit, slightly higher levels not exceeding 100 mumol/g Cr. were frequently observed in the patients with BPH or with benign urological disease. Setting the upper limit at 100 mumole/g Cr., the positive rate amounted to 33% (low stage 17%) in
renal cell cancer
, 23% (low stage 14%) in pelvic
ureter
cancer, 13% (low stage 0%) in bladder cancer and 4% (low stage 0%) in prostate cancer. The positive rate was low especially in low stage cases.

A case of synchronous contralateral
renal cell carcinoma
and ureteral transitional cell carcinoma is reported. A 56-year-old man, who had been incidentally found to have an abnormal mass in the upper pole of the right kidney on ultrasound sonography, was admitted on January 8, 1985. CT scanning and renal arteriography revealed right renal malignancy. Right radical nephrectomy was performed and histological examination showed adenocarcinoma, granular cell type of the right kidney. He was discharged on February 3, 1985. Two months postoperatively, he was rehospitalized for macroscopic hematuria. Left retrograde pyelogram showed obstruction at middle
ureter
and cytology of urine from left
ureter
was positive. So a left ureteral tumor was suspected, and partial resection of left
ureter
and ureteroureterostomy were performed. Histological examination revealed ureteral transitional cell carcinoma. He is now doing well at 6 months following the lat surgery, without any evidence of recurrence.

A cooperative study was carried out in 32 institutions throughout the country to evaluate the clinical efficacy of recombinant human leukocyte A interferon (rIFN-alpha A, Ro 22-8181) on malignant tumors of the urogenital tract. The responses were evaluated according to the "Criteria for the Evaluation of Clinical Effects of Cancer Chemotherapy on Solid Tumor" proposed by Koyama and Saito. Out of 269 cases which were examined in the present study, 138 cases were evaluable. Among 108 cases with
renal cell carcinoma
, efficacy was observed in 15 cases; complete response (CR) in 2 cases and partial response (PR) in 13 cases, indicating an efficacy rate of 13.9%. PR was obtained in 1 case out of 14 with bladder cancer and 1 case out of 6 with tumors of the renal pelvis and
ureter
. Main subjective and objective adverse reactions observed were fever, malaise, anorexia and nausea and/or vomiting. Laboratory test abnormalities consisted of leukopenia, thrombocytopenia and elevation of GOT X GPT. All of these disappeared with discontinuation of rIFN-alpha A or after administration of its therapeutic dose.

In some serums of patients with transitional cell carcinoma (TCC), a factor is present which induces lymphocytes from most donors with or without TCC to become cytotoxic against TCC-derived target cells. The induced cytotoxicity was directed against target cells derived from TCC's of the renal pelvis,
ureter
, and urinary bladder, but not against cells derived from normal kidney, bladder, testis, or skin or from
renal cell carcinoma
. Cytotoxicity occurred without complement but did not occur without effector cells.

Circulating immune complexes (CIC) were detected and quantitated in 49 patients with urological malignant diseases (9 cases of
renal cell cancer
, 3 cases of renal pelvic and
ureter
cancer, 21 cases of bladder cancer and 16 cases of prostatic cancer), 9 patients with urological benign diseases and in normal subjects by the polyethylene-glycol precipitation complement consumption test (PEG-CC test). The average CIC level was 2.7 +/- 3.0% in 18 normal subjects and the normal range was less than 10% of the CIC level. CIC level of patients with
renal cell cancer
was 14.3 +/- 20.1%, being elevated in 3 of the 9 patients, that of patients with renal pelvic and
ureter
cancer was 4.7 +/- 4.6%, being within the normal range in 3 cases, that of patients with bladder cancer was 4.7 +/- 4.4%, being elevated in 1 of 21 patients, and that of patients with prostatic cancer was 8.9 +/- 15.4%, being elevated in 3 of 16 patients. In urological malignant diseases such as
renal cell cancer
and prostatic cancer the CIC values were relatively high.
