During a 4-year-period, 5 cases of ureteral obstruction by aorto-iliac aneurysm were detected. Over the same period 50 aorto-iliac aneurysms were resected. One of the 5 patients was not operated on. In the remaining 4, the aneurysm was resected and replaced by a Dacron bifurcation graft. Nephrectomy was performed in the first 2 patients. Autotransplantation of the remaining kidney was carried out in one case following enucleation of a
hypernephroma
ex vivo. In the other 2 patients, ureterolysis and intraperitoneal transposition of the
ureter
was performed in conjunction with resection of the aneurysm. One of the latter patients suffered from uraemia due to a non-functioning kidney on the other side. Ureteral obstruction appears to be a relatively common complication of aorto-iliac aneurysm. Elective evaluation of patients with aortic aneurysms should include late aortograms visualizing the ureters.

We report on 3 patients who presented with an acute onset of abdominal pain, a palpable abdominal mass and a rapid decrease in hemoglobin. In 2 patients a spontaneously ruptured
hypernephroma
was found and the other patient had a squamous cell carcinoma of the
ureter
with bleeding into the tumor. Although in all 3 cases the tumors were at an advanced stage of development, the patients had been entirely free of urological symptoms until shortly before hospitalization. The importance of considering the possibility of spontaneous rupture of such a tumor in the evaluation of cases of an acute abdomen is stressed.

A modified radical nephrectomy to cope with
renal carcinoma
presenting at advanced stages, is evaluated. A total of 50 cases were treated in this hospital from 1962 to 1976. The poor results from treatment of
renal cell carcinoma
in the earlier years, have prompted the modification of the conventional radical nephrectomy to cope with this disease which is still frequently seen at advanced stages. The modification includes exclusion of distant metastasis. The adherent segment of colon is resected along with the tumour. The renal vessels are then isolated. If the IVC is involved by a right-sided tumour, it is resected along with the tumour. The
ureter
is then ligated and the tumour with the adrenal gland, the resected colon segment, regional lymph nodes and the IVC are removed in one block. 2 such patients who otherwise would have been considered inoperable have survived for over 3 years.

The propensity of a
hypernephroma
to invade the renal vein and even the inferior vena cava is widely known. However, in contrast to papillary carcinoma of the kidney, hypernephromas rarely involve the
ureter
. Review of the English literature reveals 42 reported cases of ureteral involvement. Most represented metastases as opposed to direct, contiguous extension which is extremely rare. C case of
hypernephroma
with contiguous castlike extension into the
ureter
without mucosal violation is presented. Mechanisms of ureteral involvement and implications regarding treatment of hypernephromas are discussed.

A case of contralateral ureteral metastasis from
renal cell carcinoma
is reported. A 52-year-old man underwent left nephrectomy for
renal cell carcinoma
of November 20, 1986. He was clinically asymptomatic for 4 years 8 months after the operation. He was admitted on August 14, 1991, because of right loin pain. Right retrograde pyelography and percutaneous pyelography showed a filling defect in the right
ureter
at the level of L3. After the right
ureter
was explored, the tumor lesion of
ureter
was resected and end to end anastomosis of the
ureter
was performed. Histopathologic examination showed a metastatic clear cell carcinoma consistent with a renal primary. The contralateral ureteral metastasis from

is very rare and only 15 cases have been reported previously.

, ureteric stump, bladder and prostatic fossa. After previous nephrectomy for

, urinary metastases, although rare, should be considered and investigation of unexplained haematuria should include the ureteric stump. Useful palliation and possibly cure can be achieved by resection of these lesions.

after radical surgery is exceedingly rare. We report on a 48-year-old man who underwent upper urinary tract flexible endoscopy to define a filling defect, which was found to be a perforating

. After radical nephrectomy the patient had multiple recurrences on the urinary surfaces,

Immunosuppressive acidic protein (IAP) is a non-specific immunoreactive protein arising from inflammatory or malignant conditions in the human body. We determined the IAP levels in 65 cases with urological malignancies and in 31 cases with benign diseases as a control group during a 9-month period. There were significantly higher serum levels of IAP in cases of bladder transitional cell carcinoma (p = 0.025), prostate adenocarcinoma (p less than 0.00001) and upper urinary tract urothelial cancer (kidney and/or
ureter
, p = 0.013) as compared with those of the control group. Significant differences in IAP between different tumor stages were found in the bladder cancer group with high stage cases having higher IAP levels (p less than 0.0005). However, no significant differences were found between different tumor gradings. Most of the prostate cancer patients had extremely high IAP values (1,029 +/- 490 micrograms/ml) in this study.
Renal cell carcinoma
and testicular tumors showed no statistical differences from the control group (p = 0.89 and 0.37, respectively). No differences could be found in the different age groups (by decades) or sexes. The serum IAP level can be a good non-specific tumor marker for bladder cancer staging and probably a good follow-up tool for most urological malignancy patients.
