A survey is presented, based upon a personal material of 225 cases of urinary fistulae. 173 of them were postoperative or obstetric fistulae, whereas 52 had appeared after a radiological or combined radiological and surgical treatment for cancer of the uterus. --The author's method for preparing the fistula region from a transverse incision under the external urethral orifice is described as well as his grafting techniques. Vessels are brought to the fistula region by using flaps from one or both pubococcygeus muscles, the rectus abdominis muscle or the gracilis muscle. --For vesico-vaginal fistulae situated high up in the fundus a graft from the omentum majus or an appendix epiploica is sometimes used. They can usually be grasped from below after the peritoneum has been opened. --In cases where the ureters were also damaged by irradiation a special technique was used. After the damaged part of the
ureter
had been resected the ureters were implanted into an ileum segment which was anastomosed to the bladder fistula. --In one of the 173 non-irradiated cases, a Bricker-bladder had finally to be made because of the failure to achieve a functioning urethra. The others were all cured after one or in a few cases two operations. --Among the 52 cases, which had been irradiated, 46 coud to be made, one got a fast growing local recurrence of the
carcinoma
and one had too bad general condition to allow a diversion operation. Two patients have rest fistulae but may be possible to cure conservatively.

Ileocaecocystoplasty was performed after subtotal cystectomy in a man with extensive leukoplakia of the bladder as a prophylaxis against
carcinoma
. 2 years later the
ureter
(contralateral nephrectomy had previously been performed) became blocked by an unusual interstitial inflammation complicated by necrosis and calcification. This was excised, and the tail of the ileum anastomosed to the renal pelvis with a good result at 20 months.

Five cases were related to gynaecological surgery and 4 to operations involving the colon and rectum. In digestive surgery, the ureteric lesion was noted and repaired in 3 cases out of 4. By contrast, in gynaecology, it went unnoticed in all cases and was diagnosed and repaired after an interval varying from 6 weeks to 4 years after the trauma. 4 anti-reflux uretero-vesical reimplantations and one uretero-ureteric anastomosis were carried out, with complete success in all 5 cases. It is particularly striking to note that in gynaecological surgery 4 of the 5 ureteric lesions followed surgery for a non-malignant condition and that the ureteric trauma should have been easy to avoid, in particular by the preoperative insertion of ureteric catheters. By contrast, all the cases in digestive surgery were associated with operations for a
carcinoma
. In 3 cases out of 4, excision of the tumour necessitated the sacrifice of a segment of the
ureter
in the pelvis. One end-to-end ureteric suture failed, whilst 2 uretero-ureteric anastomoses were both successful.

. Special trends in treatment are discussed. Long-term results confirm the value of using a bladder flap in cases of reimplantation of the

into the bladder. Formation of a unilateral or bilateral "psoastip-bladder" (bipartition of the bladder) is described and active surgical management is recommended in cases of operable recurrent cervical

Partial or total replacement of the
ureter
by small intestine was performed at the University of California/Los Angeles Hospital and affiliated hospitals 94 times in 92 patients from 1954 to 1978. Indications included recurrent calculi, ureteral stricture, fistula, congenital obstruction of the
ureter
and ureteral
carcinoma
with a solitary kidney. A special use has been for undiversion of an ileal conduit. Followup evaluation was possible in 95 per cent of the patients and ranged from 6 months to 23 years. Thirty-six patients were followed for more than 5 years. The operation was judged successful in 81 per cent of the cases. Serum creatinine was unchanged or decreased in 75.7 per cent and the pyelogram was unchanged or showed decreased dilatation in 84.6 per cent of the patients. Although reflux was seen in 40 of 55 patients who had cystograms 39 (97.5 per cent) were considered to be treated successfully. Indications for and results of the operation are discussed. The procedure is recommended as optimal therapy for carefully selected situations when the normal urinary tract cannot be used.

In 41 cases of roentgenologic filling defects in renal pelvis and
ureter
, which could not be determined exactly preoperatively, we could confirm the diagnostic value of cytology from washings of renal pelvis and
ureter
(lavage cytology), especially in preoperative diagnosis of
carcinoma
of renal pelvis and
ureter
. Ten of fourteen tumors (71%) were detected by this method. The diagnostic reliability increased with tumor-stage and was more accurate than the normal exfoliative urinary cytology. In five cases of hypernephromas invading the renal pelvis lavage cytology was not successful and we had obtained only false negative results. In roentgenologic filling defects that were not tumor-induced especially uric acid stones, inflammatory processes, blood clots and renal cysts, we had no false positive results. Our findings are discussed together with the literature.

Herein we review 70 cases of transitional cell carcinoma of the upper urinary tract. When complete nephroureterectomy was not performed transitional cell carcinoma developed in 30 per cent of the remaining ureteral stumps. Single incision nephroureterectomy did not include the intramural
ureter
in 50 per cent of the cases in which it was performed. When nephroureterectomy is selected as treatment for
carcinoma
of the renal pelvis or
ureter
a cuff of bladder, which includes the ureteral orifice, should be removed. A second incision may be required for adequate exposure.

a new technique of intraureteral biopsy was attempted with a Dormia stone basket. Of 8 biopsies performed on 7 patients with ureteral or pelvioureteral
carcinoma
only 1 failed to produce a fragment of tissue. Histological diagnosis was made in each of the 7 patients and histological grading in 5. Four patients with non-malignant defects of the lower
ureter
also were biopsied with this technique to ascertain the feasibility of this biopsy on minimal filling defects. In each patient a tiny piece of tissue was removed. Histological diagnosis could be made in 3 cases.

A retrospective analysis of 74 cases of transitional cell carcinoma of the renal pelvis and ureter treated at this institution over the past 30 years is presented. When nephrectomy alone or incomplete nephroureterectomy was performed, subsequent transitional cell carcinoma developed in 30% of the ureteral stumps. Subsequent bladder
carcinoma
occurred in 25% of the patients with primary upper urinary tract
carcinoma
. The type of initial surgery performed did not appear to influence this incidence of subsequent bladder tumors. Contralateral upper urinary tract

developed in only one patient. When nephroureterectomy is performed for carcinoma of the renal pelvis and ureter, a cuff of bladder that includes the ureteral orifice should be removed to obviate recurrent disease in the ureteral stump.
