This study covers 230 Wetheim operations. Sixty-seven patients had Stage II
carcinoma
(29.17%), and 163 patients had Stage I
carcinoma
(70.80%). Until 1968, x-ray treatment was used after the Vienna Wertheim operation, but since then the approach has changed. Tele- and brachio-cobalt therapy is now included. The operative death-rate was 0.96%. One patient died of peritonitis, another of renal uremia. In the Vienna Wertheim group, ureteric fistulae occurred in 7.23% of patients. With the introduction of mesentery and the
ureter
roof into the Wertheim procedure, ureteric fistulae have currently disappeared from the material examined (133 cases), although the operation was more radical than the classical Vienna Wertheim technique. Urography examination shows that changes are twice are frequent, and later anatomical changes in the urinary system are more than twice as frequent, with the Vienna Wertheim technique. A considerable number of early hydronephroses were found to have disappeared during the first year. Urography examination has shown that the mesentery of the
ureter
also has a beneficial effect on the flow of urine. It serves in the prophylaxis of fistulae, and prevents greater displacement of the
ureter
in the parametrial vacancies.

In a consecutive series of 88 cases of
carcinoma
of the kidney and upper urinary tract seen at one hospital, 31 had malignant urothelial tumours of the renal pelvis or
ureter
. Forty-two per cent of these transitional-cell carcinomas occurred in patients with renal papillary necrosis following upon prolonged and heavy analgesic ingestion. Other possible aetiological factors were heavy cigarette smoking (61% of cases), long standing urinary obstruction or infection (23%) and possible occupational exposure (6%); in only four cases (13%) was there no identifiable aetiological factor. Those cases with analgesic nephropathy were characterised by renal functional impairment, hypertension and interstitial nephritis, but there was no difference in the clinical behaviour or pathological appearances of the tumours in the two groups. The clinical and experimental evidence that certain metabolites of phenacetin are carcinogenic is reviewed.

Three types of cultured mammalian cells, from a rat
ureter
carcinoma
(RUC-2), from a mouse mammary
carcinoma
(M8013), and from a Chinese hamster lung cell-line (CH-V79), have been compared with respect to various treatments. Using cloning assay, the sensitivity of these cell-lines was measured for ionizing radiation, U.V.-light and for temperatures of 41 degrees, 42 degrees, 43 degrees, 44 degrees and 45 degrees C. RUC-2 cells are the least sensitive to these treatments, except for 45 degrees C. M8013 is sensitive to gamma-rays and U.V.-irradiation. CH-V79 is sensitive to gamma-radiation and hyperthermia. Since sensitivity to one type of treatment does not correlate with similar susceptibility to the others, it is concluded that different mechanisms may be involved in inactivation by these treatments.

The induction by gamma radiation of reproductive death in cultured cells derived from a rat
ureter
carcinoma
(RUC-2) and from Chinese-hamster lung tissue (CH-V79) was shown to be enhanced by hyperthermic treatments at 41, 43 and 45 degrees C. The degree of enhancement was found to depend on the line of cells studied, the temperature employed and the level of damage considered. The influence of accumulating sublethal damage was decreased by hyperthermia, and the final slope of the radiation survival curve was increased. The degree of enhancement of lethal damage was found to depend on the time interval between the heat treatment and irradiation, especially at 41 degrees C.

Carcinoma
metastatic to the
ureter
is uncommon, only 160 patients having been reported upon to date. During a ten year period, 82 patients with ureteral obstruction, secondary to metastatic disease, were seen at the UCLA Hospital. A critical analysis of the records of these patients revealed important clinical and diagnostic criteria to aid in the evaluation and treatment of extrinsic ureteral obstruction. Tumors of the cervix, prostate, breast and colon-rectum were the most common primary lesions to metastasize to the
ureter
. Subtle changes in urinary symptoms or in laboratory values should alert the physician to the possibility of ureteral involvement. Early detection of metastatic ureteral obstruction prior to the ravages of uremia may allow time for more aggressive use of adjuvant therapeutic modalities that would, at a later stage of the disease, be ineffectual or contraindicated.

of the renal pelvis. The procedure has proved safe and expeditious, as well as permitting more precise excision of the surrounding vesical mucosa.

The ages of the patients ranged from forty-two to eighty-three years, with the highest incidence between the fifth and seventh decades. Males were more frequently affected than females, and the tumors were usually found in the lower third of the

. Hematuria and flank pain were the presenting symptoms in the majority of cases. Poorly differentiated invasive tumors had poor prognosis when compared to well-differentiated noninvasive lesions. Total nephroureterectomy with excision of bladder cuff is the preferred treatment for ureteral

Partial or total replacement of the
ureter
by small intestine was performed at the University of California/Los Angeles Hospital and affiliated hospitals 94 times in 92 patients from 1954 to 1978. Indications included recurrent calculi, ureteral stricture, fistula, congenital obstruction of the
ureter
and ureteral
carcinoma
with a solitary kidney. A special use has been for undiversion of an ileal conduit. Followup evaluation was possible in 95% of the patients and ranged from 6 months to 23 years. Thirty-six patients were followed for more than 5 years. The operation was judged successful in 81% of the cases. Serum creatinine was unchanged or decreased in 75.7% and the pyelogram was unchanged or showed decreased dilatation in 84.6% of the patients. Although reflux was seen in 40 of 55 patients who had cystograms 39 (97.5%) were considered to be treated successfully. Indications for and results of the operation are discussed. The procedure is recommended as optimal therapy for carefully selected situations when the normal urinary tract cannot be used.

In 80 cases of roentgenologic filling defects in renal pelvis and

, which could be determined exactly preoperatively, we could confirm the diagnostic value of cytology from washings of renal pelvis and
ureter
(lavage cytology) especially in preoperative diagnosis of
carcinoma
of renal pelvis and
ureter
. Twenty of 25 tumors (80%) were detected preoperatively. There were no false positive results. The diagnostic reliability of this method was more accurate than with normal exfoliative urinary cytology from other authors. The results are discussed together with the literature.
