Localized amyloidosis of the genitourinary tract is a rare phenomenon. The differential diagnosis between
amyloidosis
and malignancy is difficult. We report the case of an 81-year-old man with extensive unilateral localized
amyloidosis
of the renal pelvis,
ureter
and ureteral orifice. Diagnosis of
amyloidosis
was confirmed only postoperatively.

Primary localized
amyloidosis
of the
ureter
is very rare, and only 22 cases have been reported in the world literature. We report the twenty-third case along with a review of the relevant literature. Due to its radiologic resemblance to malignancy, many cases were treated by nephroureterectomy in the past. The case being reported here was successfully treated by local excision and ureteroneocystostomy.

A 62-year-old woman complaining of asymptomatic hematuria was hospitalized. Although a cytologic examination of the urine was negative, abnormal findings in the right renal pelvis by DIP, RP and CT led us to suspect a pelvic tumor, and a right nephroureterotomy was performed. Light microscopy revealed deposits of amorphous, eosinophilic material in the pelvis and proximal portion of upper
ureter
, however tumor cells were not found. Electron microscopy revealed that these materials were aggregates of linear, non-branching fibrils, measuring about 8 nm in diameter. Histochemically, these were seen to be consistent with amyloid, producing an apple-orange birefringence with Congo red staining under a polarized light. Alkali Congo red staining after pretreatment of KMnO4 demonstrate that this amyloid protein was composed largely of AA protein in addition to lesser amounts of non-AA protein, and a primary localized
amyloidosis
(amyloid tumor) was diagnosed. A clinicopathologic study that was performed on 12 cases of primary localized
amyloidosis
of the pelvis, including our case, also is reported.

We present a rare case of primary
amyloidosis
involving the
ureter
and renal pelvis. Histologic examination is required to distinguish this condition from transitional cell carcinoma. Intraoperative diagnosis of
amyloidosis
by frozen section may allow for a conservative surgical approach.

An unusual case of localized
amyloidosis
of the
ureter
is described. Suspicion of the condition was first raised during operation when a frozen section was examined. An unnecessary nephrectomy was thus spared. It is a rare condition and awareness of the entity is required to recognise it clinically, radiologically or pathologically. The pathogenesis of localized
amyloidosis
is briefly discussed.

A case of primary
amyloidosis
localized to the inferior renal pelvis of a kidney with a duplex collecting system is described. The onset of gross hematuria and mild flank pain necessitated investigations which yielded a clinical impression of tumor within the lower pelvis of a duplex collecting system. At laparotomy a grossly hemorrhagic-appearing mass in the lower pelvis was noted and a left ureteronephrectomy was undertaken. Histologic examination of the surgical specimen revealed amyloid deposition within the inferior pelvis, the corresponding distal portions of the papillae, and the most proximal
ureter
. Amyloid was not present elsewhere within either the renal parenchyma or the superior collecting system.

Another case of primary isolated
amyloidosis
of the
ureter
is reported; we believe this is the seventeenth such case reported in the world literature. The previous cases are reviewed, and the clinical presentation and treatment are discussed. The importance of distinguishing the condition from transitional cell carcinoma of the
ureter
is emphasized and a conservative approach to surgical treatment is advocated.

This is a case report of a localized
amyloidosis
of the
ureter
presented with gross hematuria.
Amyloidosis
of the
ureter
is a rare entity and 21 cases have been collected from the Japanese and English literature. It is believed to be difficult to diagnose before surgery and almost all the cases were treated by nephrectomy. After malignancy was denied, autotransplantation combined with psoas hitch procedure was carried out in order to preserve the kidney. This is the second case of
ureter
amyloidosis
treated by autotransplantation.
Amyloidosis
was confirmed with Congo red stain and electronmicroscopy of which characteristics is discussed in detail.

We report a case of coexisting amyloid and neoplasm in a localized ureteral lesion. This case also represents the third reported instance of sequential bilateral ureteral
amyloidosis
. Right nephroureterectomy revealed amyloid and an in situ focus of low grade transitional cell carcinoma. Three years later amyloid in the distal left
ureter
was resected and the gap was bridged by a bladder hitch and Boari flap. Subsequent followup has been uneventful.

A 57-year-old man with a history of right-sided renal colic had a stricture of the right
ureter
which was suspected to be caused by a tumour or retroperitoneal periureteric fibrosis. Peroperative frozen section examination revealed tumour-forming
amyloidosis
of the
ureter
. The right kidney therefore could be salvaged by an end-to-end
ureter
anastomosis. Although primary
amyloidosis
of the
ureter
is rare, it should be included in the differential diagnosis of ureteric strictures.
