The oral administration of N-bis(2-hydroxypropyl)nitrosamine (DHPN) in drinking water induced lung tumors in high incidence in rats at 25 weeks. Histologically, they were

The patient was a 63-year-old women who visited our hospital with the chief complaints of swelling, chills, fever and right lumbago. Fifteen years ago, she received surgical exploration for right renal stones at another department of urology. A large, soft and round kidney was palpable from the right upper quadrant of abdomen to the right lower abdomen. A case of multicentric papillary adenocarcinoma arising in the renal pelvis and proximal ureteral mucosa of a 35-year-old man is presented. The left nephrectomy specimen demonstrated multiple, fungating, papillary tumors. The largest tumor measured 10 X 9 X 6 cm and invaded the renal parenchyma. Smaller tumors with long stalks were found. Microscopically, the papillary fronds of each neoplasm were lined by a single layer of cuboidal to low columnar epithelium admixed with a few areas of invasive tubular adenocarcinoma. The adjacent pelvic mucosa was the site of cuboidal epithelial metaplasia. The ultrastructure of both the papillary tumor and the pelvic mucosa suggested that they originated from the nonmucigenic metaplastic epithelium with a partial similarity to the lining of the proximal convoluted tubule. The growth pattern and the multicentric development of this unusual neoplasm suggest that this is a case of the malignant counterpart of the nephrogenic
adenoma
in the kidney and
ureter
, and reflect a biologic behavior corresponding to ordinary urothelial papillary tumors.

Nephrogenic adenoma is a rare, benign lesion of the bladder, occurring as an epithelial response to infection or trauma. Characteristically, it presents as a papillary lesion resembling a low grade, low stage transitional cell carcinoma. Although usually occurring in adults, there have been 5 reports in children, in whom the

occurred following either surgery or localized inflammation. We report on 2 additional children with nephrogenic
adenoma
, including one who presented with a bladder mass and total occlusion of the ipsilateral
ureter
, while in the other it was found incidentally during closure of a vesicovaginal fistula. Successful treatment consisted of transurethral resection and low dose suppressive antibiotics. Nephrogenic adenoma, while rare in children, appears to be a uroepithelial metaplastic response to trauma or inflammation and is treated best by conservative management.

This report presents three cases of colon tumors which developed after an ureterosigmoidostomy. In a checkup, four years after the operation, an inflamed polyp was found in one patient. The second patient developed an
adenoma
in the section of the colon where the
ureter
had been implanted. The last patient died of adenocarcinoma of the colon 26 years after her operation. Possible reasons for the development of the tumors, specifically the cancerous ones, are discussed. The risk of developing colon carcinoma is 500 times higher in those who have had an ureterosigmoidostomy than in healthy people. In the case that the ureters are rediverted the section of the colon where they were previously attached must be excised; since there is a strong possibility of cancerous development. To detect the early development of tumors in the colon, we suggest that patients have their stool tested for blood at regular intervals starting 3 years post operatively. If no problems arise barium enema and coloscopy are recommended every five years.
