Metastatic carcinoma of the maxillary antrum is an extreme rarity. Until 1980, less than 100 cases with distant primaries metastatic to the entire sinonasal tract had been reported. In a review of these cases, we found no mention of primary prostate cancer metastatic to the antrum. The purpose of this paper is to document the first case of this entity.

Metastatic carcinoma of the prostate is a common cause of spinal cord compression. In this review of 37 men who underwent laminectomy for this condition this was the first presentation of previously undiagnosed cancer in 11 (29%). One year after decompression 17 (50%) were alive. Twenty patients (59%) could walk after laminectomy. All but 8 were relieved of pain and bladder function was improved in 13 (38%). Those ambulant before laminectomy (7) and those with occult prostate cancer did particularly well. Poor results were associated with a rapid onset of paraparesis and pre-operative progression to paraplegia. A delay in diagnosis was detrimental to outcome. A high index of suspicion in patients with carcinoma of the prostate is essential so that early diagnosis can be made before paraplegia is established. Carcinoma of the prostate must always be excluded in men with cord compression of unknown aetiology.

Orbital metastasis caused by prostate cancer is rare. We report a case of prostate cancer diagnosed from the initial ocular signs with complaints of decreased vision and exophthalmos. The orbital tumor was surgically removed and endocrine therapy was performed subsequently. Metastatic carcinoma of the prostate should be considered in the elderly male with exophthalmos.

Metastatic carcinoma involving the hyoid bone has rarely been mentioned in the literature. The most frequent malignant lesion involving the hyoid bone metastasized from the larynx, followed by the vallecula and pyriform sinus. The authors report a unique case of occult prostate cancer that presented with an anterior midline neck mass diagnosed as a thyroglossal duct cyst. Two foci of metastatic carcinoma lesions in the hyoid bone were detected after Sistrunk's operation and occult prostate cancer was confirmed by immunohistochemical staining of prostate-specific antigen. Prostate cancer with metastasis to hyoid bone has not been described previously in the literature. The authors report this case which raises important pathological considerations for diagnosis. The unique presentation also reiterates the importance of prostate cancer bone metastasis.

Metastatic carcinoma to the testis, excluding lymphoma and leukemia, is an extremely rare condition. The most frequent primary site is prostate cancer. These lesions present clinically either as the first sign of malignancy or more commonly as a complication during the course of known disease. We present the first case of a squamous cell carcinoma of the lung diagnosed with a testicular mass, which is very rarely seen in literature, to our knowledge.
