I.v. urography is often insufficient for visualization of the intramural and juxtavesical portion of the ureter. In 30 consecutive patients (July 1st 1993-January 31st 1994) where i.v. urography was inconclusive, but had evoked suspicion of pathology in--or adjacent to--the lower ureter, abdominal ultrasound (including color Doppler) and endoluminal (transrectal or -vaginal) ultrasound were performed. A final diagnosis was obtained in 26: Ureteric stone (14),
prostatic cancer
(4), bladder cancer (2), stricture of ureteric orifice (1), TURB sequelae (1),
ureterocele
(1), bladder stone (1), detrusor hypertrophy (1), normal ureter (1). Abdominal ultrasound scanning gave more, same, and less information than i.v. urography in 8, 8, and 10 cases, respectively. In 14 of 22 cases abdominal color Doppler revealed a unilateral abnormal "ureteral jet"--in 13 of the cases on the side with suspected obstruction. Endoluminal ultrasound scanning gave more, same, and less information than i.v. urography in 20, 4, and 2 cases, respectively. The 20 cases where endoscanning gave more information included detection of 14 distal ureteric stones. In 3 cases it identified other types of distal obstruction, in two cases it determined the length of cancer obstruction and in one it detected the presence of ureteral dilatation. It is concluded, that endoluminal ultrasound of the ureter is indicated when i.v. urography evokes suspicion of pathology in the intramural or juxtavesical part of the ureter.

Two instances of simultaneous diagnosis of
prostate cancer
and
ureterocele
were recently identified. In one patient an ectopic
ureterocele
in a duplex system with an obstructed upper pole was unroofed at the time of radical prostatectomy. Surgical excision of the
ureterocele
wall provided decompression of the obstructed system. In a second patient, bilateral intravesical ureteroceles associated with normal renal units were left untreated. Complications were not associated with the untreated ureteroceles. On rare occasions a
ureterocele
may be discovered incidentally during the evaluation of patients with
prostate cancer
. When radical prostatectomy is planned, treatment of the ureteroceles should be determined by the
ureterocele
's size, anatomic configuration, and location and by the degree of obstruction of the affected renal unit. Surgical excision of the
ureterocele
at the time of radical prostatectomy may be the best approach for patients requiring treatment.

The clinical significance of cystoscopy in patients with benign prostatic hyperplasia or
prostate cancer
remains open to discussion. We have always performed cystoscopy with prostate biopsy and have discovered bladder cancer in some patients. The present study investigated the clinical significance of performing cystoscopy with prostate biopsy. Subjects were 458 patients who underwent cystoscopy and ultrasound-guided transrectal prostate biopsy from January 1998 to December 2004. Mean age of subjects was 71.3 years (range, 52-93 years). Prostate biopsy was performed modified Eskews systematic 5-region prostate biopsy (12 core). Some abnormalities were found in 43 of the 458 patients (9.3%). Among these 43 patients, bladder cancer was found in 11 patients (2.4%), and transurethral resection bladder tumor (TUR-Bt) was performed on all 11 patients. Pathological staging of bladder cancer was pT1 and G2 in all cases. Bladder stones were seen in 13 patients (2.8%), benign bladder tumor in 5 patients (1.1%), urethral polyp in 7 patients (1.5%), urethral stenosis in 6 patients (1.3%) and ureteral stones associated with
ureterocele
in 1 patient (0.2%). Appropriate examinations and treatments were performed for all cases. Cystoscopy may be needed at the time of prostate biopsy because: the above-mentioned abnormalities were first discovered on cystoscopy; and the frequency of bladder cancer was 2.4% for the total patient population, and endoscopic surgery was performed.
