Our experiments involving the use of the Dunning R3327 adenocarcinoma as an animal model of prostatic cancer as well as clinical studies on the immunocompetence of prostatic cancer patients are described. Utilizing the Dunning tumor, we have demonstrated that this transplantable adenocarcinoma of the rat prostate was similar to human prostatic cancer with respect to its macroscopic and microscopic appearances, growth rate, growth differential in male and female recipients, and some of its metastatic potential. Cryosurgery was capable of destroying the primary tumor as it can in man. Both antibody and cellular immune responses could be produced against antigens associated with the tumor cells. Tumor-bearing rats treated by cryosurgery in combination with BCG were capable of producing an antitumor immunity that protected them from rechallenge. Clinical studies of prostatic cancer patients showed a diminished in vitro immunity, but the responses of the cancer patients were not significantly different from those of patients with benign prostatic disease.

Primary prostatic disease is usually assessed by digital palpation of the prostate. This method lacks objectivity and has been shown to be inaccurate, especially in the staging of primary prostatic cancer. This paper describes the investigation of 200 subjects using per rectal ultrasound. Of 70 patients with histological evidence of carcinoma of the prostate, 96% were correctly diagnosed ultrasonically and 87% correctly diagnosed by digital palpation. Of the 37 patients with ultrasonic evidence of a capsular breach, only 65% were diagnosed as such by digital palpation. A study of 10 cadaver specimens confirmed the accuracy of per rectal ultrasound as a technique for staging primary prostatic cancer . The ultrasonic appearances of the prostate, seminal vesicles and bladder base are described. Per rectal ultrasound is acceptable to patients, and its use is recommended in all cases where accurate assessment of primary prostatic cancer is required.

The specific red cell adherence test (SRCA) for blood group antigens has been shown to have some bearing on the invasive potential of bladder tumours. Hitherto there have been few data published from patients with prostatic disease. The results of SRCA testing in 69 such patients are presented. Each of the 30 cases of adenocarcinoma was antigen negative. However, as 18 of 39 patients with only benign hyperplasia were also antigen negative, the test clearly does not reflect extant tumour and is probably not an indicator of subsequent growth of prostatic cancer. Antigen expression was also negative in sections showing prostatitis. As the test was invariably negative in patients with adenocarcinoma, whether or not metastases were present and whatever the degree of differentiation of the primary tumour, it lacks the power to discriminate invasive potential.

Computed tomography (CT) and transrectal ultrasound were performed in 88 patients with clinically and histologically proven prostatic disease. An ultrasound diagnosis of prostatic cancer was suggested in 49 of 54 patients with proven disease, whereas CT identified only 21 of those patients. There were no specific features of confined prostatic malignancy on CT and the most reliable feature of an unconfined tumour was infiltration of the posterior aspect of the bladder base. It was not possible on ultrasound to differentiate between prostatic inflammatory disease and a confined carcinoma but ultrasound was accurate in detecting defects in the prostatic capsule, the hallmark of unconfined prostatic malignancy.

We describe the performance of an RIA for the measurement of prostate-specific antigen (PA). Between-assay precision (CV) for control sera with various analyte concentrations was as follows: mean = 1.67 micrograms/L, CV = 7.1%; mean = 4.47 micrograms/L, CV = 5.6%; mean = 7.15 micrograms/L, CV = 5.5% (n = 19 each). Analytical recovery of PA (nine concentrations ranging from 2.3 to 21.1 micrograms/L) added to a serum pool averaged 101.8% (range 96.1 to 116.1%). Sensitivity (detection limit) of the RIA was 0.25 micrograms/L. Cross reactivity of prostatic acid phosphatase (PAP) in this assay was less than 0.022%. The mean percent B/B0 for 74 specimens from women was 98.9%, not statistically different from that for the zero standard. The normal reference interval for men was 0-2.7 micrograms/L ( 99th percentile), as established by assay of specimens from 276 apparently normal men. Measurement of PA and prostatic acid phosphatase in 205 consecutive serum specimens from patients with clinical evidence of prostate disease similarly placed patients into normal (98) or abnormal groups (54) in 152 cases. However, in 49 cases only the concentration of PA in serum was abnormal. Sequential measurement of both tissue markers in specimens from several patients who were undergoing therapy for prostate cancer appeared to provide supplemental information regarding treatment success.

A mobile unit for mass screening of prostatic disease using transrectal ultrasonotomography was developed in December 1980. In the following years we screened 1,396 males over 55 years of age. We have detected 463 cases [33.2%] of benign prostatic hypertrophy [BPH] and eight cases [0.6%] of prostatic cancer. The detection rate of BPH and malignancy of the prostate was shown to be very high. Thus, the unit promises to be of value in preventative medicine projects for prostatic disease in Japan.

Acid phosphatase in serum was measured in 116 patients with prostatic disease, benign in 59 and malignant in 57 cases. Comparisons were made between radioimmunoassay (RIA) and an enzymatic method. The correlation coefficient between the respective values was 0.96 in patients with untreated prostatic cancer, indicating that no significant difference between results with the two methods was to be expected. The correlation coefficient between RIA values and cancer stage was 0.48, and between catalytic activity and cancer stage it was 0.50. The validity of the two methods consequently was equal. RIA, however, was the more sensitive method, giving elevated values in 10 of 11 patients with untreated stage III or stage IV prostatic cancer, as compared with only 4 of the same 11 in the enzymatic assay. This seeming paradox most probably was attributable to differing intrinsic properties of the methods when the upper limits of normal range were established. Neither RIA nor enzymatic analysis discriminated early prostatic cancer (stages I and II) from benign lesions.

Recently, pathologic staging systems have been used to aid in accurate clinical assignment of prostatic disease. With this in mind, we reviewed retrospectively 130 patients who were diagnosed as having prostatic cancer either by needle biopsy or by study of transurethral resection chips. These specimens were graded by the Gleason and Roswell Park histopathologic grading systems. Grading was done by 2 pathologists independently and without knowledge of the results of final surgical staging. All patients were without evidence of metastases by standard diagnostic studies, and all underwent pelvic lymphadenectomy and radical prostatectomy if nodes were grossly negative. The surgical specimens were examined and the final clinical stage was correlated with the histopathologic grading. Independently, the Gleason rather than the Roswell Park system was slightly more accurate and was a more reliable predictor of the surgical stage of the disease. Of the patients with a Gleason sum of 7 or above 86 per cent had at least stage C disease, whereas the disease had been staged preoperatively as A or B. All patients with a Gleason sum of 2 to 5 had stage A or B disease.

Newly developed transurethral ultrasonic scanners were used to study 20 patients with histologically proven prostatic cancer. Fifty-two patients with or without other prostatic disease were also evaluated. The characteristic findings of prostatic cancer were asymmetry of the shape (85%), ill-defined contour (75%), protruded areas from the boundary (70%), elongation of anteroposterior diameter (65%), and enlargement of the prostate (65%). In addition, transurethral ultrasonography delineated a clearer contour of the prostate compared with the transrectal procedure.

An evaluation of longitudinal changes in PSA in men with and without prostate disease revealed that with age, the development of prostate disease is the most important factor influencing changes in PSA. Furthermore, the PSA changes with age are significantly different in men with and without prostate disease. The PSA velocity is greater in men with prostate cancer than in men with BPH and greater in men with prostate cancer and BPH than in men without any prostate disease. Thus, evaluation of PSA changes may help distinguish between men with prostate cancer and those without the disease. This idea will need to be confirmed prospectively. Finally, estimation of PSA doubling time from changes in PSA suggests that changes reflect prostatic growth. Therefore, PSA velocity could be of benefit in identifying men with prostate cancer that is destined to progress.
