Three groups of patients (total 48) with acute exacerbations of chronic bronchitis were treated orally for 10 days, in a double-blind clinical trial, with bacampicillin (an ampicillin ester) 1600 mg twice daily, 800 mg three times daily and oral ampicillin 1000 mg three times daily. Most exacerbations were caused by
Haemophilus
influenzae or Streptococcus pneumoniae. Clinical and bacteriological results were significantly more favourable in the two bacampicillin-treated groups. Both drugs were generally well tolerated. Serum and sputum ampicillin assays after the first dose showed higher and earlier peak levels after bacampicillin. Only after bacampicillin did the sputum levels regularly exceed the ampicillin M.I.C. for the
Haemophilus
influenzae strains. After the 1600 mg and 800 mg dose these levels averaged 0.85 and 0.41 mg/l respectively. One third of the
Haemophilus
influenzae strains studied required more than 0.25 mg/l ampicillin for inhibition of growth.
Bacampicillin
1600 mg twice daily appears to be an effective and safe treatment for most episodes of acute exacerbations of chronic bronchitis.

Bacampicillin
is an orally well-absorbed pro-drug of ampicillin giving high in vivo levels of the latter. Its therapeutic activity was compared with that of amoxycillin in two experimental infection models in mice. The animals were infected with suspensions of Escherichia coli III and
Haemophilus
influenzae 22863 intraperitoneally and treated orally four hours afterwards with one of the two compounds. The antibacterial activity of the compounds was determined as CD50 values or by making viable counts in the blood and in organ homogenates of the animals. Ampicillin and amoxycillin had the same inhibitory but different bactericidal activity against the test strains. Both compounds appeared to have similar therapeutic activity and were found to cause a rapid decrease of the bacterial counts in the animals.
Bacampicillin
appeared to give a more rapid bactericidal activity than amoxycillin against the
Haemophilus
influenzae strain, whereas amoxycillin initially appeared more bactericidal against the Escherichia coli infection.

The safety and clinical efficacy of dosing twice a day with bacampicillin was evaluated in clinical trials conducted at various locations in Latin America, Switzerland, and Scandinavia,
Bacampicillin
(200, 400, 600, or 800 mg) given twice a day was compared with bacampicillin (400 mg) or ampicillin (500 or 556 mg) administered three times a day in eight double-blind and six open randomized trials involving 750 patients with respiratory and urinary tract infections. In a noncomparative study, 47 additional patients were treated with 600 mg of bacampicillin twice a day. High rates of clinical cure or improvement were achieved for all types of infections investigated. Microbiologic cure was observed in greater than 90% of respiratory infections due to beta-hemolytic streptococci, Streptococcus pneumoniae, or
Hemophilus
influenzae. There were no significant differences between the responses of groups treated twice and those treated three times a day. Adverse effects of both regimens of bacampicillin were similar and occurred less frequently than in patients treated with ampicillin. Dosing twice daily with bacampicillin appears to be effective against numerous gram-positive and gram-negative bacteria in a variety of mild clinical infections. Studies comparing identical dosage regimens of bacampicillin and other aminopenicillins are needed to determine whether bacampicillin given twice a day has a therapeutic advantage.
