The somatic (O) and casular (K) antigens of Escherichia coli from the urine of patients with acute pyelonephritis,
acute cystitis
, and asymptomatic bacteriuria, and in the faeces of healthy schoolchildren have been investigated. Typing antisera for sixteen capsular acidic polysaccharide K antigens were used, and five (numbers 1, 2, 3, 12, and 13) accounted for 70% of isolates from patients with acute pyelonephritis. These five K antigens were found to a lesser extent in the three other study groups. Thus, only a few K polysaccharides are associated with virulent properties of E. coli for the upper urinary tract. This finding is similar to the association of only some capsular types of pneumococci, meningococci, and
Haemophilus
influenzae with invasiveness. The identification of virulence markers for E. coli associated with upper-urinary-tract disease may permit more successful control with reference to preventive immunisation.

Pharmacokinetics and clinical effects of cefpirome (CPR, HR 810) in children were studied. When 20 mg/kg and 40 mg/kg doses of CPR were administered to 4 children through 30 minutes' drip infusion, half-lives were 1.23 +/- 0.23 (mean +/- S.D.) hours and 1.37 +/- 0.35 (mean +/- S.D.) hours, respectively for the 2 dose levels, and recovery rates in urine in the first 6 hours after administration were 74.8% and 56.1%, respectively. CPR was administered to 15 cases (3 tonsillitis, 3 bronchitis, 5 bronchopneumonia, 1

influenzae, 3 Staphylococcus aureus, 1 Pseudomonas sp. from these cases. These bacteria in children were followed after administration of CPR. Six strains were eradicated and one was reduced in number. No adverse effects of CPR were observed except in 2 cases, one of which showed transient eosinophilia and the other showed a transient increase of transaminase. These results suggest that CPR may be an effective and safe drug to use on children clinically.

The reasons for combining trimethoprim (TMP) with sulfonamides (SUL) are still mainly theoretical but are supported by results from experimental infections and treatment of specific pathogens in humans, such as Branhamella catarrhalis, Neisseria gonorrhoeae, Brucella, Nocardia asteroides and perhaps Bordetella pertussis and Chlamydia trachomatis. Addition of SUL to TMP confers a therapeutic advantage also in patients with complicated urinary tract infection but probably not in young women with
acute cystitis
. Conditions that may enable TMP-SUL synergy in vivo can be expected to occur only in occasional cases of infection due to staphylococci, streptococci,
Haemophilus
or enteric bacteria. This fact together with ethical problems and availability of alternative therapies make further evaluations of the clinical significance of the SUL component of TMP-SUL very difficult. Although the use of TMP alone has shown promise in exacerbations of chronic bronchitis the role of the SUL component in TMP-SUL treatment of infections outside the urinary tract remains to be defined in comparative clinical trials.

Hemophilus
influenzae has rarely been reported to cause urinary tract infections, but media supportive of its growth are not routinely used for urine cultures. At two Veterans Administration medical centers, H influenzae was isolated from the urine of eight men in the past four years. All had anatomic or functional genitourinary abnormalities, and half had had chronic pyelonephritis or recurrent urinary tract infections. Three patients had
acute cystitis
, two patients had pyelonephritis, two patients had prostatitis, and one patient had asymptomatic bacteriuria with pyuria. Cases were discovered by primary isolation on chocolate agar or sheep's blood agar, by "satelliting" around staphylococci, or by positive urine Gram's stains. Urine Gram's stains disclosed organisms in all six nonprostatitis cases. Organisms were all nonserotypable, were of biotypes 2, 3, or 4, and were beta-lactamase negative.

This study tested the equivalency of minimal inhibitory concentrations (MICs) obtained by two reference susceptibility testing methods, agar dilution and broth microdilution, for gepotidacin when tested against various gram-positive and gram-negative organisms. Equivalency, measured as the essential agreement >89.9%, was established between the two methods for determining gepotidacin susceptibility results against Staphylococcus spp., Streptococcus spp., and Escherichia coli.
