Cultures from the gallbladder and blood of a 60-year-old man with acute cholecystitis grew Haemophilus aphrophilus. This organism, an unusual isolate in clinical specimens, is most frequently seen in patients with either endocarditis or brain abscesses. Haemophilus aphrophilus may be distinguished from Eikenella corrodens and Actinobacillus actinomycetemcomitans on the basis of colonial morphology and the biochemical tests for oxidase and catalase production and fermentation of lactose, sucrose, glucose, mannitol, xylose, and trehalose.

Haemophilus parainfluenzae was isolated from the bile specimens of 2 patients with acute cholecystitis. The strains were genetically unrelated by the random amplified polymorphic DNA (RAPD). The 2 H. parainfluenzae strains represented 11.7% of all positive bile cultures inoculated in blood culture bottles. The routine inoculation of bile specimens in media able to support growth of Haemophilus species should be considered.

A 6-year old girl was admitted to our hospital with high fever and right upper quadrant abdominal pain. At 5 years of age she had undergone allogeneic bone marrow transplantation because of intractable congenital pure red cell aplasia, after which she had asymptomatic cholelithiasis. Imaging studies and laboratory findings on admission suggested acute cholecystitis with a gallstone. The patient recovered completely after laparoscopic cholecystectomy was performed and was discharged on day 7 after the operation. Haemophilus influenzae grew on the aspirated purulent bile. It should be noted that H. influenzae can cause cholecystitis in children.
