In a study of groups of patients with atopic (extrinsic)
asthma
, non-atopic (intrinsic)
asthma
, and chronic bronchitis, no difference could be detected in the numbers having precipitating antibodies against species specific antigens from Staphylococcus aureus or Streptococcus pneumoniae compared to suitably matched control subjects. Precipitating antibodies against species specific antigens from
Haemophilus
influenzae, demonstrated in this investigation by double diffusion in agar gel, were found much more frequently in patients with chronic mucopurulent or obstructive bronchitis (50%) than in either asthmatic subjects (6%) or normal controls (6%) (P = less than 0.0005). While the precipitating antibody demonstrated in these patients against the extracts of Str. pneumoniae and Staph. aureus was in the IgG class alone, IgM and IgA antibody were detected against the species specific but not the non-species specific antigens of H. influenzae. These results underline the importance of H. influenzae as an infecting agent in chronic bronchitis and suggest that the finding of precipitins against the species specific H1 and H2 antigens of this bacterium denotes infection either concurrently or in the recent past. There is no evidence to suggest from this study that infection with Staph. aureus, Str. pneumoniae or H. influenzae is any more common in asthmatics as a group compared to controls or between patients with the non-atopic (intrinsic) and atopic (extrinsic) form of the disease.

The frequency of precipitating antibody to heat-labile (H(1--2) and heat-stable (HCW and HCF) antigens of
Haemophilus
influenzae was determined in patients with
asthma
, chronic bronchitis, cystic fibrosis and bronchiectasis and compared with that in a control group. This showed that the immune response of asthmatic patients to heat-stable antigens was different from that to the heat-labile antigens. Exposure to antigens of H. influenzae is common in all the disease groups. Skin test reactions having the time course and macroscopic appearance of Type I (immediate) and Type III (late) were obtained after prick and intracutaneous skin testing with HCW antigen in varying concentrations in a group of patients with
asthma
, chronic bronchitis or cystic fibrosis and in a control group. It is suggested that IgE and short-term sensitizing IgG antibodies may be responsible for the immediate reactions while activation of the alternative pathway of complement by endotoxin contained in HCW could be responsible for the late reactions. HCW antigens were shown to release histamine from non-sensitized human leucocytes; HCW and HCF antigens were shown to release histamine from non-sensitized human lung. None of the antigens tested had an effect on beta-receptors in tracheal preparations. It is proposed that these reactions may contribute to the pathogenicity of H. influenzae in the lower respiratory tract.

The object of the authors work was the examination of nasopharyngeal swabs on the prevalence of Neisseria meningitidis. A group of 222 patients suffering from
asthma
bronchiale of bacteriological etiology was under observation in order to be able to prepare an autovaccine. Nasopharyngeal swabs were inoculated on special nutrient media. Neisseria meningitidis was found in 22%, mainly from nasopharyngeal swabs, together with
Haemophilus
sp. and Streptococcus pneumoniae. In 59% of examined cases the positive findings lasted longer than 1 year. 46% of isolated strains belonged to group B, about 30% of strains were untypable. The problem of allergisation of people suffering from
asthma
bronchiale with these strains and the question of preparing an autovaccine from Neisseria meningitidis is discussed.

Two antigens designated Pseudomonas aeruginosa cytoplasmic antigen (P(1-5)) and P. aeruginosa cell wal antigen (PCW) were prepared by ultrasonic disintegration and hot phenol extraction of a smooth polyagglutinable strain of P. aeruginosa isolated from the respiratory tract. It was shown that P(1-5) and PCW are immunologically distinct, that P(1-5) is heat-labile while PCW contains a heat-stable component which stains positively for polysaccharide, is positive for endotoxin and cross-reacts with a cell wall antigen of
Haemophilus
influenzae prepared by hot phenol extraction. Both antigens were able to activate the alternate pathway for complement. A statistically significant number of patients with cystic fibrosis and bronchiectasis have precipitating antibody to that fraction of cytoplasmic antigen specific for P. aeruginosa (P(1-2)) and PCW compared to controls, whereas patients with
asthma
and chronic bronchitis do not. The use of both antigens increases the number of patients with antibody to P. aeruginosa. Radioactive immunodiffusion studies indicate that 80.8% of controls have precipitating antibody to PCW antigen and that antibody to it is IgG, IgA and IgM. These studies indicate that consideration should be given to PCW as well as P(1-5) in any consideration of the pathogenesis of P. aeruginosa in these conditions.

Infections of the respiratory airways are frequently responsible for exacerbations of chronic obstructive pulmonary disease (COPD) and attacks of
asthma
. However, the causal infectious agents in practice are rarely precisely identified. We have undertaken a prospective study with the aim of researching into the bacteria and viruses associated with these exacerbations. Forty-seven patients who were in hospital between 1987 and 1989 for attacks of
asthma
(13 episodes) or exacerbations of COPD (35 episodes) were included in this study. The microbiological analysis consisted of: 1) the bacteriology of expectorated material or the products aspirated by fibroscopy with direct examination, quantitative cytology and culture; 2) samples taken from the nasal airways to identify and isolate pneumotropic viruses and mycoplasma; 3) serial serology looking for antibodies against pneumotropic bacteria and viruses. One of more infectious agents were shown in 47% of the episode studies of which 57% were exacerbations of COPD and treated 23% attacks of
asthma
. In the cases COPD bacteria were identified in 13 cases including
Haemophilus
influenzae [3], Streptococcus pneumoniae [3], Pseudomonas aeruginosa [3]. Amongst the 14 viruses recovered, the influenza virus [8] and the respiratory syncytial virus (VRS) [4] predominated. In 14 cases of acute
asthma

Twenty one children with
asthma
aged 1.0-10.5 years (mean (SD) 3.3 (2.5) years) were admitted to the hospital to evaluate pulmonary right middle lobe or lingular collapse lasting one to 12 months (mean (SD) 4.4 (3.8) months). Seven children had mild
asthma
and were treated with inhaled beta 2 agonists as needed. Nine had moderate
asthma
treated with either sodium cromoglycate or slow release theophylline. Five had severe
asthma
treated with inhaled steroids. Each child underwent fibreoptic bronchoscopy under local anaesthesia and a bronchoalveolar lavage. Differential cell counts of the lavage fluid revealed predominance of neutrophils in 12 patients (57%). In nine of these patients cultures grew pathogenic bacteria, mainly
Haemophilus
influenzae and Streptococcus pneumoniae. There was no correlation between the severity of
asthma
and a positive bacterial culture. There was also no correlation between the duration of the right middle lobe collapse and a positive culture. We conclude that longstanding right middle lobe collapse in asthmatic children is often associated with bacterial infection.

Some studies suggest a potential role for bacterial respiratory tract infections in the development of bronchospasm and the progression of chronic obstructive pulmonary disease (COPD). Patients with bronchiectasis or cystic fibrosis have exaggerated airway reactivity; croup in children can also cause exaggerated upper and lower airway responsiveness. Bronchial obstruction after inhalation of
Haemophilus
influenzae and other bacteria has been reported. Between January 1989 and June 1990 we and two other centers studied 193 patients suffering from acute exacerbation of
asthma
. Fifty-two (27%) of these patients had bacteria in their sputum. Streptococcus pneumoniae, Streptococcus pyogenes, Staphylococcus aureus, Moraxella catarrhalis, and H influenzae were the most commonly isolated bacterial species. Antibiotics may be of value in the treatment of infective lung disease, not only by killing bacteria but also by preventing increases in bacterial histamine levels within the lung airways. Moreover, an antibiotic of proven efficacy can reduce airway reactivity in patients with bacterial exacerbations of COPD or bronchial
asthma
. In 12 patients with acute bacterial exacerbation of
asthma
and high airway reactivity to methacholine, a ten-day course of treatment with cefaclor and existing bronchodilators induced microbiologic cure and a slight but nonsignificant change in airway reactivity in nine patients. Antibiotic therapy has a minor but clear role in the control of acquired bronchial hyperreactivity during bacterial respiratory infections in asthmatic patients; however, because of airway inflammation, an antibiotic's efficacy is evident only when the inflammatory process subsides. Isolated bacteria from respiratory infectious diseases were collected in cooperation with institutions located throughout Japan, since 1981, and Ikemoto et al. have been examining sensitivities of the isolates to various antibacterial agents and antibiotics, relationships between the isolates and the backgrounds of the patients and so forth each year. We report here the research results for the year 1988. In 18 institutions around the entire Japan from October 1988 to September 1989, 554 strains of bacteria were isolated mainly from the sputa of 439 patients with respiratory infectious diseases and assumed to be the etiologic bacteria. MICs of various antibacterial agents and antibiotics against 68 strains of Staphylococcus aureus, 102 strains of Streptococcus pneumoniae, 120 strains of
Haemophilus
influenzae, 86 strains of Pseudomonas aeruginosa, 65 strains of Branhamella catarrhalis, 18 strains of Klebsiella pneumoniae and so forth, were determined, and the drug sensitivities of these strains were examined except for the strains which died during transportation. The drug sensitivities of the main strains were almost the same as those determined last year for each drug. However, S. aureus strains for which MICs of methicillin were higher than 12.5 micrograms/ml (methicillin-resistant S. aureus) accounted for 38.2%, and the frequency of drug resistant bacteria increased over last year's 18.2%. Also, we examined changes in the backgrounds of patients, the infectious diseases, and the etiologic bacteria and so forth. As to patient backgrounds, there were many infectious diseases found in a high age bracket, and the patients over age 60 accounted for 57.2% of the diseases. In the distribution by disease, bacterial pneumonia and chronic bronchitis accounted for greatest numbers of cases 32.1% and 31.4%, respectively, followed by bronchiectasis and bronchial

As for frequencies of etiologic bacteria by disease, S. aureus (22.5%) and S. pneumoniae (15.4%) in pneumonia, S. pneumoniae (25.7%) and H. influenzae (24.1%) in chronic bronchitis, H. influenzae (32.5%) and P. aeruginosa (23.8%) in bronchiectasis, and H. influenzae (31.4%), S. pneumoniae and B. catarrhalis (20.0%) in bronchial
asthma
were the most frequent. Regarding effects of administration of antibiotics and isolates obtained on each day after infection, those bacteria which were isolated before antibiotic administration and which decreased after administration included S. pneumoniae, H. influenzae, and B. catarrhalis. Frequencies of S. aureus and P. aeruginosa, however, increased after antibiotic administration. Also, when dosing continued for more than 15 days, the frequency of P. aeruginosa increased rapidly.

To better understand the factors involved in chronic sinusitis in childhood, we cultured the sinuses, middle meatus, and nasopharynx in 39 children requiring surgical intervention. Sixty-nine percent of these patients had other medical problems, including
asthma
(49%) and immunologic compromise (18%). We cultured coagulase-negative staphylococcus in 18 patients, Streptococcus viridans in 14 patients, normal flora in 10 patients, Staphylococcus aureus in nine patients, group D streptococcus in five patients, Corynebacterium in five patients,
Haemophilus
influenzae in three patients, Neisseria in three patients, and Streptococcus pneumoniae, group A streptococcus, Escherichia coli, Pseudomonas aeruginosa, Klebsiella oxytoca, Propionibacterium acnes, Actinomyces, and an anaerobic gram-negative bacillus in one patient each. Cultures yielded no growth in nine patients. A strong association between cultures of the middle meatus obtained ipsilaterally and cultures of the maxillary (83%) and ethmoid sinuses (80%) occurred. A poor correlation was found between cultures of the nasopharynx and maxillary (45%) and ethmoid sinuses (49%). All seven patients who had both maxillary and ethmoid sinus cultures showed the same organisms in both sinuses. Only 41% of organisms were found on both sides when procedures were performed bilaterally. Cultures of the middle meatus appear to be sensitive and specific for organisms within sinuses. The presence of predominantly nonvirulent organisms in low titers suggests that additional factors other than bacterial overgrowth contribute to the pathogenesis of chronic sinusitis in children.

Sixty-one patients with chronic sinusitis who were referred for an allergy evaluation were evaluated for immunologic competence including assessment of quantitative serum immunoglobulin levels, IgG subclass levels, and response to pneumococcal and
Haemophilus
influenzae vaccines. In addition to chronic sinus disease, recurrent otitis media and
asthma
exacerbation were common problems in this group. Five patients had an elevated age-adjusted IgE level and 22 patients had positive prick tests to one or more environmental inhalants; these findings suggest an allergic component in this subgroup. Twelve additional patients had highly reactive intradermal tests to common environmental allergens, which also may be clinically significant for underlying atopy. Eleven patients had low immunoglobulin levels, 6 had low immunoglobulin levels and vaccine hyporesponsiveness, and 17 had poor vaccine response only. Thus, 34 of 61 patients with refractory sinusitis had abnormal results on immune studies, with depressed IgG3 levels and poor response to pneumococcal antigen 7 being most common. In addition to allergy, immunologic incompetence may be an important etiologic factor in patients with chronic, refractory sinusitis.
