The purpose of this study was to evaluate the effect of selenium (Se) in the prevention of human primary liver cancer. Three intervention trials were conducted among the residents at high risk to primary liver cancer (PLC) in Qidong county, Jiang-su province, the People's Republic of China. This area has the second highest rate of PLC in China. One trial was undertaken among the general population in a township with supplement of table salt fortified with 15 ppm anhydrous sodium selenite (Se-salt) for 5 y and the other four townships with similar PLC incidence rate served as the controls using normal table salt. The second trial was undertaken among hepatitis B virus surface antigen carriers (HBVsAg+) receiving supplement of 200 micrograms Se in form of selenized yeast (Se-yeast) daily vs placebo for 4 y. The third trial was carried out in members of families with high PLC incidence using Se-yeast (200 micrograms of Se daily) vs placebo for 2 y. The results showed that nutritional supplement of Se could reduce the PLC incidence significantly.

A long-term study, using male Wistar rats, was initiated to determine whether the effects of dietary constituents on AFB1-induced liver cancer could be associated with altered microsomal enzyme activity. They were maintained on mice pellets mixed with specific dietary constituents for 7 days and then given a single carcinogenic dose of AFB1 (500 micrograms/rat). After three months, the dietary constituents were discontinued and the animals were left on mice pellets and drinking water only for a period of about 20 months. At the end of the trial period, it was observed that dietary mixtures containing small quantities of either beta-carotene, ascorbic acid, GSH, vitamin E, selenium salt, or uric acid, effectively inhibited the development of AFB1-induced liver cancer and induced increased microsomal enzyme activity. Whereas beta-carotene and uric acid were the most effective inhibitors, vitamin E was the least, yet a significant inhibitor of liver cancer. Hepatic levels of cytochrome P-450, aniline hydroxylase and chlorpromazine demethylase were significantly induced in rats fed fortified food followed by AFB1 treatment than in control animals. The inhibition of liver cancer by dietary factors was probably due to their ability to induce the activity of hepatic microsomal enzymes. Increased enzyme activity could lead to rapid activation of AFB1 metabolism, resulting in loss of activated AFB1 metabolites that attack cell components. Inhibition of liver cancer is therefore associated with induction of increased microsomal enzyme activity.

This study was conducted to investigate the relationship between life style factors and adult disease for Chinese living in Japan. The mortalities of major cancers and other major diseases of Chinese in Japan were compared with those of Japanese by calculating Standardized Mortality Ratios (SMR) for the Chinese using death rates in the Japanese population the standard. The life style data on smoking, drinking and dietary habits for Chinese in Japan were collected by self-administered questionnaire surveys, and age-adjusted proportions were calculated with the truncated world population as the standard. Then the corrected indexes on life style for Chinese in Japan were compared with those of Japanese. The results are summarized as follows: 1. The mortality rates of heart disease, diabetes mellitus, hypertensive disease, liver cirrhosis, rectum cancer, liver cancer (both sexes), lung cancer (females), breast cancer and cerebrovascular disease (females) for Chinese in Japan were higher than those for Japanese, but the rates of stomach cancer, pancreas cancer (both sexes), uterus cancer (females) and cerebrovascular disease (males) were lower than those for Japanese. 2. The prevalence of current smokers for Chinese males in Japan was lower than that of Japanese, and that of females was higher than that of Japanese. The prevalence of non-smokers for Chinese males was higher than that of Japanese, and that of females was lower than that of Japanese. 3. Although the prevalence of regular drinkers for Chinese of both sexes in Japan were lower than that of Japanese, the prevalence of heavy drinkers who drank over 80 ml of ethanol every day for Chinese males was higher than that of Japanese males. 4. Significant differences were not found in the prevalences of frequent consumers of meat, milk, eggs, fish, other vegetables and food using oil between cooks and non-cooks of Chinese of both sexes in Japan. 5. The age-adjusted prevalences of frequent meat and milk consumers for Chinese in Japan were higher than those of Japanese in both sexes, but those of frequent pickled vegetable and MISO soup consumers were lower than those of Japanese. The dietary pattern of Chinese in Japan was different from that of Japanese with intakes of much fat and less salt. 6. It is assumed that the mortalities due to adult disease for Chinese in Japan are related to their heavy drinking and to their dietary habits.

Nonhistone chromosomal proteins (NHCP) from normal, regenerating rat liver, fetal liver, stages during acetylaminofluorene (AAF) and diethylnitrosamine (DEN) induced carcinogenesis, and resultant primary hepatocellular carcinomas (PHC) were analyzed by two-dimensional polyacrylamide gel electrophoresis (2D-PAGE). These studies sought to determine if changes in proteins putatively involved in catalyzing specific gene expression (NHCP) occur during liver cancer development that might be related to the malignant phenotype. NHCP extracted in high salt-urea buffers, analyzed by 2D-PAGE and silver staining were resolved into some 530-560 polypeptides. Increases in number of NHCP amounting to 8.4, 8.6 and 8.8%, respectively, were detected in AAF induced nodules (AAF-NOD), AAF-PHCs and DEN-PHCs when compared to normal chromatins. The majority of the 51 qualitative changes detected reflected cell cycling and/or reexpression of fetal-NHCP. Within the total changes, seven new NHCP were found only in AAF- and DEN-induced PHCs. Further, four NHCP with isoelectric points and relative molecular weights (pI/MW) of 5.62/19.3, 5.96/30.7, 6.25/46.6 and 8.16/53.5 occurring in both AAF- and DEN-PHC also were found in AAF-NOD, a carcinogenesis stage considered to represent premalignant nodules. Reciprocally, three NHCP of pI/mol.wt.: 6.81/34.0, 5.82/43.7 and 8.18/67.0 present in normal liver, disappeared in all carcinogen involved tissues analyzed. These findings indicate that while AAF and DEN exposure results in a number of qualitative NHCP changes specific for the particular carcinogen, a total of only ten changes, seven inductions and three losses, occurred in common during hepatocarcinogenesis induced by these diverse agents. At least four of these NHCP may prove critical inductions during malignant conversion or alternatively might serve as tumor markers since they appear first in a well characterized premalignant stage and persist in resultant tumors.

The geographical differences in mortality from cancer of seven sites of the digestive organs and consumption of foods in 46 of the prefectures, excluding Okinawa and their capital cities were statistically observed. The groups of foods statistically associated with cancer death are: pork, cooking oil and shochu (low class distilled spirits) for esophageal cancer; fresh fish, salted fish, vegetables and alcoholic beverages for stomach cancer; alcoholic beverages, salted or dried fish, vegetables, bread, milk, butter, margarine, ketchup, beer and fresh fish for colonic cancer; fresh fish, salted or dried fish, salt and popular grade sake for rectal cancer; pork, popular grade sake and green tea for cancer of the biliary passages; salted or dried fish, vegetables, alcoholic beverages, oil and fresh fish for pancreatic cancer; beef, poultry, eggs and vinegar for liver cancer. Further epidemiological analyses are required to find the biological causal relationships.

High rates of hepatitis B virus (HBV) infection and primary liver cancer (PLC) are present in Qidong county. Epidemiological surveys demonstrated an inverse association between selenium (Se) level and regional cancer incidence, as well as HBV infection. Four-year animal studies showed that dietary supplement of Se reduced the HBV infection by 77.2% and liver precancerous lesion by 75.8% of ducks, caused by exposure to natural environmental etiologic factors. An intervention trial was undertaken among the general population of 130,471. Individuals in five townships were involved for observation of the preventive effect of Se. The 8-yr follow-up data showed reduced PLC incidence by 35.1% in selenized table salt supplemented vs the nonsupplemented population. On withdrawal of Se from the treated group, PLC incidence rate began to increase. However, the inhibitory response to HBV was sustained during the 3-yr cessation of treatment. The clinical study among 226 Hepatitis B Surface Antigen (HBsAg)-positive persons provided either 200 micrograms of Se in the form of selenized yeast tablet or an identical placebo of yeast tablet daily for 4 yr showed that 7 of 113 subjects were diagnosed as having PLC in the placebo group, whereas no incidence of PLC was found in 113 subjects supplemented with Se. Again on cessation of treatment, PLC developed at a rate comparable to that in the control group, demonstrating that a continuous intake of Se is essential to sustain the chemopreventive effect.

In order to investigate the aetiological factors of Primary Liver Cancer (PLC) in Shunde City of Guangdong province, 96 clinically diagnosed PLC patients and 144 matched hospital controls were interviewed and their blood samples were examined for HBV, HCV and other seven indices. Monofactorial and multifactorial analyses were fitted by using Non-conditional Logistic Regression model. The findings confirmed the strong association between HBV infection and PLC. Histories of hepatitis and histories of eating raw fish, shrimp or salt fish were also noticed. The history of alcohol intake might have associated with PLC. The present study did not find associations between PLC and drinking water, histories of blood transfusion and injection- or exposure to insecticides. Antibody of HBV surface antigen seemed to be a protective factor with a relative risk of 0.3064 (0.1647-0.5701). The results showed that the combined effects of HBsAg infection and HCV infection were worthy for further study.

The role of glucose-6-phosphate dehydrogenase (G6PDH) in oxidative stress responses was investigated in isolated intact living hepatocytes of immature female and male European flounder (Platichthys flesus L.) because it is the major provider of NADPH needed as reducing power for various detoxification pathways. Hepatocytes were exposed to sublethal concentrations of effective prooxidants such as 100 microM hydrogen peroxide (H(2)O(2)), 100, 200 and 400 microM benzo[a]pyrene (B[a]p) and 50 microM nitrofurantoin (NF) during culture. Since there is evidence that 17-beta-estradiol inhibits certain pathways of xenobiotic biotransformation, we tested also the effects of different concentrations of 17-beta-estradiol (0.2, 1 and 2 microM) alone and 1 microM in combination with 100 microM B[a]p on G6PDH activity. After short-term (1 day) and long-term (9 days) exposure, G6PDH activity was quantified in intact living hepatocytes by a tetrazolium salt method using tetranitroblue tetrazolium salt (TNBT). Hepatocytes obtained from male fish generally showed higher G6PDH activity than those of females. We observed significant inhibition of G6PDH activity by all oxidative stressors and 17-beta-estradiol in both sexes of fish independently of culture conditions, but inhibition was stronger in cells of females than in cells of males. A cumulative effect of the steroid and B[a]p was not found. Our results indicate a sex-dependent inhibitory effect of all stressors and 17-beta-estradiol on G6PDH activity in flounder hepatocytes independent of prooxidant activity of the specific compound. Consequently, NADPH supply for xenobiotic detoxification and other cellular antioxidative defence mechanisms may be different in livers of female and male flounder. The strongly decreased supply of NADPH in hepatocytes of females may explain the reduced and/or delayed NADPH-dependent activity of xenobiotic biotransformation systems such as cytochrome P450 (CYP450) and a lower capacity of non-enzymatic defence systems such as reduced glutathione that is particularly observed in female flounder. Moreover, the strong inhibition of G6PDH in livers of female flounder may explain higher susceptibility for xenobiotic toxicity and, therefore, potentially a higher risk to develop liver cancer.

A total of forty-five halophilous microorganisms were isolated from the sediment of saline lake in Qinghai Province, among which, filamentous fungus F16 showed the highest activity of anti-microorganism and anti-tumor. The ethyl acetate extract of F16 culture filtrate showed a strong cytotoxicity, and could inhibit the growth of four kinds of bacteria, especially Escherichia coli. When the concentration of the crude extract was 50 microg x ml(-1), the inhibition rate to liver cancer cell BEL7402 reached 76. 91%. The optimal temperature for F16 growth was 15 degrees C , and the increase of salt concentration in media would inhibit its growth. When the concentration of salt surpassed 15% , F16 could not survive. F16 grew well when the pH value ranged from 5 to 9.

Nitrosamines are know as the most potent group of carcinogens. Approximately 300 of these compounds have been tested, and about 90% of them have been found to be carcinogenic in laboratory animals. N-nitrosodimethylamine causes liver cancer, whereas some of tobacco specific nitrosamines causes lung cancer. Volatile N-nitrosamines induce tumors in a variety of human organs, including the tongue, esophagus, lung, pancreas, liver, kidney and bladder. They are formed during reaction of secondary or tertiary amino compounds and nitrite or nitrogen oxides. Nitrosamines occurs as contaminants in many foodstuff including food and beverages: beers, cheeses, sausages, smoked and pickled foods. They are formed during frying, smoking and food preserved with pickling salt. These compounds can also be produced in man and other mammals under the acidic conditions in the stomach. The present study was carried out to determine level of N-nitrosodimethylamine in selected 13 meat products. The extraction procedure was based on Raoul's method, ie. on two consecutive extraction-concentration step using extrelut and florisil columns. The level of N-nitrosodimethylamine was varied from 0.049 mg/kg to 16.47 mg/kg. The highest level of NDMA was found in smoked sausage.
