Vitamin D has been proposed as a risk factor of ischaemic heart disease. In 12 patients with
acute myocardial infarction
the major circulating vitamin D metabolite, 25-hydroxy-cholecalciferol (25-HCC), did not show any fluctuations during the first 4 days after onset of symptoms. The serum 25-
HCC
level was then measured in 128 patients consecutively admitted because of chest pain, 53 of whom had myocardial infarction and 75 had angina pectoris. The values found did not differ from those measured in 409 normal persons. The seasonal variations of serum 25-
HCC
were less pronounced in heart patients than in normals, probably due to less sun exposure in the summer months. The levels of serum 25-
HCC
did not correlate with the concentrations of serum cholesterol, glycerides, calcium or magnesium. Low serum calcium and magnesium were observed in all patients. Serum calcium was further reduced in the course of acute myocardial infarctions while serum parathyroid hormone rose significantly. We conclude that patients with ischaemic heart disease are not ingesting or producing in their skin elevated amount of vitamin D.

The association between oral contraceptives (o.c.) and disease risk was reviewed on the basis of data from a network of a case-control studies conducted in northern Italy since the early 1980's on about 150 cases below age 55 with
acute myocardial infarction
, 150 with gallstone disease, 350 with uterine fibromyoma, 170 with endometrial cancer, 700 with benign or malignant ovarian tumours, 2000 with breast cancer, 360 with intraepithelial and 370 with invasive cervical cancer, 20 with
liver cancer
plus over 2000 control women admitted to hospital for acute, non hormone-related non neoplastic diseases. The relative risk (RR) of myocardial infarction was 2.1 (95% confidence interval from 0.7 to 7.1) among current o.c. users, but only 4% of women were current users. There was no association between gallstone disease, uterine fibromyoma and o.c. use. Significant protections were observed with reference to endometrial cancer and benign, borderline and malignant ovarian tumours, while the RR was above unity (RR = 1.9) for invasive cervical cancer, but not for intraepithelial cervical neoplasia. A significantly increased risk was observed for primary
liver cancer
, which is however extremely rare in young women. With reference to breast cancer, there was no consistent duration-risk relationship, and the RR was 0.8 for use for 5 or more years. Thus, these data provide reassuring information on the relationship between o.c. use and the risk of several important diseases in a Southern European population.

The relationship between drinking habit surveyed in 1965 and cause-specific mortality over 19 years was investigated in 5135 male Japanese physicians taking into account smoking habit and separating ex-drinker from non-drinker. As compared with non-drinkers, daily drinkers with high consumption had a significantly increased mortality from all causes. Drinking was significantly related to the so-called alcohol-related causes of death; upper aerodigestive cancer,
liver cancer
and liver cirrhosis. Mortality from
acute myocardial infarction
was inversely related to drinking, whereas other coronary heart disease showed a somewhat higher mortality among men consuming a large amount of alcohol than among non-drinkers. There was a weak, but significant, association between stroke and drinking, and the relation did not differ between haemorrhagic stroke and other stroke. No obvious relationship with drinking was observed for cancers of the stomach, large bowel, pancreas and lung.

After unsuccessful cardiopulmonary resuscitation efforts, brain death was diagnosed. In the absence of anemia, acidosis, electrolytic alterations or hypothermia, and after ruling out hemoperitoneum,

Terahertz (THz; 10(12) Hz) waves have a frequency from 0.1 to 10 THz between the visible light and microwave domains. THz waves are expected to be useful for analysis of the histological features, without any staining procedure that is an indispensable prerequisite for optical microscopy. It has been demonstrated that THz transmittances at cancer and normal tissues are different. However, spectroscopy that is currently used is applicable for imaging only small areas at fixed-wavelength. In this study, we have developed a spectrometer employing a gallium phosphide (GaP) THz-generator and applied it to examine large areas of tissue specimens using a wide range of wavelengths. We thus examined the whole areas of two paraffin sections (metastatic
liver cancer
and
acute myocardial infarction
) in a frequency range of 1 to 6 THz, and compared the THz images of ordinary paraffin sections with the histological features detected by microscopy. THz imaging showed striking contrasts between cancerous and non-cancerous regions at 3.7 THz. Likewise, the precise imaging was achieved in the infarct myocardium at 3.6 THz. Images of THz transmittances in optimal wavelength were well matched with HE histological features both in cancer and myocardial tissues. Cancer regions showed higher transmittance than non-cancerous regions in liver. Old scar regions showed low transmittance, and necrotic regions showed relatively higher transmittance than normal myocardial areas. Thus, THz imaging precisely reflects tissue conditions such as tumor, non-tumor tissues, tissue degeneration and fibrosis. The newly established THz spectroscopy would be useful for pathological diagnosis of routinely processed specimens.
