Diclofenac sodium
is a non-steroidal anti-inflammatory drug widely used as an opioid sparing agent for postoperative
analgesia
. Anaphylaxis due to intravenous diclofenac sodium is very rare. We report a case of anaphylactic reaction to IV diclofenac sodium, occurring postoperatively in a 25-year-old primigravida, the clinical features of which mimicked pulmonary embolism. The rarity, clinical importance and the diagnostic dilemma associated prompted us to report this case.

Diclofenac sodium
is a very common drug used in medical practice for
analgesia
and is freely available over the counter without any medical prescription. The drug is also the most commonly used or misused by quacks working all over India. Many case reports have been published on upper limb catastrophe of unintentional intra-arterial injection of barbiturates, thiopental sodium, narcotics, and tranquilizers but only two cases of inadvertent intra-arterial injection of diclofenac have been reported till date. Potential serious complication of inadvertent intra-arterial injection of diclofenac sodium leading to gangrene has been recently reported in medical literature. It is a medical emergency and literature has shown that timely diagnosis with early intervention can salvage the limb. We hereby report a case of accidental intra-arterial injection of diclofenac sodium in the ulnar artery by a quack for pain abdomen leading to gangrene of the distal part of affected little, ring and part of middle finger of right hand.

The current study aimed to evaluate the analgesic effects of crude extract and fractions of leaves of Adansonia digitata L. (Malvaceae). Fresh leaves were collected, dried under shade and then powdered. Moreover, powder was usedto extract with Ethanol and concentrated under reduced pressure, followed by partitioning with organic solvents i.e. Hexane, Ethylacetate, Butanol and Aqueous. These fractions were analyzed for analgesic effect at dose of 50, 100, 200; mg/kg body weight by acetic acid induced writhing, tail flick and hot plate analgesic models. No acute oral toxicity was observed and extracts considered being saved at a dose of 50-3000 mg/kg body weight. Orally administered extract of A. digitata showed analgesic activity described by decrease in writhes counts which was contrast to control.
