The use of alfentanil was studied in 49 patients undergoing surgery with a duration of at least 90 minutes. Induction was performed with droperidol (Dehydrobenzperidol) and etomidate (Hypnomidate), maintenance with nitrous oxide/oxygen 2:1 and alfentanil. Alfentanil was administered as an initial bolus of 2-3 mg depending on the body weight of the patient, immediately followed by a continuous infusion of 0.75 microgram/kg/min. In case a stressful or painful event in surgery caused inadequate analgesia, additional increments of 1 mg of alfentanil were administered. Anesthesia was considered to be good in 90% of the patients. The number of additional increments needed did not increase as the anesthetic procedure progressed in time. In virtually all patients postoperative recovery was fast and not related to the number of increments. It is concluded that alfentanil infusion for longer procedures in combination with nitrous oxide/oxygen provided a good and easy method for maintaining a satisfactory level of analgesia.

As mini-abortions or vacuum aspirations (VA) are almost always considered painful procedures, effective analgesia and anesthesia have to be provided. The most effective form of anesthesia was studied. Hypnomide (ethyl-1-phenylethyl-1-H-imidazole-5-carboxydate) made by the Jansen firm was administered iv to 106 patients in doses of 0.2-0.3 mg/kg. In the first 5 cases where only plain anesthetic was given, strong spontaneous muscle contraction was observed. As the efforts to neutralize the side effects caused by diazepam proved ineffective, fentanyl was later used for the preparation of anesthesia and proved to be the most effective. An average dose of 0.05-0.0525 mg fentanyl and 10-12 mg Hypnomidate was given iv and the optimum time of operation was 70-80 seconds after administration of the preparation. Adequate verbal response was observed after an average of 10 minutes and adequate motor response and clear consciousness after an average of 13 minutes. Spontaneous muscle twitching also occurred in 7% of the cases, however, this did not interfere with the progression of the procedure. No one found the procedure painful. The combination of Hypnomidate-fentanyl was considered the most effective, as it was less traumatizing psychologically, it could be applied on an out-patient basis, and since the cessation of its effect was fast, it would not prolong the operation.

This study compares methohexitone (Brietal; Eli Lilly) and etomidate (Hypnomidate; Janssen) as hypnotic agents for the induction and maintenance of general anaesthesia in outpatients given fentanyl and nitrous oxide for analgesia but no premedication. While both agents provided suitable anaesthesia with a minimal incidence of side-effects, 66% of the patients who received methohexitone were noticeably drowsy 2 hours after the operation, whereas all but 3% of the patients who received etomidate were wide awake.
