A comparative study was made of the circulatory conditions during ESA via paravertebral electrodes and halothane anaesthesias and neurolept

. During 654 operations (cholecystectomies,
struma
-surgery, vein by-pass grafting and varicotomies) heart rate and mean arterial pressure before the onset of anaesthesia and the highest intraoperative measured rates were compared. The same parameters after intubation/before the start of the operation respectively before and after the start of the operation were compared with regard to the specific type of anaesthesia. Before the start of surgery no marked reactions of the circulatory system were observed during ESA compared to other anaesthesia techniques. No difference in heart rate and mean arterial pressure were found during the start of the operation with the different anaesthetic techniques. For

surgery the reaction of the circulatory system during ESA was significantly better than in halothane anaesthesia while during surgery of the upper abdomen and vein by-pass grafting in ESA patients there were significantly greater rises in heart rate and mean arterial pressure.

Remifentanil undergoes extensive placental transfer and has been used to provide fetal immobilization and anaesthesia for in utero fetal endoscopic interventions. We report three cases of the ex utero intrapartum treatment performed under neuraxial anaesthesia where the maternal administration of remifentanil was used to provide fetal immobilization and
analgesia
. Fetal pathology included
goiter
and arthrogryposis, with one case requiring a tracheostomy. The longest time on placental circulation was 21 min. No clinically significant maternal sedation or respiratory depression was observed. In all cases, remifentanil provided adequate fetal immobilization and obviated the need to administer other analgesics or neuromuscular blocking agents. Remifentanil is a useful adjunct for ex utero fetal procedures.

Anesthetic management of patients with mediastinal masses remains challenging as acute cardiorespiratory decompensation may follow induction of anesthesia. We describe a 57 year old lady with massive retrosternal
goiter
and severe intrathoracic tracheal compression who had a total thyroidectomy. Comprehensive contingency plans were an essential prerequisite for successful management of difficult airway, including multidisciplinary involvement of otorhinolaryngologic and cardiothoracic surgeons preparing for rigid bronchoscopy and cardiopulmonary bypass. Awake oral fiberoptic intubation was performed under dexmedetomidine sedation. Severe tracheal narrowing necessitated usage of a 5.0 mm uncuffed flexometallic endotracheal tube. Anesthesia was maintained with sevoflurane and dexmedetomidine infusion with target controlled infusion of remifentanil as
analgesia
. No muscle relaxant was given. Surgical manipulation led to intermittent total tracheal compression and inadequate ventilation. The tumor was successfully removed via the cervical approach. A close working relationship between anesthesiologists and surgeons was the key to the safe use of anesthesia and uneventful recovery of this patient.
