Central nervous system involvement with mycosis fungoides complicated the clinical course of a patient at a time when his skin was clinically free of disease following systemic chemotherapy. A leptomeningeal syndrome of
blurred vision
and papilledema, and confusion progressing to coma, was associated with elevated spinal fluid pressure and abnormal spinal fluid cells morphologically similar to those seen in the Sezary syndrome. The symptoms were dramatically reversed by intrathecal methotrexate, brain irradiation, and steroids. Mycosis fungoides recurred in the skin, in the spinal fluid, and in both eyes. Despite continued systemic and intrathecal chemotherapy, the patient died from mycosis fungoides. This is the second patient reported with meningeal mycosis fungoides.

Thirty-five patients with known cerebral lesions complained of recently acquired
blurred vision
. None of them had evident oculomotor or pupillary abnormalities, and each had intact central fields and normal visual acuity by conventional standards. Examination of spatial contrast sensitivity was carried out with sinusoidal grating patterns, by determining the minimum contrast between alternate light and dark bars required to distinguish the pattern from a homogeneous field. This was done at each of several spatial frequencies which were established by varying the width of the bars and hence the coarseness of the pattern. The contrast sensitivities were plotted as "visuograms" which, by analogy to audiograms, record the sensitivities in comparison to normal standards. Of the 35 patients, most showed significant losses, amounting to greater than 50 per cent elevation of contrast thresholds. Eighteen showed high frequency losses; 11 had uniform reductions over the entire visible spatial frequency range and 6 had selective frequency losses in the intermediate frequency ranges. These defects in spatial contrast sensitivity, which were not predictable from standard acuity scores, indicate that the visual symptoms in our patients may have been caused by damage to frequency-selective neural elements in the central visual systems. The method may be used to advantage in clinical investigations as well as in physiological investigations of the functional pathways subserving central vision.

A surgically cured case of chronic optochiasmal arachnoiditis presumably caused by concomitant right frontal arteriovenous malformation (AVM) is reported. When a 12-year-old boy, suffering from

for about 1 month, visited the Ophthalmological Clinic of Hirosaki University Hospital, only a chiasmal lesion was suspected because neuro-ophthalmological examination revealed no more than ophthalmic signs, such as bilateral diminution of vision (Vd, Vs = 0.2), concentric narrowing of visual fields and slight bilateral papilledema. The patient was referred to the Neurosurgical clinic for further examination. Left carotid angiograms showed no abnormalities but, thereafter the brain scintigram revealed an abnormal hot area in the right frontoparietal region. A right carotid angiography demonstreated an AVM in the posterior part of the right frontal lobe. This was fed through the right anterior and middle cerebral arteries and drained into the superior sagittal sinus through meandering dilated cortical vein.

A case of central nervous system actinomycosis is reported. A 33-year-old male complained of headache, vomiting and
blurred vision
lasting for eight days prior to admission. On examination, a right hemiparesis, as well an intracranial hypertension were detected. The cerebrospinal fluid showed mild lymphomononuclear hypercytosis. Necropsy disclosed three abscess in the cerebral hemispheres, in addition to moderate cerebral edema on the left side but without purulent leptomeningitis. Actinomyces filaments and granules were demonstrated in the cerebral and lung abscessess. The Brazilian literature on actinomycosis is reviewed and six published cases with nervous system involvement were found. Relevant clinical and anatomical aspects of the cases and of the present one are discussed.

Patients were treated with protriptyline or nortriptyline (double-blind). They were assessed on the Zung Depression Scale and on the Hostility and Direction of Hostility Questionnaire (HDHQ). and dry mouth were constant symptoms. Impairment of salivary and lacrimal secretion were detectable for months. In the absence of clinical signs, electromyographic studies did not reveal neuromuscular involvement. Administration of antitoxin in the late course of such cases is not recommended, but guanidine can be used. Doubt is cast upon a new clinical entity called "acute autonomic neuropathy."

In this paper, further investigation by fluorescein angiography and electroretinography (ERG) was undertaken to substantiate the possible toxic maculopathy due to quinine usage. Again, positive Amsler chart tests were found in 3 of 14 (21%) of the drug addicts with
blurred vision
, a common complaint among these patients. The results of 5 fluorescein angiography examinations demonstrated no leakage of dye, especially in the macula. Twenty-four ERG examinations revealed no statistical differences between the test group patients and the control subjects. It was concluded that there is a definite need to carefully examine the eyes of any person who has used quinine for a long time since positive Amsler chart tests were found in 37% of the drug addicts.

in both eyes which markedly increased during the following days. We found flat anterior chambers, an increased ocular pressure up to 34 mm Hg, and a myopia up to-3,5 D. on both sides. Bloodsugar tests revealed varying results. After the diabetic metabolic state was normalized, the myopia and the raised intraocular pressure ceased to exist.

At their first visit to a hospital clinic 178 patients referred with a diagnosis of hypertension were given a self-administered questionnaire. They received a similar questionnaire 12 months later. Of the 178 patients 99 were not initially on treatment. Similarly 78 normotensive subjects were drawn randomly from the local population and sent a second questionnaire 10 months later. The symptoms at the first visit of the normotensive controls, the untreated hypertensive patients, and 477 patients on long-term treatment in the hypertension clinic were compared. Treated and untreated hypertensive patients complained more of nocturia and also of unsteadiness either on standing or in the morning. Treated hypertensives complained more of sleepiness, dry mouth, diarrhoea, and, in men, impotence and failure of ejaculation. Similarly, untreated hypertensives complained of excessive depression,

, and waking headache. Fifty-five of the normotensive subjects and 110 of the newly referred hypertensive patients responded to the second questionnaire. The proportions losing and gaining symptoms were calculated together with the proportions always complaining and never complaining of a symptom. Hypertensive patients tended to lose the complaints of unsteadiness and headache but to gain the symptoms of vivid dreams, a slow walking pace, and diarrhoea. The net improvement for a symptom was defined as the excess of patients who lost a symptom over those who gained the symptom, expressed as a percentage. Over the follow-up period the control subjects had a net improvement averaged over 14 symptoms of +2-4 per cent. A similar result was obtained for the hypertensive patients of +2-0 per cent, the symptoms lost being balanced by those gained. The changes in symptoms with time were related to the changes in blood pressure and it is suggested that only headache, 'unsteadiness, lightheadedness, or faintness' and nocturia can actually result from raised blood pressure and then only in a proportion of patients complaining of these symptoms.
