The authors report a case of nodular scleritis in Sweet's syndrome. The patient was a 40-year-old Malay woman with a history of Sweet's syndrome presented with a 6-day history of a red and painful right eye associated with blurred vision. Clinical examination revealed nodular scleritis of the right eye which was deemed secondary to Sweet's syndrome after investigations for other systemic diseases were negative. The patient demonstrated good response to systemic corticosteroid therapy with complete resolution of scleritis in the right eye. However, her inflammatory skin condition worsened each time the systemic corticosteroids were tapered so colchicine and dapsone were added to treat the dermatologic disease. Ocular manifestations are uncommon in Sweet's syndrome but it is important to diagnose and treat them. This will prevent ocular morbidity and ensure a good clinical outcome.

A 47-year-old Thai female with underlying Sweet's syndrome and disseminated non-tuberculous mycobacterial infection presented with a history of blurred vision on both eyes after being lost to follow-up for eight months. The visual acuity was hand motion on the right eye and counting finger on the left eye. There was moderate inflammation in the anterior chamber and vitreous cavity. Multiple foci of round chorioretinitis were found throughout the fundus on both eyes. The patient denied intravitreous tapping and antibiotic injection. Nevertheless, specimens from several sites were collected for culture and sensitivity test. The result of the culture and sensitivity test revealed rapidly growing mycobacteria on specimens taken from the right inguinal lymph node. The presented case may demonstrate the rare event of bilateral endogenous endophthalmitis with chorioretinitis arising from non-tuberculous mycobacterial infection. The pattern of chorioretinitis demonstrated by the presented report may provide useful clinical information for this rare condition.

Sweet's syndrome is rare acute febrile neutrophilic dermatosis whose onset is either idiopathic or associated with other underlying conditions, such as infections, autoimmune diseases, pregnancy, use of certain medications, or malignancy. We report the case of a young male with known history of ulcerative colitis and abrupt onset of high fever, malaise, blurred vision and eruption of painful erythematous nodules and papules, localized on the head, neck, trunk and upper limbs. Ophthalmological examination established the diagnosis of anterior uveitis. Inflammatory markers were positive. Histological examination of skin lesions revealed a dense neutrophilic infiltrate of the dermis. Clinical, laboratory and histological findings were suggestive for the diagnosis of Sweet's syndrome and uveitis on a background of ulcerative colitis. Systemic and ophthalmic administration of corticotherapy leads to a prompt resolution of symptoms and inflammatory syndrome. The particularity of this case is the occurrence of two simultaneous extraintestinal manifestations in a young male with inflammatory bowel disease and colonic involvement. Although a relatively rare condition, Sweet's syndrome should be considered as a differential diagnosis in patients with acute onset of high fever and skin rash, as it may have notable internal involvement and can be easily treated.
