We present two uncommon underlying causes of a sore throat which, if missed or delayed in diagnosis, can lead to disastrous consequences. Our first case is of
Lemierre's syndrome
diagnosed in a 21-year-old man presenting with a 5-day history of sore throat, fever, right-sided
pleuritic chest pain
and bilateral pulmonary nodules on CT imaging. Fusobacterium necrophorum cultured from peripheral blood and an occluded left internal jugular vein on ultrasound lead to an eventual diagnosis. Our second case presents a 29-year-old woman with a 5-day history of sore throat, fever and right-sided
pleuritic chest pain
. A left-sided quinsy was diagnosed and aspirated and the patient was discharged home. She represented shortly with worsening pleuritic pain and was found to have a right-sided pleural effusion with descending mediastinitis originating from the tonsillar abscess. Delayed diagnosis resulted in open thoracotomy, decortication and prolonged intravenous antibiotics.

Lemierre's disease is a rare but life-threatening condition characterized by an oropharyngeal infection complicating with thrombophlebitis of the internal jugular vein and disseminated abscesses. pleuritic chest pain
. She then developed progressively worsening acute hypoxic respiratory failure requiring intubation and mechanical ventilation. Interval chest X-ray showed worsening bilateral effusions. She also developed septic shock requiring pressors. Blood culture showed
Fusobacterium
, and antibiotics were changed accordingly following which there was a clinical improvement. The diagnosis of
Lemierre's syndrome
was then established based on her presenting age and bilateral pulmonary empyema in the setting of septicemia with

. The patient was sent home her first 2 visits with an incorrect diagnosis. Despite having a presentation consistent with
Lemierre's syndrome
, on her third visit, she underwent an extensive work-up investigating other etiologies prior to receiving a correct diagnosis and adequate treatment. This case demonstrates how a thorough history and physical exam can aid Radiologists in expediting the diagnostic process for a potentially fatal disease. Although rare, a history of pharyngitis and neck pain in a septic patient should help Radiologists to exclude more common etiologies which, through their investigations, prolong life-saving treatment.
