After returning from Thailand, a 23-year-old Japanese man was admitted because of fever, sore throat, neck pain, and
chest pain
. Contrasted-enhanced CT scanning of his neck revealed an absence of flow through the right internal jugular vein representing thrombosis, and moreover, an increase in wall thickness of the right internal jugular vein and enhancement of the surrounding tissue representing thrombophlebitis. Lung abscesses were also identified by a chest CT scan. Fusobacterium nucleatum was cultured in bronchoalveolar lavage fluid. He was diagnosed with
Lemierre syndrome
, and a good result was obtained by the administration of antibiotics. Physicians are encouraged to be aware of this syndrome when they manage patients complaining of neck pain and fever.

A young woman with no significant past medical history presented with worsening cough, swinging fevers, persistent
chest pain
and neck tenderness. On examination she was tachycardic, hypotensive, pyrexial and became increasingly confused. as a rare infectious disease. A 20-year-old man complained of high fever, right neck discomfort and
chest pain
. Chest X-ray revealed infiltrative shadows, suggesting bacterial pneumonia. Although cefcapene pivoxil hydrochloride hydrate (CFPN-PI) was given in a local clinic, his symptoms did not improve. Then he was referred to our hospital. Chest CT findings showed bilateral multiple nodular shadows with small cavities, suggesting septic embolization. Fusobacterium necrophorum was cultured from specimen of the blood, and an enhanced neck CT scan showed thrombosis in the right internal jugular vein. These findings led us to a diagnosis of
Lemierre syndrome
. Four weeks of antibiotics and anticoagulants ameliorated inflammatory findings in blood, but internal jugular vein thrombosis remained. Currently, there is no consensus opinion on the use of anticoagulation in patients with
Lemierre syndrome
complicated by septic internal jugular thrombosis and embolism. Early and effective antibiotics therapy may prevent the development of the syndrome and its associated complication, although it is unclear whether the outcome will be favorable.

We report the case of a previously healthy, immunocompetent 23-year-old male who presented to the Emergency Department with general malaise, difficulty in breathing, fever, and
chest pain
. He reported a two-week history of progressively worsening sore throat that he presumed to be a viral infection and thus initially neglected. However, when his condition deteriorated, he was admitted to hospital acutely unwell and in respiratory distress. He quickly developed septic shock requiring intensive care admission for inotropic support. Ultrasound and CT imaging revealed internal jugular vein thrombosis with associated septic emboli reaching the lungs to form bilateral cavitations and consequently pleural effusions. Blood cultures were positive for Fusobacterium necrophorum. Based on these findings, a diagnosis of
Lemierre's syndrome
was made. The patient was treated with appropriate antibiotics and anticoagulation and gradually recovered. He was discharged 20 days after admission with advice to complete a six-week course of antibiotics.

Lemierre's syndrome
is characterized by thrombophlebitis in the internal jugular vein after an infection in the area of the head or neck, mostly in the form of pharyngitis and/or tonsillitis. It may also result from less common conditions, such as odontogenic infections, but this condition has not often been reported in the dental literature. The syndrome's main etiological agent is Fusobacterium necrophorum, and it most commonly occurs in young adults. This study reports the clinical case of a 34-year-old female patient of poor socioeconomic status with a history of pericoronitis, who reported having a
chest pain
that radiated to the upper limbs, along with dyspnea. The computed tomography of her neck provided evidence of an acute thrombus in the right internal jugular vein. The treatment comprised antibiotic and anticoagulant therapy in addition to the extraction of her third molar and some residual roots. This report highlights the perils of systemic complications through dental infection, including the risk of death.
