After acute pharyngitis a 21 year old patient developed signs of severe
bacteremia
with a well demarcated infiltrate in the left lung. The typical course and a prompt response to antibiotic therapy with Imipenem (Tienam) led to the diagnosis of
Lemierre's syndrome
(post-anginal sepsis)--in spite of negative blood cultures. The patient recovered quickly, a chest radiogram after two weeks showing but pleural scarring and restitution ad integrum after four weeks.

Among 5 patients with
bacteremia
due to Fusobacterium necrophorum, 3 young adults had post-anginal sepsis (
Lemierre syndrome
), including one with the classical secondary metastatic complications of pulmonary abscesses, pleural empyema, arthritis and hepatobiliary disturbances. The primary focus was a cholangiogenic abscess in an 81-year-old woman, and fetid otitis following a radical operation for cholesteomatous chronic otitis media in a 29-year-old male. Septicemia due to Fusobacterium necrophorum, and in particular
Lemierre syndrome
, are presented in the light of the literature.

Deep neck-space infections may cause potentially life-threatening complications of head and neck infections. Lateral pharyngeal space infections in particular predispose to development of suppurative jugular vein thrombosis, which may be associated with anaerobic
bacteremia
and septic pulmonary emboli (
Lemierre's syndrome
). We describe a case of
Lemierre's syndrome
, a very rare entity in the antibiotic era, complicating mastoiditis. Surgical debridement and drainage in conjunction with antibiotic therapy resulted in prompt improvement in the patient's condition.

Lemierre's syndrome
is an uncommon clinical entity. It consists of oropharyngeal infection and anaerobic
bacteremia
, followed by jugular vein septic thrombophlebitis with embolization to lungs and other areas. Although it occurs less frequently than in the pre-antibiotic era, it is important that the typical presentation be recognized because of its lethal potential. A case of
Lemierre's syndrome
in Louisville, Kentucky, is described.

Lemierre syndrome
seldom follows an episode of pharyngotonsillitis. Characteristically, it is comprised of septic thrombosis of the internal jugular vein and
bacteremia
, leading to lung emboli and metastatic abscess formation. We describe
Lemierre syndrome
that complicates an acute mastoiditis, with considerations regarding its pathogenesis and management. Despite its sporadic occurrence, awareness of
Lemierre syndrome
is important, since early recognition reduces both the morbidity and mortality associated with it.

Previously healthy people without interfering conditions are rarely affected by anaerobic infections. We report a young patient with extended septic emboli in the lungs, after an episode of sore throat, due to anaerobic
bacteremia
with Fusobacterium necrophorum. The first description of oropharyngeal infection complicated by sepsis was given by Lemierre in 1936. Knowledge of
Lemierre's syndrome
should lead to early recognition and prompt action against this sporadic and possible fatal illness.

It has been suggested that a previous viral or bacterial pharyngitis may predispose to
Lemierre's syndrome
, an uncommon entity nowadays. A case of
Lemierre's syndrome
and co-infection with Mycoplasma pneumoniae is described, association published before in only 2 other cases. A 16-year-old girl with trombophlebitis of the left jugular vein, Fusobacterium necrophorum
bacteremia
and septic emboli in the lung requiring mechanical ventilation due to severe respiratory insufficiency. Despite early treatment with penicillin G and clindamycin, fever and part of pulmonary affectation persisted until that an antibiotic agent active against M. pneumoniae was instaurated, after its serological confirmation. The physicians should be easily recognized this disease because of its characteristic clinical findings, and co-infection with other organisms including M. pneumoniae, should be considered.

Considerable information has been accumulated in the field of anaerobic bacteria and anaerobic infections in the last ten years. Here we tried to briefly introduce several selected topics of clinical importance in this field: Proposal of the term "Nanaerobe", Changes of classification and nomenclature of anaerobes, Anaerobic
bacteremia
,
Lemierre's syndrome
as a revival anaerobic infection, Atopobium vaginae as Bacterial Vaginosis-associated bacteria, and new actions of the Clostridium perfringens toxins.

Necrobacillosis due to Fusobacterium necrophorum is an uncommon anaerobic infection. It has a wide range of presentations and commonly presents as
Lemierre's syndrome
. We present a case of necrobacillosis defined by F. necrophorum
bacteremia
with epidural and pararectal fluid collection without evidence of internal jugular vein thrombophlebitis.

Fusobacterium necrophorum can cause endemic pharyngitis and the
Lemierre syndrome
. Four previous case reports and one epidemiologic study have documented that some F. necrophorum pharyngitis patients develop
bacteremia
without developing the complete
Lemierre syndrome
. We report two more patients who have bacteremic F. necrophorum pharyngitis. We summarize the clinical presentation of these six patients. All received early diagnosis and excellent response to antibiotics. We speculate that prompt antibiotic treatment may have prevented the more serious
Lemierre syndrome
. Adolescents and young adults who present with significant pharyngotonsillitis and bacteremic symptoms should have blood cultures and receive antibiotic regimens that treat F. necrophorum. Recognition of such patients might prevent
Lemierre syndrome
.
