Choroidal osteomas are benign, juxtapapillary, choroidal tumors that mandate no treatment. They occur predominantly in young (median age, 20 years) females (90%). While ophthalmoscopy and fluorescein angiography reveal suggestive ophthalmic findings, echography, plain radiography, and especially computed tomography (CT) are important in establishing the correct diagnosis. These lesions may be clinically confused with various dystrophic lesions, atypical or amelanotic malignant choroidal
melanoma, metastatic
carcinoma, leukemic or lymphomatous infiltrates, choroidal
hemangioma
, choroidal scars of the macula, and organized subretinal hemorrhage. The exclusion of the differential consideration of malignant melanoma is most important since radiation and enucleation are therapeutic possibilities. Review of the literature shows that all nine reported cases with CT scans had clearly demonstrable calcific densities in the posterior pole of the affected eye. Four other cases are reported here with a summary of the typical clinical, radiologic, and pathologic findings. The role of CT in the evaluation of choroidal osteoma and the distinctiveness of the CT findings are stressed.

Of the more than 200 patients recently evaluated for venous disease, 8 were diagnosed with lower extremity masses. Three patients were referred for superficial phlebitis and four for deep venous obstructive disease. The eighth mass was found during work-up for varicose veins. Five masses were identified by palpation, and three were identified by duplex scan. All were confirmed by magnetic resonance imaging (MRI) or computed tomography (CT). Of the eight masses, three were malignant: a

, a histiocytoma, and a myxoid liposarcoma. Nonmalignant masses included a hematoma, an inflammatory lesion, a
hemangioma
, and an intramuscular lipoma. One patient presented with deep venous thrombosis secondary to an occluded popliteal artery aneurysm compressing the popliteal vein. Thus, patients presenting with ostensible venous disease may have other pathologic conditions responsible for symptomatology. Careful physical examination will reveal a mass in a majority of patients who have one. Duplex scanning will identify masses that should be confirmed by MRI or CT. Definitive diagnosis should be made by biopsy, due to the high possibility of malignancy.

Our preliminary experience with the bronchoscopic application of cryotherapy using rapid decompression of liquid nitrous oxide as cooling agent is reported. Seventeen applications through rigid bronchoscopy in twelve patients were performed. A single cryotherapy session was successful in the debulking of obstructive malignant lesions of the central airways in five patients (four non-small cell carcinoma, one renal cell cancer metastasis), and in the treatment of a capillary
haemangioma
(one patient). Two sessions were successful in the treatment of a
metastatic melanoma
(one patient) and benign granulation tissue (one patient). Cryotherapy was also successful in the treatment of early bronchial cancer (carcinoma in situ) in four patients, requiring repetitive sessions in two. There were no complications or side-effects. These preliminary findings confirm the safety and efficacy of bronchoscopic cryotherapy in a variety of airway lesions.

A young woman with a melanoma of the left forearm was found to have a right lung mass. This was initially interpreted as
metastatic melanoma
on the basis of clinical, radiographic, and light microscopic features, together with positive staining of tumor cells with antibody HMB-45. Electron microscopic examination performed for confirmation of the diagnosis revealed no evidence of melanocytic differentiation. Instead, there were features suggestive of the alternative diagnosis of sclerosing
hemangioma
(SH). This diagnosis was confirmed with additional immunocytochemical stains. To the authors' knowledge this is the first report of HMB-45 positivity in SH. This case illustrates a potentially disastrous diagnostic pitfall in interpreting lung tumors in patients with melanoma, and the vital role of electron microscopy in resolving conflicting and/or misleading immunocytochemical results.
