An asymptomatic tumor developed on the upper lip of a 63-year-old man. Histologically, the tumor contained glandular and cystic structures forming many branching lumina, and many scattered single cells in an abundant mucoid to chondroid stroma. The tumor was diagnosed as mixed tumor of the skin. Histochemically, the cells composing the tubular structures contained neutral mucopolysaccharides and the stroma, acid mucopolysaccharides. Immunohistochemically, the cells of the glandular and cystic structures showed epithelial and sweat gland differentiation (

-, CEA-, BRST-1- and BRST-2-positive), while the cells scattered in the stroma showed a tendency toward myoepithelial differentiation (S-100 protein- and vimentin-positive).

The purpose of this study was to examine and understand the nature of capsules consisted of fifty cases of unerupted wisdom teeth using radiological, pathological and immunohistochemical methods. Radiologically, the pericoronal space was measured with slide caliper and divided into three groups (-0.9 mm, 1.0-1.9 mm, 2.0mm). Pathologically, all specimens were examined by the routine paraffin method and were stained with hematoxylin.eosin and PAS-alcian blue pH 2.5 stainings. Immunohistochemically, PAP (peroxidase-antiperoxidase) method using various keratin antibodies, such as, anti-keratin K 528 (40-68 KD), anti-cytokeratin PKK 1 (40, 45, 52.5 KD), anti-cytokeratin PKK 2 (40, 46, 48, 54 KD), anti-keratin A 575 (56, 64 KD) and anti-cytokeratin high molecular weight (68 KD), and other various antibodies of anti-

, anti-IgA and anti-SC was used in order to study the nature of the epithelium of capsule of impacted teeth. The following results were obtained; 1. Radiologically, the width of the capsules was 1.6 mm in the average. Histopathologically, the inner surface of the capsule was covered with two types of epithelium which were composed of non-typical epithelium with cuboidal (basal layer) and columnar (superficial layer) cells, and typical stratified squamous epithelium. The epithelium lining was observed in 74% of all cases. Among the cases, cyst like structure was more often seen in the cases with typical squamous epithelial lining and with pericoronal space more than 1.0 mm width. 2. In the connective tissue's findings of capsules of impacted teeth, fibrosis was found in almost all the cases. Inflammatory and myxomatous changes were recognized in ca. half cases. Epithelial rest and calcification with dystrophy and epithelial product were observed in about one third of the cases. The epithelial proliferation in the capsule was also rarely seen in the specimens. 3. Immunohistochemically, keratins of 56 KD and 64 KD were identified in the superficial layer of non-typical epithelium and basal, intermediate and superficial layers of the typical squamous epithelium of the capsules of imparted teeth. Keratin of 68 KD was only observed in superficial layer of squamous epithelium. 4. Epithelial membrane antigen was positive in the intermediate and superficial layers of typical squamous cell epithelium. However, it was not seen in the non-typical epithelium of capsule of impacted teeth. 5. Immunoglobulin A and secretory component were negative in the epithelia of capsules.

Thirty-nine patients with choriocarcinoma and one with post molar trophoblastic tumour are presented. It was often found that there had been a long interval between the preceding pregnancy and the time of diagnosis or presentation. As a result, there was a high incidence of non-gynaecological presenting symptoms and 95 pc of our patients belonged to the high risk or poor prognosis group. The highest rate of cerebral metastases from choriocarcinoma so far reported in the world literature is presented here. At the beginning of 1987, a modified

regimen as introduced and has produced a great improvement in survival. The mortality from choriocarcinoma in Harare from 1985 to 1986 was 81 pc. In 1987 to 1988, this has fallen to 31 pc. The follow-up for the patients on the modified

regimen varies from four to twenty-four months.

This study reports six non-Hodgkin's lymphoma cases that we called histiocyte-rich B-cell lymphoma (BCL) because of the prominent reactive histiocytic infiltrate obscuring the malignant B-cell population. The involved lymph nodes are characterized by a mixed nodular and diffuse infiltrate and occasionally feature prominent sinuses. The infiltrate is composed of reactive lymphocytes and numerous histiocytes obscuring a tumor population composed of variably sized scattered cells with irregular or multilobar vesicular nuclei. Immunostaining of paraffin sections for the B-cell marker recognized by L26 helps in the identification of these neoplastic cells. The clonal nature and further evidence of the B-cell lineage of this condition is shown by immunoglobulin gene rearrangements detected in three cases. The six cases of histiocyte-rich BCL are remarkably similar clinically: all presented with stage IVB disease with splenomegaly and follow an aggressive clinical course. Except for these features, our series show striking similarities to paragranuloma lymphocyte-predominant Hodgkin's disease, including male preponderance (all patients are male), age distribution (mean age, 41 years), propensity to progress to a diffuse, large B-cell lymphoma (two cases), as well as morphology of the neoplastic B-cell population and expression of Hodgkin's cell markers (Leu-M1 positivity after neuraminidase digestion in three cases, Leu-M1 positivity without neuraminidase digestion in one case, and additional epithelial membrane antigen [
EMA
] positivity in two cases). Both morphologically and clinically, the present series can be differentiated from other types of infiltrate-rich BCL, such as T-cell-rich BCL. Although additional cases will have to be recognized, histiocyte-rich B-cell lymphoma most likely represents a distinct clinicopathological entity. We speculate that it develops from a subset of B cells that also gives rise to the lymphocytic-histiocytic (L/H) cell, the Hodgkin's cell variant of lymphocyte-predominant Hodgkin's disease, paragranuloma subtype.

The aim of the present investigation was to characterize the various cell types of classical and chondroid chordomas. Eight cases of classical chordoma, 1 case of sacrococcygeal chordoma with chondroid areas and 2 cases of spheno-occipital chondroid chordoma were studied. Ultrastructurally and immunohistochemically (immunoreactivity for cytokeratins, epithelial membrane antigen [
EMA
], tissue polypeptide antigen [TPA] and human milk fat globule protein [HMFG]) the 3 cell types (physaliferous, epithelial-like, and spindle-shaped) recognized light-microscopically presented features of epithelial differentiation and rather formed a continuous spectrum than being distinct cell types. The chondroid areas of the chondroid chordomas had similar ultrastructural and immunohistochemical properties except for the lack of immunoreactivity for
EMA
and HMFG. The results of the critical electrolyte concentration technique according to Scott and Dorling indicated that there was no difference in the sulfated glycosaminoglycan content between classical and chondroid chordomas: all the tumors contained chondroitin sulfate. The presence of chondroitin sulfate, immunoreactivity for vimentin and S-100 protein and areas of cartilaginous differentiation in three cases indicate a relationship both to chondromatous tumors and to normal notochord, from which chordoma is believed to originate.

This is a study of selected information on incidence and prevalence of Parkinson's disease (PD), mortality from Parkinsonism, and levodopa use (LDU). The concept of effect modification (EM) by age (
EMA
), a variation across age of incidence or odds ratios, was here extended to comparisons of prevalences, mortalities and drug use, and mathematically defined following a reported model. Age-specific EM from comparisons of empirical data was measured and combined with figures for calendar time and the annual infant mortality rates (IMRs) in the population of each observation, using linear regression analysis. Statistically significant associations of EM at ages 70-79 and 80+ were found: with IMR from prevalence comparisons; and with IMR and calendar time in comparisons of proportionate mortalities in longitudinal observations. A similar age-specific pattern of associations was observed with time from incidence in Rochester. The observations for LDU and PD incidence in Iceland and Turku, Finland, fitted well the age-specific regression lines of the IMRs with the age-specific EM components from prevalence data. We concluded that there is a universal age-specific pattern of variation of epidemiological characteristics of PD related to calendar time, and to local factors represented by the IMR. Moreover, this change can be perceived from different parameters under certain conditions. Comparisons of survey, death-records, and LDU data when used for generation of causal hypotheses for PD require that

Glucoamylase II (

) immobilized to Eupergit C and CIZ as a porous and nonporous matrix shows enzymatic characteristics indistinguishable from those of the free enzyme, except for reduced specific activity. Since this decrease is equally observed for both matrices, it has to be ascribed to nonproductive fixation of the enzyme or steric hindrance rather that perturbations caused by "inner diffusion" effects. Authenticity refers to the optimum pH for catalytic activity, Michaelis constants for starch and maltoheptaose, as well as identical stability toward temperature, pH, and guanidinium chloride (GdmCl). On the basis of these data, the two-state mechanism observed for the equilibrium transitions of the free enzyme may be assumed to hold also for the immobilized enzyme. Renaturation after preceding denaturation in 6.4 and 7 M GdmCl leads to widely differing yields depending on the conditions. Shifting the denaturant concentration stepwise back to nondenaturing GdmCl concentrations leads to a broad range of "hysteresis" accompanied by aggregation. Rapid dilution of the free and immobilized enzymes at pH greater than 6 and sufficiently low protein concentration leads to reactivation yields of 80 and 45%, respectively. For the free enzyme, reconstitution at lower pH is determined by the kinetic competition of folding and aggregation. In the case of the immobilized enzyme, "entangling" of the matrix with the unfolded polypeptide chain competes with renaturation.

Cystic mesothelioma of perinephric retroperitoneum origin are very uncommon tumors and considered potentially malignant. We report one such case and discuss the clinical and pathological findings. A 70-year-old man was seen with complaint of discomfort in the right flank and hospitalized in May 1989. Computerized tomography revealed multiple cystic masses in the right retroperitoneal space which appeared to be infiltrating the kidney and the iliopsoas muscles. Magnetic resonance imaging showed deformation of the right kidney with many impressions in the parenchyma. The cystic lesions and the right kidney were extirpated on June 2, 1989. Grossly the specimen was 16 x 10 x 8 cm in size and 630 g in weight. The multiple cysts surrounding the kidney were each approximately 10 mm in diameter and had thin outer walls. The cyst fluid was clear and serous. The kidney had not been infiltrated but had only external impressions caused by the cystic lesions. Microscopically, the cysts were lined by a single layer of cuboidal cells accompanied by some hobnail-shaped cells, and no evidence of malignancy was found. The epithelium was focally positive for periodate acid Schiff and slightly positive for Alcian blue. It was strongly positive for cytokeratin and vimentin, and slightly positive for
EMA
but negative for lectins. The diagnosis was diffused benign multicystic mesothelioma. However, CT taken four months after the operation revealed local recurrence and radiotherapy (40 Gy) was instituted. Since the cystic mass tended to grow in size thereafter, the lesion appeared to be malignant clinically. We consider this is the first case of cystic mesothelioma of perinephric retroperitoneum origin reported in Japan.

To date, one hundred and ten cases of malignant neoplasms arising from untreated bladder exstrophy have been reported. We describe another case of uncorrected bladder exstrophy with invasive adenocarcinoma and in situ squamous carcinoma discovered in a 51 year old woman. Postoperative radiotherapy was performed after cystectomy with nephroureterectomy and hysterectomy. The neoplastic samples were investigated by mucin histochemistry and immunocytochemistry. The epithelium lining the tumor villi and the surrounding mucosae was colonic with mucin histochemistry and reacted to anti KL1,
EMA
and ACE antibodies. These features were those of colonic and vesical adenocarcinomas. Although early vesical reconstruction is the best oncologic prevention, patients with bladder exstrophy run the risk of sigmoid adenocarcinoma after cystectomy and urinary diversion including ureterosigmoidostomy.
