Eight cases of thymic carcinoma (mean age 62 years, 5 males and 3 females) were treated in our hospital from Apr. 1988 to Mar. 1993, which were 21% of thymic epithelial tumors. The histologic subtypes of thymic carcinoma were 3 squamous, 4 undifferentiated, and 1 adenocarcinoma, respectively. Computed tomography showed extrathymic metastases to the mediastinal lymph nodes in 6 cases. All cases underwent thoracotomy, and postoperative radiotherapy or chemotherapy were performed. A case of squamous cell carcinoma in stage II who underwent total resection followed by irradiation is alive and disease free 5 years after surgery. However, the remaining 7 cases died of the disease within 29 months. 8 cases of thymic carcinoma and 7 of thymoma were studied immunohistologically on the formalin-fixed and paraffin-embedded tissues. Higher positivity of EMA in thymic carcinoma was noted comparing to ordinary thymoma, suggesting that EMA is a useful parameter of potential malignancy of the thymic epithelial neoplasms.

The autopsy of a 76-year-old Japanese female patient, which revealed thymic carcinoma with various tumor markers such as NSE, CYFRA, and CA-125, is presented. The patient died from hepatic failure because the liver was overtaken by the tumors. At autopsy, the thymic carcinoma was found to have metastased only in the liver. From microscopical analyses and electron microscopical findings, we diagnosed poorly differenciated squamous cell carcinoma of thymic origin. In the histochemical analyses, the tumor cells were positively stained in CA 125, CA 19-9, EMA, NSE, AE 1, AE 3, CEA, S-100, glimerius and Bcl-2. These date suggest that the tumor cells produced various tumor markers. In 222 autopsy cases of thymic malignant tumor observed in Japan over a period of 4 years, the dominant pathohistological image was squamous cell carcinoma. It is interesting that the greatest number of combined malignant tumors with thymic malignancies were thyroid papillary carcinomas.

Intrathyroid thymic carcinoma (ITCA) is a rare neoplasm of the thyroid gland, that is difficult to diagnose because of its complex pathologic and immunohistochemical characteristics. Here, we report two cases that may provide a better understanding of this disease. Case 1 is a 30-year-old man, and Case 2 is a 42-year-old man. The two cases showed an irregular solid thyroid mass on sonography. Histological examination revealed the solid growth of epithelial cells separated by bands of dense, hyalinized, and fibrous stroma. Tumor cells were positive for the expression of CD5, HMWK, CK19, CK5/6, CK7, EMA, CD117, p63, Bcl-2, and NANOG. It is noteworthy that the neuroendocrine markers Syn, NSE, and CgA were also observed. In Case 1, chemotherapy combined with radiotherapy, chest CT showed lung metastasis 4 months after resection. The patient was alive as of postoperative follow up lasting 46 months. Radical operation combined with radiotherapy was performed in case 2, and the patient was alive as of follow-up lasting 42 months.
