Based on an experience over two years with 316 cases the principles of general anaesthesia with controlled hypotension in
ENT
surgery are described and discussed. Indications, contraindications and methods of application are outlined. The method reduces bleeding considerabley and proved to be useful for rhinoplasties, major sinus surgery as well as parotid gland and necksurgery.

This paper is a comparative study of the early and late functional results of tympanoplastic operations, as modified by Khan. Following such surgery, we examined 494 patients who had been operated on between 1970--1972 in the
ENT
Clinic of the Free University of Berlin (Klinikum Westend). Preliminary and final hearing results were recorded with the aid of sound audiograms, which were critically assessed by two different types of evaluations. These included the results of pure-tone changes and speech reception thresholds as achieved by the tympanoplastic operations. On the basis of socially useful hearing improvements, the functional results of cases operated in our clinic are above average. The hearing gain shown from post-operative controls to the final studies is especially satisfactory, since this also includes an improvement in bone conduction.

The diagnostic and therapeutic approach to tumors of the salivary glands is discussed on the basis of 327 cases of parotid gland tumors seen at the
ENT
Department of the University of Zurich between 1959 and 1976. In view of the slow growth rate of salivary gland tumors, history and clinical findings cannot be used to identify malignant neoplasms. The histological diagnosis is therefore the prerequisite for correct therapy. Diagnostic errors may result when the total tumor mass is not available for histological investigation. For this reason we advise "surgical biopsy" (total extirpation of the tumor) when a malignant tumor cannot be ruled out by aspiration biopsy or examination of frozen sections. Radical surgical removal plays the major role in the therapy of salivary gland tumors. Highly malignant neoplasms demand a combined oncological approach usually involving surgery and radiotherapy. Familiarity with the temporal bone is necessary for the complete removal of highly malignant tumors of the parotid gland, because of their frequent infiltration of the intratemporal portion of the facial nerve. The application of the modern principles of reconstructive surgery and of microsurgical techniques has widened the scope of radical surgery of salivary gland tumors and considerably reduced the subsequent morbidity.

Many surgical departments indlcuding the

field have installed video recording systems or simple b/w or color TV camera and monitors. Using a TV color system has been quite successful for
ENT
purposes in the operating room. The camera is used in microsurgical work (tympanoplasty etc.) and is connected to the microscope so that the picture can be controlled, corrected, analysed and explained by the teacher. Recording systems are presented and the "utopic" question of compatibility is discussed.

Growth and development of metastases depends on 1. Tumor cells themselves, 2. Manipulations on the primary tumor, 3. Lymphatic vessels in the surrounding area of the primary tumor, 4. Blood composition. 5. Extent of tissue resistence through which tumor cells pass. 6. Certain circulatory conditions of the blood. Tumor cells are distinguished from normal cells often by reduced (Verbrauchskoagulopathie, tendency to bleed). An increase in bloodclotting supports the development of haematogenic metastases and the tumor growth. Malignant tumors of the kidney and the intestines may develop micrometastases of the lungs which, for years, as dormant cells, remain undiscovered. Then after 12--14 years metastases (further satelites) are seen e.g. in the
ENT
-field. In the spreading of tumor cells the flow parameters of lymph and blood play a very important role.

47 patients with orbital complications resulting from acute sinusitis have been treated at the Freiburg
ENT
Department during the last seven years. Acute ethmoiditis predominated in children, while pansinusitis was more common in adults. Previous sinus surgery did not prevent orbital complications arising from the same sinus. Orbital involvement was classified into periostitis, subperiosteal abscess and orbital cellulitis. According to clinical findings, 44.7% of the patients were managed conservatively. Prognosis was best in periostitis, whereas consersative treatment was unsafe in orbital cellulits. Children usually respond well to appropriate antibiotic coverage but the occurrence of common upper respiratory or anaerobic pathogens should be kept in mind.

64 bronhcoscopies performed in 1977 under general anesthesia are reported from the
ENT
Department of the Protestant Hospital, Oldenburg, West Germany. Indications for bronchoscopy were primarily for the diagnosis of bronchial tumors, but also included treatment of foreign body aspirations as well as the definition of tracheal stenoses and injuries from intubation anesthesias. The hypnotic agent "Etomidat" was used as anesthesia, during which the parameters of heart and peripheral circulation were carefully monitored. These remained stable; patients needed only a short time to waken, and were quickly able to again cooperate with the doctor.
