Hydatid cysts in the neck are quite rare, even in areas where the disease is endemic. The role of radiological investigation is important and, in these cases, the involvement of other organs should be investigated. The cyst should be excised as a whole, without being ruptured, to prevent any treatment complications. This paper presents a case of an isolated hydatid cyst localised in the supraclavicular region without any pulmonary or hepatic involvement.

Hydatid disease located in the head and neck is uncommon, and hydatid cysts rarely present as a cervical mass. We report an unusual case of primary hydatid cyst arising in the soft tissues of the neck. The clinical presentation was non-specific. The diagnosis was suspected by the radiological findings, and confirmed by histopathology. Characteristics of this rare disease, its diagnostic difficulties, and treatment are discussed.

This case of 52-year male with swelling in the left submandibular area for 1 year came to ENT OPD of Patna medical college, Patna. Clinical examination showed swelling to be multiple nontender cysts with enlarged submandibular salivary gland. CT-scan revealed large well-defined encapsulated multicystic lesions in the left submandibular gland. FNAC was suggestive of aspirate to be ectopic to that area. Excision biopsy was done. HPE showed features suggestive of parasitic infestation along with chronic sialadenitis. Gross picture and investigations were highly suggestive of parasitic infestation to be primary hydatid cyst of the gland. Scanning for other areas like liver and lungs done, no lesion was found.

We report an unusual case of primary hydatid cyst of the mandibular angle without glands involvement, in the left supraclavicular region of the neck with no involvement of any other regions of the body. In July 2012, a 25-yr old woman, from Golestan Province, Northeast Iran was admitted to our ENT Clinic, with one-year history of a progressively increasing swelling, pain and gradually growing mass located in the left side of neck region. The patient was diagnosed by Fine Needle Aspiration Cytology (FANC) and histopathology examination. Hydatid cyst should be considered in differential diagnosis of soft tissue mass such as branchial cleft cyst (BCC) and or dermoid cyst in the cervical region especially in endemic areas. Moreover, FANC could be recommended as a valuable, rapid, simple, and safe procedure to diagnose hydatid cyst especially in unusual locations.
