The healthy dentition of 432 patients treated by radiotherapy for ENT and salivary gland lesions was preserved and subjected to a programme of strict hygiene and daily local fluoride application using a gel containing 1% of sodium fluoride. Effectiveness in terms of the prevention of caries was satisfactory. Caries developed in only 7% of the patients. None of the patients in whom there was a clear indication and who followed treatment carefully to an extent greater than 70% suffered decay. Thorough initial assessment, and very careful selection of cases should make it possible in the future to avoid the majority of sources of failure observed. The prevalence of osteo-necrosis secondary to dental preservation was 1.2%, identical to the prevalence of trophic complications seen in patients who underwent total extraction before radiotherapy. The technique of fluoride application is simple, with no risk of toxicity, but must be continued indefinitely. Preservation of the dentition must be undertaken only with the close cooperation of the responsible medical and odontological teams, the latter being responsible for the initial assessment, care and surveillance. Preservation of a healthy dentition together with the possibility of early fitting of a prosthesis if necessary contribute to the rapid functional rehabilitation of patients irradiated for malignant disease in the ENT area.

The patients referred from our hospital at the Department of Oral Surgery, University of Tokyo, during the two years from January 1990 to December 1991 were analyzed statistically. The results were as follows: 1. Our subjects were 1,152 patients (24.2% of first-time patients), 540 males and 612 females. 2. Concerning age distribution, the majority was in their forties, fifties and sixties. 3. More patients were from internal medicine, ENT clinic, plastic surgery and cardiothoracic surgery. 4. On the number of patients categorized by disease at other departments, the largest number was cardiovascular disease. 5. On the number of patients categorized by disease at our department, the majority were dental diseases (dental caries, marginal periodontitis and missing teeth).

A 13 year review of patients diagnosed to have Ludwig's angina admitted to the Christian Medical College and Hospital, Vellore, India, between March 1982 and April 1995 is presented. The patients were either admitted to the ENT or paediatric surgical units. There were 41 patients, 24% being children and 76% adults. The clinical profile and outcome of these two groups were compared. In the paediatric group, none had dental caries while in the adult group, 52% had associated dental caries. In children, 70% were controlled with conservative medical management unlike the adults of whom 81% needed incision and drainage. Tracheostomy was necessary in 10% of the children and in 52% of the adults. The mortality was 10% in both groups.

Pott puffy tumor, a rare complication of sinusitis in adults, appears as a localized swelling over the frontal region or forehead. This swelling is due to a subperiosteal abscess resulting from osteomyelitis of the underlying frontal bone. This condition may be associated with an epidural purulent collection, subdural empyema, and intracerebral abscess. This entity is rare--even forgotten--since the advent of modern powerful antibiotics. A 27-year-old male was referred to ENT OPD with forehead swelling, fever and headache. The patient had mooth doughy swelling of the foreheadwith surrounding cellulitis and swelling spreading to the orbits as periorbital odema. On examination, patient had very poor oral hygiene with severe dental caries. CT with contrast conformed pansinusitis with erosion of frontal bone and abscess. An early recognition of this forgotten complication of sinusitis will help avoid severe neurological sequelae and increased morbidity and mortality.
