The hemoglobin scavenger receptor (HbSR/CD163) is an interleukin-6- and glucocorticoid-regulated macrophage/monocyte receptor for uptake of haptoglobin-hemoglobin complexes. Moreover, there are strong indications that HbSR serves an anti-inflammatory function. Immunoprecipitation and immunoblotting enabled identification of a soluble plasma form of HbSR (sHbSR) having an electrophoretic mobility equal to that of recombinant HbSR consisting of the extracellular domain (scavenger receptor cysteine-rich 1-9). A sandwich enzyme-linked immunosorbent assay was established and used to measure the sHbSR level in 130 healthy subjects (median, 1.87 mg/L; range, 0.73-4.69 mg/L). To evaluate the sHbSR levels in conditions with increased leukocyte stimulation and proliferation, 140 patients admitted to a hematological department were screened. Several patients, with a broad spectrum of diagnoses, had a level of sHbSR above the range of healthy persons. Patients with myelomonocytic leukemias and pneumonia/sepsis exhibited the highest levels (up to 67.3 mg/L). In conclusion, sHbSR is an abundant plasma protein potentially valuable in monitoring patients with infections and myelomonocytic leukemia.

By immunoprecipitation we have identified a soluble plasma form of CD163 (sCD163), the IL-6 inducible macrophage-receptor for clearing haptoglobin-haemoglobin complexes. A sandwich ELISA for measuring sCD163 was established and used to determine the sCD163 levels in normal subjects and patients with inflammatory and myeloproliferative diseases. In normal subjects, the concentration of sCD163 was high (median 1.9 mg/l) with low intra-individual variation. Highly increased levels were seen in patients with sepsis, myeloid leukaemia and in patients with Gaucher disease characterized by accumulation of tissue macrophages. Although the physiological role of sCD163 remains unknown, our present data suggest that sCD163 might prove to be a valuable marker molecule in infectious and myeloproliferative diseases.

Fibronectin (FN) is a widely distributed glycoprotein which is present in different bodily fluids, on the surface of cells and in the extracellular matrix (ECM). It plays roles in various processes, including cell adhesion, migration, growth, proliferation, and tissue repair. Fibronectin exists in 2 forms: a soluble, inactive molecule, called plasma FN (pFN), which is synthesized by hepatocytes in the liver, and an insoluble cellular form (cFN), which is produced locally by different types of cells and is a key component of the ECM. Fibrinogen fibrils ensure structural support for cell adhesion and promote cell migration, proliferation and apoptosis. Additionally, FN controls the availability of growth factors. The plasma form of FN is a crucial component of the fibrin clot in the early wound-healing response, while the cellular form of FN supports efficient platelet adhesion, activation, aggregation, and procoagulant activity. Alternative splicing of the FN gene results in the generation of protein variants which contain the additional isoforms - extra domain A of FN (EDA) and extra domain A of FN (EDB); these are associated with, e.g., tissue remodeling, fibroblast differentiation, inflammation, and tumor progression. Fibronectin also serves as a target for a large number of bacterial proteins, and as part of a 3-component bridge (FN, integrin and FN-binding proteins - FnBPs) it contributes to bacterial colonization of endothelial and epithelial cells. Fibronectin has been identified in sepsis in humans as a negative acute-phase protein, and a low level of FN seems to be a marker of a poor prognosis for a patient. Here, the role of FN in inflammatory processes and sepsis is presented.
