A review of the literature reveals that of the eight reported cases of patients with acquired immunodeficiency syndrome acquiring
Norwegian scabies
, three of these have been complicated by
sepsis
. We describe such a patient who contracted
sepsis
from Pseudomonas aeruginosa. We propose that the fissures often seen in severe cases of
Norwegian scabies
may serve as a port of entry for bacteria, thus placing these patients at a high risk for
sepsis
. We also believe that empiric antibiotic treatment is justified in these patients and that the choice of agent should be based on the institution's bacterial flora profile.

Crusted scabies
(
Norwegian scabies
) is uncommon in clinical practice and may present as papulosquamous dermatosis. A case of crusted scabies in systemic lupus erythematosus (SLE) is described. The skin lesions simulated those of subacute cutaneous lupus erythematosus of the papulosquamous type, and became the source of epidemic scabies in the hospital. The colonization with Staphylococcus aureus within crusted lesions may have contributed to the patient's
sepsis
. This case illustrates the pitfalls in recognition of crusted scabies and the importance of rapid diagnosis.

Crusted or
Norwegian scabies
is a parasitic infectious disease caused by Sarcoptes scabiei var. hominis that mainly affects immunocompromised individuals and those with neurological patients. We report a case of crusted scabies in a 4-month-old infant who had been treated erroneously for atopic dermatitis with high doses of corticosteroids. This initial misdiagnosis associated with the abusive use of corticosteroid facilitated the evolution of scabies to crusted scabies and its main complications of secondary infection and
sepsis
.

Crusted scabies
is a rare and extremely contagious infestation by
Sarcoptes scabiei
. Kaposi's varicelliform eruption (KVE) refers to herpes simplex virus infection superimposed on pre-existing dermatosis such as atopic dermatitis, Darier's disease, and pemphigus. We report a case of KVE superimposed on crusted scabies in a middle-aged woman. Her condition was complicated with
sepsis
. She was treated with IV meropenem, vancomycin, and acyclovir and was released 2 weeks later in good condition. To our knowledge, only rare cases of crusted scabies complicated by KVE have been reported.

Crusted scabies
is a highly contagious variant of classic scabies. Affected individuals are often elderly or immunocompromised and disease is associated with significant morbidity and mortality. Herein, we report an elderly woman residing in an assisted living facility who presented with diffuse sand-on-skin scale on her trunk, proximal extremities, scalp, hands, and feet. She was diagnosed with crusted scabies, isolated from other patients, and subsequently treated with permethrin 5% lotion and ivermectin. She died two weeks after initial presentation owing to
sepsis
.
