While taking orally administered anticoagulants, three patients had hemorrhages into their retropharyngeal and submandibular spaces, suffering eventual acute airway obstruction. One of the patients died. Despite the life-threatening nature of this complication of anticoagulant therapy, the diagnosis was obscure and initially veiled in complaints of sore throat or hoarseness, suggesting infection. Thorough investigation of such complaints is necessary in patients receiving anticoagulation therapy. If a hematoma is discovered, the patient should be admitted to the hospital for close observation and prompt reversal of anticoagulation with plasma. Intubation or tracheostomy also may be required.

The prevalence of pharyngeal carriage of group B streptococci was evaluated in patients with and without the complaint of a sore throat by three methods (blood agar plates, Columbia CNA agar plates, and a selective enrichment broth containing gentamicin and nalidixic acid). The overall carriage rate of group B streptococci was 12%, and there was no significant difference between the two groups of patients. The selective broth medium was more sensitive than the two solid agar plate methods in detecting carriage, and 37% of all group B streptococci were recovered solely from the broth. Use of the broth alone would have permitted a 94% detection of the group B streptococcal carriers.

Susceptibility of 64 beta-hemolytic streptococcal strains isolated from the patients with sore throat was studied by the method of serial dilutions in fluid nutrient medium (Konikov broth). Heterogenecity with respect to the sensitivity was investigated in 34 strains among separate populations of the microbes (10 to 15 in every strain). The MIC of benzylpenicillin, oxacillin and erythromycin ranged within 0.007--0.24 U/ml, 0.02--0.36 gamma/ml and 0.005--0.1 gamma/ml respectively. The MIC of benzylpenicillin with respect to separate populations most sensitive to it was 0.007--0.015 U/ml, while that with respect to the lease sensitive populations ranged from 0.015 to 0.24 U/ml. The respective values for oxacillin were 0.02--0.12 and 0.18--0.36 gamma/ml and those for erythromycin were 0.005--0.025 and 0.05--0.1 gamma/ml. Therefore, the beta-hemolytic streptococci isolated from the patients with sore throat were characterized by a rather high sensitivity to the antibiotics which was important precondition for their efficiency in treatment of the patients with the above disease.

RA27/3 rubella vaccine (Almevax) was used for the first time in Australia. The seroconversion rate was similar to that seen with Cendehill vaccine (Cedevax), but there were fewer subjects with low antibody titres after Almevax vaccine. Although Almevax vaccine was associated with a significantly higher incidence of sore throat, rash and joint involvement in seronegative adults, the incidence of side effects in a double-blind trial in schoolgirls was similar for the two vaccines, and the morbidity was low in both groups. Almevax vaccination successfully boosted low antibody titres in 11 of 14 women who had previously responded poorly to Cendevax vaccination. It would be difficult to differentiate the better vaccine for the schoolgirl programme. In some clinical situations, however, one vaccine has advantages over the other.

The various complaints after gastroscopy and the acceptability of the procedures are verified by a questionnaire in 300 consecutively examined patients with or without gastric lesions. The extent of psychic lability, neurosis and extraversion was determined by the Maudsley Personality Inventory test of Eysenck. The time necessary for the passage of the instrument (swallowing time) and the time taken by gastroscopy were registered on each patient. More than half of the patients complained of sore throat lasting more than 1 day. Less than a quarter had abdominal dyspepsia. The intensity of the sore throat was correlated with the swallowing time but not with the extent of neurosis or gastroscopy time. 98% of the patients consented to a control examination. The necessity of a gastroscope with less diameter and a non mucosal damaging top is stressed.

Seven patients, aged 12 to 19 years, had atypical measles. Prodromal symptoms of fever, malaise, myalgia, headache, nausea, and vomiting were commonly followed by coryza, sore throat, conjunctivitis, photophobia, nonproductive cough, and pleuritic pain. The characteristic rash was erythematous, maculopapular, and progressed frequently to vesicular, petechial, or purpuric lesions. It initially involved palms and soles with subsequent spread to proximal extremities and the trunk, sparing the face. Six of six chest roentgenograms showed infiltrates. Findings not previously described in atypical measles included liver enzyme elevations, thrombocytopenia, disseminated intravascular coagulation, possible transmission among three siblings, and suspected cardiac involvement. Measles complement fixation titers compatible with recent infection were seen in all patients. All patients had previously received killed measles vaccine. A substantial number of persons who are older adolescents or young adults may be at risk of developing atypical measles.

Four adults with an illness similar to the systemic variant of juvenile rheumatoid arthritis seen in children (Still's disease) are described. All four had fever, an erythematous maculopapular rash, splenomegaly and arthritis. The arthritis was asymmetric and involved only a few joints simultaneously. Erosive arthritis developed in one patient. Three patients had a sore throat, two had pleurisy and pericarditis, and one had transient abnormalities of liver function. The laboratory features included anemia, leukocytosis and high leukocyte counts in the synovial fluid. High titres of rubella hemagglutination-inhibiting antibody were detected in two patients, one of whom was found to have rubella virus in the urine. Only one patient responded well to therapy with acetylsalicylic acid; the other three were given prednisone therapy, and two continue to require it.

The first symptoms of cancer of the oesophagus in 82 Xhosa patients were found to be dysphagia (43%), interscapular back pain (23%), sore throat (21%) and epigastric pain (11%). More than half of the patients in whom a sore throat was the first symptom of the disease described the simultaneous occurrence of oropharyngeal lesions (amaqhakuva). No correlation could be found between the first symptoms and the characteristics of the oesophageal carcinoma at the time of presentation.

A jet airliner with 54 persons aboard was delayed on the ground for three hours because of engine failure during a takeoff attempt. Most passengers stayed on the airplane during the delay. Within 72 hours, 72 per cent of the passengers became ill with symptoms of cough, fever, fatigue, headache, sore throat and myalgia. One passenger, the apparent index case, was ill on the airplane, and the clinical attack rate among the others varied with the amount of time spent aboard. Virus antigenically similar to A/Texas/1/77(H3N2) was isolated from 8 of 31 passengers cultured, and 20 of 22 ill persons tested had serologic evidence of infection with this virus. The airplane ventilation system was inoperative during the delay and this may account for the high attack rate.

A 50-year-old woman with a 27-year history of ankylosing spondylitis developed cricoarytenoid joint arthritis that was indicated by hoarseness, sore throat, and vocal cord fixation. This unusual manifestation of ankylosing spondylitis responded to treatment with moderate doses of prednisone, but it recurred when the cortiocosteroid therapy was discontinued.
