Recent evidence indicates that very early therapy will improve prognosis and perhaps even eradicate HIV; however, primary HIV infection is not being recognized because the diagnosis is not considered. To aid in early HIV diagnosis, the natural history of Acute Retroviral Syndrome (ARS) is outlined, including clinical presentations. It is known that a large proportion of patients seek medical attention during the acute retroviral infection stage at which time the patient most commonly suffers from either fever,

fatigue
, weight loss, and/or myalgia. Although data are limited, there is a strong case for starting antiretroviral therapy as soon as possible after a patient is diagnosed with ARS. Duration of treatment has yet to be determined. Recommendations for an initial diagnostic and therapeutic approach when a patient presents with possible ARS are provided.

A comprehensive description of working conditions, exposure patterns for organic solvents, and related health symptoms among workers in ten small screen printing companies located in Seattle, Washington, is presented. Sampling methods included continuous area monitoring, grab sampling, personal sampling, and time study observation. A total of 27 workers were observed and monitored for solvent exposure. Short-term peak exposures were characterized in terms of magnitude, duration and repetition, and their contribution to time weighted average (TWA) exposures were evaluated. A health questionnaire addressing the symptoms potentially attributable to solvents was used to investigate the possible health effects from exposure. Significant differences in the prevalence of headaches, dizziness, intoxication, and dry skin (p < 0.01) were reported among workers who had some solvent exposure compared with the referent group that was not exposed. , and a low alcohol tolerance. The study documented highly variable levels of solvent exposures. Screen printing workers in different companies, while performing the same basic tasks, had time weighted average (TWA) exposures ranging from 2% to 100% of the recommended threshold limit value (TLV) for mixtures. Continuous monitoring indicated that high short-term exposures are responsible for the bulk of TWA exposures. Grab samples and continuous monitoring verified that recommended Short Term Exposure Limits (STEL) for individual solvents may be exceeded on a routine basis. Frequent skin contact with solvents was also observed. Health problems in this industry and other small industries using organic solvents may result from these complex patterns of exposure.

Non-specific health symptoms are commonly reported in communities exposed to environmental hazards. However these symptoms commonly occur in the general community as a result of lifestyle factors, illness such as cold and allergy, medical treatment, in addition to environmental and occupational exposures. This paper describes the prevalence of non-specific health symptoms in South Australia. A cross-sectional survey of 3016 persons aged 15 years and over across the State was undertaken. Stuffy nose was the most commonly reported symptoms, with a 2-week prevalence of 46%. Headaches, unusual

and itchy eyes were each experienced by at least one in five persons in the previous 2-week period. Symptom reporting was greater among females compared with males and higher in younger age groups. The 1994 case definition of chronic fatigue syndrome is widely used not only for diagnosis but also for clinical and laboratory-based observations of this clinical entity. The criteria for the 1994 case definition are based primarily on symptoms and not on physical signs or chemical or immunological tests. This situation has resulted in conflicting clinical and laboratory observations that in all likelihood is due to different populations of patients being studied in different centers. Based on some of the recent publications, there appears to be an emerging picture of this disease entity that we propose could be used to subgroup chronic fatigue syndrome into four different subclasses. with primarily nervous system disorders such as impaired memory or concentration and headache, chronic

with primarily endocrine system disorders such as unrefreshing sleep and postexertional malaise, chronic

No link between infections and CFS has been clearly established but the immune system is activated, there are aberrations in several hypothalamic-pituitary axes and involvement of other parts of the central nervous system. No specific treatment has been found. Cognitive behavioural therapy is established to be of value to improve quality of life. More effective treatment should result, as advances in biomedical as well as psychological research continue.

A 14-year-old girl was admitted to the hospital because of persistent

, 25 pound weight loss, and leukopenia. On physical examination she was thin, ill-appearing, and had necrotic papules on the face and palpable cervical lymph nodes. Presumptive differential diagnosis included occult malignancy and infection. Numerous investigative procedures failed to elucidate a source. Vasculitis was eventually appreciated after repeat skin biopsy. Numerous serologic studies were performed and were notable for a very low level of the second component of complement without direct evidence of lupus erythematosus (LE) or other autoimmune conditions. A diagnosis of C2 deficiency-associated vasculitis was made. She was treated with high-dose prednisone and cyclophosphamide with resolution of her symptoms. Two years later she returned with marked malar erythema. Antinuclear and Smith antibodies were then detected and a diagnosis of LE was made. She was treated with hydroxychloroquine and sun-avoidance measures with clearance of the malar rash.
