Cancer mortality in the 35-74 year age-range for selected sites during the period 1979-88 was investigated for the 26 district council areas of Northern Ireland. Trends in rates during the period were also studied and compared with trends in an earlier period, and with trends reported from the rest of the United Kingdom. Statistically significant differences between the age-standardised death rates in the 26 areas were observed for stomach cancer (women only), pancreatic cancer (women only),
lung cancer
(men and women) and for all cancers (men and women). Some evidence of spatial aggregation of rates was apparent for ovarian cancer even though rates in the 26 areas did not differ significantly. The patterns are illustrated with maps and some difficulties of interpretation are discussed. Mortality rates for oesophageal cancer increased during the period in both sexes while rates for stomach cancer decreased.
Colon cancer
rates increased significantly only in men, while an increase in
lung cancer
rates was confined to women. The mortality from all cancers increased significantly during the period by 0.8% per annum in men and 0.9% per annum in women. These trends were found to be broadly comparable with those reported elsewhere in the United Kingdom.

Data were analyzed from 33 prospective studies to assess the evidence for a long-term association of low serum cholesterol with cancer. In subjects with cancer diagnosed within two years of the cholesterol measurement or causing death within five years (n = 4,661), the level of serum cholesterol was on average lower than in controls by 0.18 (SE = 0.02) mmol/l in men and 0.11 (SE = 0.04) mmol/l in women; this effect can be attributed to preclinical cancer. For cancers presenting after these intervals (n = 22,030), the average differences were smaller but statistically significant (0.04 [SE = 0.01] mmol/1 [P less than 0.001) in men, and 0.03 [SE = 0.01] mmol/1 [P = 0.005] in women), equivalent to about a 15 percent increase in cancer incidence in the lowest cholesterol quintile. This cannot be attributed entirely to preclinical cancer. In men, there was significant (P = 0.01) heterogeneity between studies as to the extent of a long-term association. The heterogeneity could be substantially explained by socioeconomic status, the association being pronounced in studies of manual workers but absent in studies of professional men. The overall long-term association was attributable mainly to
lung cancer
in men, and partly to hemopoietic cancers (representing prolongation of survival by treatment).
Colon cancer
and other cancers unrelated to smoking showed no long-term association with low cholesterol. The data collectively do not justify concern that lowering serum cholesterol to reduce ischemic heart-disease risk might cause cancer. The long-term association with
lung cancer
is probably caused by smoking and we propose a mechanism.

Bioactive terpenes were isolated from the following plant species: 1) Pseudolaric acids: The active compounds of novel diterpenes, Pseudolaric acid A, B, C, and D, were isolated from Pseudolarix kaempferi Gordon (pinaceae). Their structures were determined by spectroscopic data and chemical correlations. In continuous studies, absolute configurations, confirmation in solutions, fragmentation mechanisms of MS, and assignments of all nuclear magnetic resonance (NMR) spectral signals were also reported. cell lines. 2) Daphnane diterpenes: In further studies on the plants of Thymelaeaceae, besides 10 known diterpenes, 16 new daphnane diterpenes were isolated from Daphne genkwa, D. tangutica, D. giraldii, Wikstroemie chamaedaphne that showed antifertility activities. In continuous studies, besides 6 known compounds, 17 new daphnane diterpenes were isolated. 3) Tripterygium diterpenes: 14 new diterpenes were isolated from Tripterygium wilfordii, T. regeli and T. hypoglaucum. Tripterygium wilfordii has been used as an anticancer drug, male contraceptive, and immunedepressant in Chinese folk medicine for a long time. Some of them showed antitumor activities. The CD spectra showed that A/B ring of all diterpenes had the same transconfiguration as tripdiolide and triptolide as determined by X-ray diffraction. In 1972 triptolide, tripdiolide and triptonide were isolated with significant anti-tumor activities. Celastrol demonstrated immunodepressive activities. In addition to 10 known triterpenes, 15 new diterpenes were isolated from the plants of Tripterygium by silica gel chromatography. 4) Pregnane glycosides from Marsdenia koi: Two new pregnane glycosides, marsdenikoiside A and marsdenikoiside B, which can terminate early pregnancy were isolated from Marsdenia koi. Their structures were elucidated by hydrolysis and spectroscopic methods.

The establishment of reliable preclinical test is essential for the reasonable clinical trial. As a methodology for the screening of new active anticancer agents, disease oriented strategy using human tumor cell lines has been proposed in USA. The important questions in DOS are to determine the representative cell lines of specific cancer and it is also extremely important to decide the numbers of cell lines used for the screening. CPT-11, topoisomerase I inhibitor, developed in my country has been demonstrated to be active against
lung cancer
cell lines compared with mice tumors such as S-180 and P-388. However, no compound is demonstrated to be clinically active so far by this methodology. The criteria for the application of clinical trial are obscure and each drug company decides empirically by themselves. We have proposed to use PEI (predictive efficacy index) for the prediction of antitumor activity of new compounds. The clinical effect of new platinum analogue well correlated with this value. We have conducted phase II trial of 5-FU + LV against NSCLC without no prior chemotherapy. No responder was observed in the trial. Augmentive effect of leucovorin on the cytotoxicity of 5-FU and FdUrd was examined in vitro against NSCLC and

Hawaiian Japanese men (n = 432) who had undergone subtotal gastrectomy for peptic ulcers before 1971-1975 were followed up for detection of cancer development. They showed a significant increase in
colon cancer
risk (P = 0.008) and
lung cancer
risk (P = 0.002) compared with 6161 nongastrectomized men. The association with
lung cancer
persisted after adjustment for cigarette use (P = 0.03). Alcohol consumption was associated with colonic cancer in this cohort, and gastrectomized men consumed more alcohol than nongastrectomized men; however, the association of gastrectomy with
colon cancer
persisted after adjustment for alcohol use (P = 0.02). Gastrectomized men were lighter and had lower serum lipid levels than controls, suggesting that undernutrition might favor the development of some cancers. The type of gastroenteric anastomosis did not influence the cancer risk level in the colon or lung.

, one of impending rupture of abdominal aortic aneurysm and one of purulent cholecystitis with cholelithiasis, were discussed. Mean age was 62.0 years (range, 36 to 73); four were male and two were female. Case 1 and 2 were admitted with anterior chest pain, Case 3 with lumbago and abdominal pain, Case 4 and 5 with an abnormal shadow on chest x-ray film and Case 6 with abdominal pain. Of the two with unstable angina, one was operated on with right upper lobectomy during the first months after aorto-coronary bypass. Of the two with
colon cancer
, one was operated on with right upper lobectomy during about 5 weeks after right hemi-colectomy. Case 3 with abdominal aortic aneurysm operated on with left upper lobectomy during 4 weeks after replacement of abdominal aorta. Case 4 with cholecystitis was operated on with left pneumonectomy during about 3 weeks after cholecystectomy. The postoperative course of 4 cases and the post-chemotherapy condition of 2 cases were uneventful.

Out of 11,136 Japanese men identified on the island of Oahu, Hawaii in 1965 by the Honolulu Heart Program, 8006 responded to a mailed questionnaire and were examined. Some 1871 responded only to the mailed questionnaire, and 1259 did not respond at all. After 15 years of follow-up, the examined men had significantly lower risk of death from all causes and death from cancer. Minor differences were also noted between the two groups in the risk of cancer of the lung, stomach, colon, and rectum. However, the examined men had a significantly higher risk of prostate cancer. In general, the strength of these non-response effects was mainly due to risk differences in the first five years of the 15-year follow-up period. The relative risk (RR) of each of the seven endpoint events tended towards 1.0 as each of the three successive five-year follow-up intervals were considered. An exception to this was the prostate cancer incidence RR which favoured the unexamined men throughout the entire 15 years, but significantly so only in the last five-year follow-up interval. When the 8006 examined and 1871 unexamined men who responded to the mailed questionnaire were evaluated with respect to the association of cigarette smoking with
lung cancer
incidence, the RR for smokers was 9.77 for the examined men, and 6.73 for the unexamined men. Since these RRs are not significantly different, there should be little bias in RR estimates of cigarette smoking for
lung cancer
if the observation was limited to only the examined men. With regard to the association of body mass index (BMI) with
colon cancer
in older men, the RRs for men in the highest BMI quintile were quite comparable, at 1.37 for the examined group and 1.60 for the unexamined men. We conclude that although some non-response effects on cancer incidence exist in this cohort, they do not appear to be serious enough to have changed conclusions drawn about risk relationships.

In 1971-1973 at the third examination of the Basel Study started in 1959, the major antioxidant vitamins and carotene were measured in the plasma of 2974 men. A subsample and their families were reinvestigated in 1977-79. During the 12-y observation period (1973-85) 553 men died, 204 of cancer (
lung cancer
68, stomach cancer 20;
colon cancer
17, all other malignancies 99). We found significantly lower mean carotene levels for all cancer, bronchus cancer, and stomach cancer (all P less than 0.01) compared with the 2421 survivors. The relative risk of subjects with low carotene (less than 0.23 mumol/L) was significantly elevated (P less than 0.05) for
lung cancer
(Cox's model). Higher risks were noted for all cancer (P less than 0.01) if both carotene and retinol were low. Low plasma carotene which is known to reflect carotene intake is in our study associated with increased cancer risk.
