Although various materials have been used for reinforcement in lung-volume-reduction surgery to buttress pulmonary staple-line, absorbable materials are not available for use in thoracoscopic surgery. Moreover, even nonabsorbable types of reinforcements have been used only for lung volume reduction surgery. However, elderly patients with spontaneous
pneumothorax
secondary to emphysematous lung are well treated with staple-line reinforcement. The authors developed a new type of polyglycolic acid felt to buttress staple-line. This felt is absorbable, easier to cut with a stapler knife than is the conventional polyglycolic acid felt, and inexpensive enough to use for various types of thoracic surgeries for emphysematous lungs in Japan, and it can be attached to staplers with a small amount of fibrin glue. These strips were used to reinforce pulmonary staple lines for resection of emphysematous lungs in 14 patients: pulmonary emphysema (n = 1), bilateral giant bullae (n = 1), ipsilateral giant bullae (n = 6), spontaneous
pneumothorax
with multiple bullae in an emphysematous lung (n = 5), and
lung cancer
in a patient with pulmonary emphysema (n = 1). There were no air leaks during surgery. Air leaks were noted in three patients after surgery. In two patients, the air leaks stopped within 2 weeks. In one patient, the air leak was found to originate from an untouched lobe during reoperation. No infection or allergic reaction developed in a patient during a mean follow-up of 12 months (range, 1 to 24 months).

In bullous lung disease the incidence of
lung cancer
occurs with significant frequency among the general population, but
lung cancer
with a spontaneous
pneumothorax
is rare, and the prognosis is frequently poor. Five cases of
lung cancer
with emphysematous bullae of the lung were reported. In 4 cases there were simultaneous occurrences of
lung cancer
and
pneumothorax
. The patients were 49-, 36- and 68-year-old men and a 72-year-old woman, and the initial sign in all was
pneumothorax
. Histologically, there was one squamous cell carcinoma, two poorly differentiated adenocarcinomas and one well-differentiated adenocarcinoma. In the other case, giant emphysematous bullae were diagnosed in a 51-year-old man and were treated by bullectomy. Histological examination incidentally revealed a small well-differentiated adenocarcinoma, which extended along the wall of the largest right bulla. About 18 months later, the patient was found to have poorly differentiated adenocarcinoma of the left lung, which was treated surgically. We should always be careful of

associated with emphysematous bullae and

Photodynamic therapy (PDT) has now achieved the status of a standard treatment modality for centrally located, early-stage
lung cancer
and is introduced on the home page of the US National Cancer Institute. Over the past decade, 145 patients (191 lesions) with central-type early-stage lung cancers have been treated with PDT in our hospital. Overall complete remission was obtained in 86.4% of the total number of lesions. In patients with advanced stage III disease, however, bronchi opening was successful in 61 out of 81 lesions (75%) for the PDT group compared with 143 of 177 (81%) in the Nd-YAG laser therapy group. Although quite recent, treatment using PDT has been introduced for the first time in patients with peripheral
lung cancer
, who did not previously meet the criteria for surgery. Eight patients underwent this trial, of which 3 achieved partial remission. No serious complications except for one case of
pneumothorax
, were noted. As increasing number of patients consider quality of life after therapy, the indications for PDT are expected to expand. The success in clinical trials of PDT for cancer treatment offers encouragement for its future use.

are numerous whereas the use of a needle size larger than 18 gauge is the major risk factor of bleeding. Although a coaxial system is highly suitable in any case, two categories of needles can be used; those providing aspiration and those for core biopsies. Both offer similar yields for the diagnosis of malignancy, but core biopsies are more efficient for the specific diagnosis of benignity and lymphoma. Technical improvements of guidance, needle design and pathological techniques may contribute to lower the size limit of the nodule to be biopsied, to decrease the complication rate and their severity and to increase the yield of diagnosis.

We document an uncommon variant of pulmonary hamartoma (PH), discovered in the left upper lobe of a 60-year-old man after an episode of
pneumothorax
, a unique clinical presentation for such an occurrence. The tumour showed a prominent leaf-like pattern and was mainly composed of mature fat with adipocytes of different size and scattered throughout lipoblasts and floret-like cells. A lobectomy was performed and more than 7 years after surgery the patient's course is still uneventful. This case represents an exceedingly rare example of an histologically malignant change in PH and bears a close clinicopathologic resemblance to atypical lipomatous tumours of soft tissues.
