Stroke is a common cause of morbidity and mortality in adults worldwide. Because patent foramen ovale (PFO) is commonly found in normal population, we need to identify a subset of cryptogenic stroke patients who are likely to have experienced paradoxical embolization. Various factors need to be considered such as atrial anatomic variation (PFO size, atrial septal aneurysm, eustachian valve anatomy), hemodynamic parameters, presence of venous thrombosis and presence ofhypercoagulable state. The presence of any of these findings increase the chance of PFO contributing to stroke. We describe a 54-year-old patient with a history of well controlled hypertension and
dyslipidemia
who presented with 3 attacks of expressive
aphasia
lasting 5 minutes each. General medical and neurological examinations were normal. Transesophageal echocardiography with agitated saline injection revealed presence of PFO flap. Transcranial Doppler ultrasonography with three agitated saline injections showed multiple unilateral microembolism signals in the M1 of left middle cerebral artery. Aspirin was given as well as percutaneous endovascular PFO closure was performed with no immediate complication. Patient has had no further attack of stroke after 6 months follow-up.

We present an ex-smoker, 76-year-old man, with high blood pressure, diabetes mellitus and Barrett's oesophagus (treated with esomeprazole since 2003) admitted due to fluctuating
aphasia
. Neurovascular and neuroimaging studies were normal.
Dyslipidemia
and atrial arrhythmia were discovered. The patient was discharged with the diagnosis of left middle cerebral artery transient ischaemic attack and anticoagulation treatment was recommended. The patient returned to the emergency department on further two occasions (confusional episodes) and was admitted in order to complete the neurological study that was normal. The patient was discharged with the diagnosis of probable epileptic seizures. After a week, he was admitted due to generalised temblor and unsteadiness. A complete blood test was performed and showed a severe hypomagnesaemia (not previously performed).

aphasia
. His past medical history was significant for a previous left middle cerebral artery stroke, from which he fully recovered, hypertension,
dyslipidemia
, coronary artery disease, one episode of atrial fibrillation postoperatively, and thalidomide exposure in utero. Although initially he was thought to be aphasic, on further examination, he demonstrated significant abulia. His level of consciousness was normal, and neurological examination was otherwise unremarkable. A CT angiogram of the head and neck was performed. The patient was not a candidate for acute therapy, as he had established stroke on imaging, and the time of onset was unclear.
