Haematological and parasitological investigations were carried out on apparently healthy subjects in a rural area of Natal. Anaemia was highly prevalent and of a moderate to severe degree (haemoglobin value less than 11,0 g/dl) in 42% of males and 52,5% of females. In 50% of subjects the anaemia was hypochromic and iron deficiency appeared to be the major contributing factor. However, failure to utilize iron, giving rise to hypochromia, was not excluded as a complicating factor. Subnormal serum vitamin B12 levels were found in some subjects, and this frequently coexisted with iron deficiency. The presence of parasites was not considered to have contributed to the production of anaemia. It was concluded that the anaemia was to a large extent nutritional in origin.

The spectre of methods for the diagnostics and differentiation of haemolytic anaemias, particularly for the establishment of congenital, autoimmune haemolytic, drug-conditioned and other anaemias is treated. The clear delimitation of an iron deficiency from a disturbance of the iron distribution is advantageously to be achieved by iron staining of the bone marrow and by a determination of serum ferritin. The value of the diagnostic methods in megaloblastic anaemia is classified according to newer knowledge, in which case the vitamin-B12-absorption test and the serum level determination of vitamin B12 by no means range in the first place. Long-term culture results of haematopoietic stem cells are particularly evident in the aplastic syndrome of the bone marrow and further haematological diseases concerning the establishment of the intensiveness of proliferation. The classification of the acute leukemias demands conventional as well as cytochemical staining methods; recently, it is essentially improved using monoclonal antibodies. In leukemias cytogenetic investigations are more and more attracted to the estimation of the prognoses. In lymphogranulomatosis among others functional disturbances of the cellular immunity, in the group of the non-Hodgkin-lymphomas haematological, protein-analytic and immunological laboratory investigations are methods supporting the diagnosis. Altogether is to be established that the haematological diagnostics has become more and more perfect, in which case apart from new techniques old approved methods are still further used.

104 patients with positive stool for hookworm ova were studied in detail with regard to anemia, iron and protein deficiency, and their relation to hookworm ova load. A variable degree of anemia was seen in 80 of 104 patients. Hypochromia was noticed in 66 (64%). In 48 (46%), morphological changes due to vitamin B12 and/or folic acid deficiency were recorded. Iron deficiency was most common in anemic patients. Less than 15% saturation of transferrin was the most sensitive biochemical index of iron deficiency in these patients. Severity of anemia was significantly associated with iron deficiency. Hypoalbuminemia (serum albumin less than 3.25 g%) was found in 33 (32.6%) of the patients. Anemia and hypoalbuminemia were both significantly associated with the hookworm load. However, the association of hookworm load was seen with severe anemia (hemoglobin less than 5 g%) and hypoalbuminemia (serum albumin less than 2.75 g%). It has been suggested that besides parasitic factors, nutritional deficiencies of vitamin B12 and/or folic acid and protein are contributory factors in the pathogenesis of anemia and hypoalbuminemia respectively found in these patients.

Haematological indices, malarial parasitaemia, serum and red cell folate (SFA, RCF), serum vitamin B12 and haemoglobin (Hb) electrophoretic patterns were studied in 228 non-elite young Hausa primigravidae at less than 24 weeks of gestation. The study was conducted in the guinea savanna of Nigeria, where malaria is hyperendemic. Ninety-nine (43%) were anaemic (Hb less than 11.0 g dl-1). The commonest cause of anaemia was malaria, in 28% of all and 40% of anaemic subjects. Plasmodium falciparum was predominant; P. malariae was seen in 1.3% and P. ovale was not recorded. Parasitaemia was more frequent and more dense in the wet than the dry season. Iron deficiency was diagnosed in 18% of all and 25% of anaemic women; 14% of all patients were folate-deficient; high MCV and MCH correlated with anaemia, and low SFA was associated weakly with anaemia and malaria. Serum vitamin B12 was normal or high in all 145 in whom it was measured; 3% had congenital elliptocytosis, but this did not contribute to the anaemia. Sickle-cell trait was present in 26% and Hb-AC in less than 1%. Hb-AS was associated with significantly lower frequency and density of P. falciparum; this has not been demonstrated in pregnancy in Africa previously. However, the parasitological advantage was not reflected in any haematological advantage. The roles of malaria, folate-deficiency and iron-deficiency in the causation of anaemia in Hausa primigravidae will be defined further by a double-blind trial of antimalarial prophylactics, iron supplements and folic acid supplements.

Over the first three months of continuous ambulatory peritoneal dialysis (CAPD) the level of hemoglobin (Hb) rises significantly in most patients. To elucidate this further we studied the hematological response over 3 months of 8, previously non-transfused new patients treated with CAPD. Mean Hb rose by +2.78 g/dl (P less than 0.02). Mean RCM rose by 284 ml (37.7%) (P less than 0.05) and 3.7 ml/kg (29.6%) (P less than 0.05). PV fell relative to BW only, by -8.6 ml/kg (P less than 0.05). There was no significant change in serum vitamin B12 or folate concentrations or evidence of hemolysis. Plasma ferritin fell in all patients, but hematological changes of iron deficiency appeared in only one. Bio-assayable erythropoietin (EPO) levels were generally in the normal range, but inappropriately low for the degree of anemia. EPO did not change significantly apart from in two patients, one with polycystic disease. These results indicate that over the initial 3 months of therapy the majority of CAPD patients have a rise in Hb, due mainly to a rise in RCM, unrelated to changes in serum EPO level.

A full haematology screen was carried out on 398 patients under investigation for recurrent oral ulceration or stomatitis. Thirty-three patients were found to have lowered serum folate and/or red cell folate levels without iron deficiency. Of these only six were found to have a mean corpuscular volume (MCV) outside normal limits or to have recognisable erythrocyte abnormalities. No correlation was found between serum or red cell folate levels and the MCV. Eighteen patients were found to have lowered serum B12 levels without iron deficiency, of these seven were found to have a MCV outside normal limits. A significant negative correlation was found between serum B12 levels and the MCV. It is concluded that haematological screening in these cases should include estimations of serum folate, red cell folate and serum B12 levels even in the face of an apparently normal peripheral blood film.

Nutritional anemia results from deficiency in either iron, vitamin B12 or folic acid, although in most areas of the world, nutritional anemia is synonymous with iron deficiency. Nutritional anemia occurs most commonly at times of heightened physiologic requirements. It is considered a major health problem in developing countries, although it is not a serious concern for the general U.S. population. Iron deficiency without frank anemia, however, does affect some segments of the U.S. population, such as pregnant and menstruating women and children.

Since physicians are routinely taught to review the peripheral blood smear results of all anemic patients, we analyzed the diagnostic value of the laboratory's blood smear reading and the incremental value of a physician's personal reading in anemic inpatients. Blood smear abnormalities, as reported by the laboratory and two hematologists, were poorly reproducible, with only five of the 11 types of abnormalities being more reproducible than could be expected by chance. The blood smear performed no better than RBC indices in detecting probable iron deficiency or low serum levels of folate or vitamin B12. In anemias not caused by deficiency states, the blood smear reading performed by the hospital laboratory provided unique information in 6% of the cases and helpful information in another 25%, but the additional reading performed by a hematologist never provided unique information and provided incremental helpful information in only 4% of the cases. The peripheral blood smears of all anemic inpatients should be read by the hospital laboratory, but in our hospital, a routine additional personal reading by a physician had limited incremental value and could be reserved for selected cases.

The haematological aspects of life with an ileostomy have been studies in 51 patients, of whom 39 had had ulcerative colitis and 12 had Crohn's disease. The findings in these patients have been compared with those in 39 healthy volunteers who were matched for age and sex with the 39 patient who had had ulcerative colitis. There was evidence of a mild degree of iron deficiency in the patients with an ileostomy. This was partly due to a pre-existing iron deficiency resulting from their preceding illness and operation, as the abnormality was less pronounced in the patients in whom the ileostomy had been established for more than 3 years. There was some evidence of excessive iron loss and a controlled trial of ferrous fumurate showed that the iron deficiency was largely corrected by this means. Circulating levels of vitamin B12 were normal, but it is relevant that some of the Crohn's disease group were receiving parenteral supplements. The absorption of vitamin B12 wa low in the patients with Crohn's disease who had had an ileal resection of more than 17 cm. The absorption of vitamin B12 in the patients who had had ulcerative colitis was increased and possible mechanisms are discussed. All but one of the patients had normal levels of plasma folate and in all the red cell blood folate was normal, which can be taken as an indication of a good dietary intake and adequate absorption.

Sexually active patients (12 to 19 years of age) participated in a study designed to determine if adolescents who take oral contraceptives are susceptible to hematologic abnormalities. This involved individual medical histories recording vitamin ingestin, alcohol consumption, type of contraceptive used, duration of usage and menstrual cycle. Blood samples were drawn for determinations of serum folate, B12, ferritin, blood count and polymorphonuclear leukocyte lobe count. Results indicate that adolescents with nutritionally poor diets are potentially high risks for developing folic acid deficiency while taking oral contraceptives. The data indicate no significant differences in the mean hemoglobin, red blood cell indices or polylobe count in either the patient or control group. The authors stress a need to gather more data on the effects of long term contraceptive pill usage. It is concluded that although contraceptive pill use appears to increase the risk of developing cellular folate deficiency, especially in white women, the pill appears to offer some protection against iron deficiency and is not associated with anemia.
