Leiomyosarcoma
of the inferior vena cava is reported with increasing frequency but remains a rare lesion. In this article we report one case and review 69 cases. The disease affects mainly women approximately 50 years of age. The symptoms vary according to the location of the tumor; Budd-Chiari syndrome and
leg edema
occur in the more centrally located tumors and abdominal pain and swelling are seen in the more peripherally located ones. In 40% of the cases distant metastases are evident at the time of the patients' first presentation and are found most commonly in the liver and/or lungs. After resection the majority of the patients eventually die of metastatic disease with a mean survival of approximately 40 months.

Leiomyosarcoma
of the inferior vena cava is a rare tumor that is usually fatal. The tumor may grow very slowly or occasionally very rapidly, shows extensive local invasion, and metastasizes more frequently than previously believed. Complete surgical resection remains the only potential curative therapeutic option. The aim of this study was to report the clinical experience in the management of a patient with leiomyosarcoma. A 65-year-old woman with a history of vague abdominal pain and leg swelling underwent computed tomography which demonstrated an occlusion of the inferior vena cava. The patient received a complete excision of the tumor without reconstruction and histological analysis confirmed the diagnosis of leiomyosarcoma type 1. At 3 years, the patient is still doing well with minimal
leg edema
and a contrast-enhanced CT demonstrates no evidence of recurrence locally or in distant sites.
Leiomyosarcoma
is a rare and aggressive tumor that presents with non-specific symptoms. Computerized tomography with 3-D reconstruction is a useful tool to define the presence and entity of the collateral circulation and therefore to decide on the surgical strategy. Resection probably offers the best opportunity for long-term survival.

Leiomyosarcoma
is an aggressive soft tissue sarcoma derived from smooth muscle cells. Of all soft tissue sarcomas, approximately 5-10% are leiomyosarcomas. Vascular leiomyosarcoma constitutes about 2% of all leiomyosarcomas and involves veins 5 times more than arteries. When they arise from a major blood vessel, symptoms of vascular compromise or

may be present. Because they are rare, definite diagnosis is often delayed. We present the case of an 88-year-old man who was admitted to our department with acute limb ischemia stage 4 according to Rutherford's criteria. His personal medical history included arterial hypertension under medication with nonspecific conduction disturbances showed in the electrocardiography. The duplex scan revealed the presence of thrombotic material to the distal superficial femoral and popliteal artery, whereas the presence of popliteal artery aneurysm was excluded. After the initial diagnostic approach, he underwent 2 unsuccessful embolectomy procedures. During the amputation procedure, a 6-cm mass was palpated in the popliteal fossa, and it was excised. The immunohistopathologic study revealed a grade 3 according to the French Federation Nationale des Centers de Lutte Contre le Cancer classification leiomyosarcoma. The patient was discharged 10 days later and referred to an oncologic center. He returned 6 months later with edema of the amputated limb and inguinal lymphadenopathy. Specimen of the inguinal lymph nodes was sent for histopathologic examination, which indicated the recurrence of the disease. Leiomyosarcomas should be taken into consideration in elderly patients presenting with acute limb ischemia.
