A 71-year-old man was admitted for right
loin pain
and fever. DIP showed a non-visualizing right kidney. Retro- and antigrade pyelography demonstrated right hydronephrosis secondary to uretero-pelvic junction obstruction with leakage of the dye from the right kidney into the duodenum. Diagnosis of spontaneous pyeloduodenal fistula was confirmed at the operation. Right pyeloplasty was performed after closure of the duodenum. We believe that this is the 9th case reported in the Japanese literature.

A clinical survey was performed on 24 cases of primary ureteral tumors treated in our University Hospital between Jan. 1972 and Dec. 1981. The incidence of primary ureteral tumors was 0.17% among the outpatients in our urologic clinic. The patients ranged from 41 to 84 years old in age (average age: 65 years), and the male to female ratio was 3 to 1. The left ureter and the lower third of the ureter were involved more frequently. The most frequent initial symptom was macrohematuria seen in 19 cases (79%), followed by
loin pain
seen in 6 cases (25%). A cytologic study of the voided urine or ureteral urine was carried out in 18 cases and positive results were obtained in 9 cases (50%). Cystoscopic examination revealed a tumor protruding through the ureteral orifice in 16.7% (4 of 24 cases), followed by bulging orifice in 8.3% (2 of 24 cases). Excretory pyelography showed ureteral filling defects in 41.7% (10 of 24 cases). Retrograde pyelography showed ureteral filling defects in 100% (10 of 10 cases). CT showed a mass region in 44.4% (4 of 9 cases). A total of 14 patients underwent nephroureterectomy with partial or total cystectomy (66.7%). The overall 5-year-survival rate was 56.5% and could be directly associated with grade and stage of tumors.

During the last 8 years we localized 725 urinary tract infections in 213 patients via the bladder washout test. With this test the infections were located in the upper urinary tract in 11.9 per cent of the patients without vesicoureteral reflux, 27.1 per cent with reflux and 25.3 per cent without reflux after a successful antireflux operation. Of all supravesical and vesical bacteriuric episodes 57.6 and 67.1 per cent, respectively, were asymptomatic. The presence of symptoms differed only slightly among patients with and without reflux, and with and without pyelonephritic scars. Vesical symptoms, such as dysuria and frequency, were seen in 18.8 per cent of the patients with supravesical and 26.2 per cent with vesical infections, while fever and/or

A 16 year old male with a history of recurrent synpharyngitic macroscopic hematuria presented with severe

, macroscopic hematuria and oliguric acute renal failure. Although renal biopsy showed mesangial IgA nephropathy with focal and segmental proliferation with crescents, the extent of glomerular involvement could not explain the severe functional disturbance. Intratubular obstruction and tubular necrosis secondary to the presence of severe glomerular bleeding provide a better explanation. Although renal failure resolved spontaneously, the long-term effects of episodes of this type on structure and function is unknown.

We report on a man with bilateral multiple renal oncocytomas who had enlarged kidneys, and presented with
loin pain
and gross hematuria. A provisional diagnosis of multiple renal oncocytomas was made after renal angiography demonstrated the characteristic appearance of this tumor. The diagnosis was confirmed with histology of an open renal biopsy specimen. This case is unique in that the lesions were multiple and bilateral. The patient had impaired renal function and, hence, the necessity for conservative management once the histological diagnosis had been established. Since in previously reported cases oncocytomas have been removed completely our case provides an opportunity to observe the natural history of the disease.

An unusual case of localized amyloidosis of the ureter is described and a review of the 12 previously reported cases is presented. No case had been diagnosed prior to surgery.
Loin pain
, with or without haematuria, associated with a sterile pyuria and an abnormal pyelogram were the commonest clinical signs. In the case reported an incidental small renal cell carcinoma was also present and the association of amyloidosis and neoplasia is discussed.

Cystoscopy, intravenous urography and urine culture frequently fail to reveal the cause of
loin pain
and haematuria in young women. Two such women are presented in whom renal biopsy was not only unhelpful but also misleading. Subsequent renal arteriography showed arteriovenous fistulae which were considered erroneously to be the cause of the symptoms in both patients; in fact the fistulae were traumatic post-biopsy fistulae rather than congenital malformations. These patients were probably examples of the "loin/pain/haematuria syndrome" which is thought to have a vascular basis and may have predisposed to the formation of fistulae.

. These problems both had other simple explanations. Accessory spleen should always be considered by the urologist when investigating a retroperitoneal mass, even when the patients has already had a splenectomy.

During an 18-year period, 28 adults with ureterocele were admitted to hospital with uncertain urological or abdominal symptoms. All of the ureteroceles were treated with transurethral diathermy incision. At follow-up examination after a mean observation time of 7 years, 16 of 20 patients were free from symptoms and the other 4 had only minor

or dysuria. Significant vesico-ureteric reflux or depression of the renal filtration rate did not develop in any case. It is concluded that simple transurethral incision is adequate for treatment of small or medium-sized ureteroceles in adults.

A review of 23 cases of renal cell carcinoma was undertaken. The sex distribution was 3.6 males to 1 female, the male preponderance being higher than in most reported series. The tumour was most common in the 6th and 7th decades of life. Haematuria was the commonest presenting symptom (52.2%); however, all the features of the classical triad of haematuria,
loin pain
and swelling were not present in any of our patients; 26% of patients presented with non-urological symptoms. At the time of diagnosis, four patients (17.3%) had evidence of metastasis. Another four developed secondaries after nephrectomy. Our approach to renal cell carcinoma has been aggressive and all 23 patients including those with metastasis were treated by nephrectomy.
