The clinical presentation of a congenital mucocele is a blue-gray
tense
mass located inferior to the medial canthal ligament. The differential diagnosis is that of tumor and encephalocele. Diagnostic modalities such as simple transillumination and ultrasound aid the physician, but dacryocystography confirms the marked
lacrimal
sac distension. In three infants the sac could not be decompressed by massage, indicating that the canalicular-punctal system as well as the inferior
lacrimal
system were blocked. When the distended
lacrimal
sac compresses the two canaliculi and bends them on themselves, there is a functional trapdoor-type block. During the probing of these patients, we felt a snap when the
lacrimal
syringe was withdrawn from the distended sac. We concluded that this was due to kinking of the canaliculus by the enlarged sac.

Seven infants had
tense
, blue-gray swellings inferior to the medial canthal tendon with otherwise normal-appearing eyelids and puncta. All
lacrimal
sacs transilluminated and A-scan ultrasonography performed in one case demonstrated a nonloculated cystic cavity. Four (57%) infants had uncomplicated mucoceles. One was treated with massage, and has remained asymptomatic for 14 months. The other three were cured with a single probing and irrigation of the entire
lacrimal
system. Three (43%) infants had developed erythema of the tissues overlying the swollen
lacrimal
sacs by the time of referral. Because none of the mucoceles could be decompressed by massage, prompt probing and irrigations were performed in each case. Cultures from the aspirates of all three sacs yielded Staphylococcus organisms. In contrast to the uncomplicated cases, two patients developed recurrences that necessitated additional treatment.

Lacrimal duct cysts are not common. It is extremely rare when a
lacrimal
duct cyst and an ectopic
lacrimal
gland develop in the orbital cavity. A unique case of an ectopic
lacrimal
gland cyst of the orbit is presented. A 33-year-old man had a palpable mass above the inferior medial orbital rim for nearly two years. An ocular examination was normal except for a movable, firm mass found in the anterior nasal inferior orbit of the right eye. An echogram revealed a homogeneous, hypoechoic cystic mass. Computed tomography of the orbit showed a well-encapsulated lesion in the lower orbit of the right eye near the inferior rectus muscle, without bony erosion. A

, thin-walled, clear fluid-filled cyst measuring 15 x 12 x 13 mm in size was completely enucleated without rupture by anterior orbitotomy. Pathologic examination disclosed a small nest of normal gland tissue surrounded by a cystic lesion lined with two layers of

A previously healthy woman was admitted to the intensive care unit (ICU) with severe H1N1 influenza. She had prolonged hospital stay due to multiple complications of critical illness, including pelvic deep vein thrombosis (DVT), which was treated with subcutaneous enoxaparin. The patient was referred to the ophthalmology service for bilateral proptosis. On examination, she had bilateral
tense
proptosis, worse on the left side with exposure keratopathy. Laboratory tests showed that she had thrombocytopenia and raised activated partial thromboplastin time (APTT). A CT scan revealed well-circumscribed soft tissue density lesions in the superolateral orbits and was reported as bilateral
lacrimal
gland enlargement. However, based on a clinical suspicion of subperiosteal hematoma collection, a diagnostic tap was performed. Following aspiration of six mls of dark blood from the left superior orbit, there was a reduction of proptosis with improvement in chemosis and resolution of exposure keratopathy. Enoxaparin is one of several antithrombotic agents which are increasingly being used for DVT prophylaxis in severely compromised patients. Furthermore, ICU patients ventilated for prolonged periods are at risk of developing chemosis and exposure keratopathy. Thus, the clinician should maintain a high index of suspicion in identifying subperiosteal hematomas, when managing such cases. The spontaneous bilateral vision threatening subperiosteal hematoma was probably caused by a combination of enoxaparin therapy and prolonged ventilation.
