We studied three patients with a progressive neurologic disorder, all of whom had worked for over 12 years in the same potroom of an aluminum smelting plant. All had
incoordination
and an
intention tremor
. Two of the three patients had cognitive deficits, and the most severely affected patient also had spastic paraparesis. None had involvement of the peripheral nervous system. Despite extensive evaluations, the cause of these patients' problems remains obscure. It is tempting to implicate one of the numerous substances to which the patients were exposed in the potroom, but none is known to cause the neurologic problems seen in these patients. Neurotoxic effects of aluminum in animals are directed at the central nervous system, and theoretically long-term low-level exposure to aluminum in the potroom could explain the findings in our patients.

Biceps brachii tonic vibration reflexes were elicited in patients with either focal or diffuse cerebellar damage and spino-cerebellar degenerations. As compared to normal controls, tonic vibration reflex amplitude was reduced in cerebellar patients, particularly in cases with unilateral hemispheric lesion, who exhibited a clear cut tonic vibration reflex asymmetry even when clinical symptoms were mild. These reflexes were absent or very weak in patients with spino-cerebellar degenerations. Muscle vibration induced in most of the patients an enhancement of mild or latent clinical symptoms such as

An electro-mechanical device has been used to measure muscle tone in 12 children with cerebellar ataxia and in 12 healthy children matched for age and sex. All the children also had their tone assessed clinically. The machine measurement showed that six (50%) of the 12 ataxic children had hypotonia, one was hypertonic, while five had normal tone. There was significant correlation between estimation of muscle tone by the machine and by clinical examination. The machine will serve as a valuable addition to other devices already being used to measure the other motor deficits encountered in children with cerebellar disease--such as devices that measure gait ataxia, truncal balance,
intention tremor
and
incoordination
of the limbs. It is re-emphasized that assessment or confirmation of clinical signs by reliable and reproducible instrumentation, offers a more objective basis for management and follow-up of patients, than clinical testing alone, and eliminates the often-encountered inter-tester discrepancies.
