1 The natural history of acute gouty arthritis was studied in 11 volunteers with podagra. 2 Two patients withdrew from the study on day 4 because of severe persistent pain. Of the remaining patients all showed some improvement in pain by day 5 and in swelling by day 7.

In spite of these improvements only three patients noted resolution of their pain during the study period. 3 These data indicate that while the majority of patients show spontaneous improvement, resolution is unlikely over a period of 7 days without the use of effective non-steroidal anti-inflammatory medication. 4 Documentation of the natural history of the acute gouty attack may assist clinical investigators in interpreting the results of uncontrolled evaluations of non-steroidal anti-inflammatory drugs.

Twenty-one cases of hamulus fracture are presented. Diagnosis depends on clinical acuity. The most common symptom is pain in the palm that is aggravated by grasp. Weakness of grasp and dorsal wrist pain are also common. Ulnar nerve paresthesia or weakness and mild carpal tunnel syndrome are frequently present.

Tenosynovitis, tendon fraying, or tendon rupture may be demonstrated in 25% of the cases and is not related to the use of steroids. Lateral trispiral tomography is clearly superior to the other diagnostic methods. Excision produced generally excellent results, particularly in patients with an athletic injury or with no associated additional injury. A nonathletic injury or the presence of associated trauma adversely affected results. Immediate immobilization of acute fractures may result in fracture healing and obviate operative intervention. Open reduction and internal fixation is feasible but offers little advantage over excision.

Pathologic tenderness should be a contributing criterion to the diagnosis of tension headache (muscle contraction headache).

The prevalence of craniomandibular disorders was studied in 285 17-year-old adolescents with the aid of a questionnaire and a functional examination of the masticatory system including evaluation of TMJs, masticatory muscles, mandibular mobility, and occlusion. The adolescents were followed up longitudinally two more subsequent years. Totally 27 subjects dropped out, leaving 258 for the longitudinal intraindividual comparisons. At the age of 17 a fifth of the subjects reported some symptoms involving the masticatory system, of which most were mild, according to the anamnestic index (Ai) used. Oral parafunctions were commonly reported of which nail-biting dominated. Grinding and clenching of teeth were reported by 8% and 11% respectively. Signs of mandibular dysfunction were found in 56% of the adolescents and were mostly mild according to the dysfunction index (Di) used. Girls more often had signs of mandibular dysfunction than boys. Morphologic malocclusion was recorded in 35%, unilateral contact in RP in 77%, lateral shift between RP and IP greater than or equal to 0.5 mm in 19% and mediotrusion interferences in 30% among the 17-year-olds. No significant difference between sexes was found. Of the sample 62% had either some sign or symptom of dysfunction and there was a positive relationship between the dysfunction indices used. Neither morphologic nor functional malocclusions were related to the Ai. TMJ sounds were related to palpation tenderness in the lateral pterygoid muscle and impaired mobility of the mandible. The number of masticatory muscles tender to palpation was related to reports of fatigue in the jaw, TMJ tenderness, and mediotrusion interferences. Recurrent headache was reported by about 18% of the girls and by almost 6% of the boys. Fatigue in the jaws and difficulties in chewing were commoner in those with frequent and more intensive headache.

to palpation of the masticatory muscles and impaired mandibular mobility were significantly commoner findings among those with recurrent headaches and those with more intense headache. Tooth-grinding and clenching were related to frequency but not to intensity of headache. Reports of TMJ sounds increased with age for girls who also more frequently reported recurrent headaches than boys. The prevalence of symptoms of mandibular dysfunction was about 20% each year, but there was no general increase of frequency and severity of symptoms during the observation period in spite of an incidence of 8%. Lesions of the biceps tendon have been studied with plain radiographs and arthrograms of the intertubercular groove. Sometimes, however, especially when the rotator cuff has a full-thickness tear anteriorly, the sheath of the biceps tendon fails to fill with contrast medium, thus preventing diagnosis of dislocation of the tendon. To solve this problem the authors have tried sonography of the biceps tendon. The suitability of sonography for examining the intertubercular groove and the biceps tendon was assessed in ten volunteers. Thirty patients with chronic shoulder pain were subsequently studied by radiography, arthrography, and sonography. The observations made with these examinations were compared, and in 21 of the patients who were treated by operation with the surgical observations as well. Sonography seemed to provide accurate information about the configuration of the intertubercular groove and the position of the biceps tendon. The technique could be used to show dislocation of the tendon in cases in which the tendon sheath is not visualized in arthrography.

The present study assessed creatine kinase (CK) activity, CK MM isoforms, and muscle soreness following an exercise regimen designed to induce skeletal muscle damage. Eight college-age subjects performed 40 maximal isometric contractions of the knee extensor muscles (10-second contraction/20-second rest). Serum samples and soreness ratings were taken prior to and 2, 6, 18, and 24 hours after the exercise. The CK MM1 and CK MM3 isoforms were determined by flatbed isoelectric focusing (IEF). In serum, the MM1 isoform (the pure gene product) is considered to be evidence of newly released CK from muscle, as upon entering the plasma, the CK MM1 is converted to MM2 and then MM3. A significant increase in serum CK activity was found at 6 hours and remained elevated at 24 hours. CK MM1 increased significantly at 2 hours, peaked at 6 hours, then approached baseline.
Soreness
did not increase significantly until 18 hours. Analysis of CK isoforms in serum can provide an earlier indicator of skeletal muscle damage than total CK or perception of soreness and may be useful in tracking the time course of skeletal muscle damage and repair.

Selective management of 473 patients with stab wounds limited to the posterior abdomen was reviewed. This group was composed of predominantly young, healthy men. Laporotomy was based primarily on clinical findings.
Tenderness
, not localized to the area of injury, or absent or rare bowel sounds best identified patients with serious injuries. Omental protrusion was frequently associated with significant organ injury. Peritoneal lavage and local wound exploration were used infrequently. All patients with fatal injuries were operated on or died within four hours of admission. Diagnosis was delayed in five serious injuries: one diaphragmatic, three retroperitoneal colon perforations and one duodenal injury, all of which were identified and treated successfully in the initial hospital admission without any complications. Seventy-six percent of the patients never required surgery. Sixteen percent of all patients had significant organ injury, and six percent had "nonessential" laporotomy. Overall morbidity was 12 percent and mortality was 1.1 percent. The colon, liver, diaphragm, and kidneys were the most common organs injured. Thus, clinical assessment alone is a reliable means of selectively managing patients with posterior abdominal stab wounds.

Scale and the State-Trait Anxiety Inventory. Blood samples were obtained from the infants just before and 60 minutes after feeding for determination of glucose and cortisol. Formula consumption was also noted. Multivariate analysis of variance demonstrated that maternal state-trait anxiety was related to infant glucose, cortisol, and formula consumption. In particular, mild maternal prefeed state anxiety was associated with lower infant cortisol difference scores, and extremely low maternal prefeed state anxiety was associated with higher infant cortisol difference scores. Mild maternal feed state anxiety, in turn, was associated with greater infant formula consumption, and extremely low maternal feed state anxiety was associated with lower formula consumption. Pearson correlations indicated a significant direct relationship between infant prefeed cortisol and postfeed glucose, but not between infant formula consumption and postfeed glucose.

Interrelationships among concentrations and maturation of intramuscular collagen, serum concentration of hydroxyproline and testosterone and meat tenderness were determined in growing bulls and steers. Sixty-four Charolais X Angus bulls were assigned to sex treatment groups (intact or castrate) and slaughter groups (9, 12, 15 or 18 mo of age). Animals were bled at 30-min intervals via intrajugular catheters between 0600 and 1400 beginning 48 h before slaughter. Serum concentrations of testosterone were determined in each sample from bulls and from four samples from steers; serum hydroxyproline was determined in the last sample from both sexes. Testosterone mean values for the collection period were calculated. Samples of the longissimus, semitendinosus and infraspinatus muscles secured within 45 min postmortem were analyzed for intramuscular collagen concentration, percent soluble collagen and collagen thermal shrinkage temperature.

of loin steaks was determined by Warner-Bratzler shear test. Serum concentrations of hydroxyproline and testosterone were higher (P less than .01) in bulls than steers. Age effects were noted for both hydroxyproline (P less than .01) and testosterone (P less than .06). Total intramuscular collagen was greater (P less than .01) in bulls than steers and was different (P less than .01) among muscles, but the muscle differences were not uniform over all ages (P less than .05). Percent soluble collagen declined (P less than .01) with age and was different (P less than .01) among muscles. Aneurysms arising from the extracranial carotid arteries are uncommon lesions. Clinically, they may present as lateral masses in the superior cervical area, as medial masses in the tonsillar fossa or parapharyngeal region, or as a mass palpable in both areas.
Tenderness
or pain may or may not be present. Pulsation is usually detectable. Embolic neurological episodes are a common event leading to discovery of these lesions. Etiologies include trauma, atherosclerosis, fibromuscular dysplasia, infection, and congenital predisposing factors. We report the CT appearance of this condition in three patients. In all three patients (four aneurysms) the CT findings were characteristic. Thus far, we have discovered only one account in the English literature that included the CT demonstration of an extracranial carotid aneurysm.
